I think it depends on the patient/person. As a science-oriented person myself (retired engineer), I appreciate as much data as I can get, and Dr. Boz promotes measuring values as a means to directing your practice. But yes, Dr. Westman’s approach is perhaps easier to comply with for folks who don’t really feel like they are helped by knowing the science and taking appropriate measurements, adjusting, re-measuring, etc.

I agree with Dr. Westman. . Protein stops hunger. What does fat stop? Fat stops sugar cravings. Dr. Atkins said if you crave sugar to eat fat. It always works. Have you noticed when you get a sugar craving its for something fatty like donuts, ice cream, cookies? It's because the body is used to getting fat via sugar. So eating fat say like a piece of hard cheese will stop sugar cravings, If I have enough protein, I don't get hungry

I keep it simple. Control carbs,(30g or less a day) prioritize protein, and don’t fear the fat which comes with the protein. Raise your heart rate at least 4 days a week. I’m down 45lbs. First time under 200lbs in 15 years.

Thanks both this was a great debate. I think Dr Westman probably nailed it in the first few min, it could be a case of self selection bias. I initially tried keto the way Dr Westman prescribes with a focus on protein, I did ok but found it very difficult to be consistent (although was in measurable ketosis about 90% of the time first thing in the morning) and I never lost more than 7lb in 8 yrs (had plenty of other benefits thought). I then tried what Dr Boz suggested with a focus on fat and found that it was much easier to be consistent, that I feel much better energy wise and lost 7lb in a week while eating twice as much. Does that mean that Dr Boz is better, for me yes, but my husband really doesn't do well high fat, so for him Dr Westman's approach is better. I think we find our way to the person who's approach works best for us.

Same here. I initially started keto, then went carnivore because plants still didn't like me, having digestive issues. Almost a year now and got stuck twice. Fasting didn't work, more walking didn't work, the only thing that worked was adding more butter. That's so strange... it's the least expected thing that worked. And i'm not the "steak and butter gal" lean and young kind of guy, I was almost double my size when I started this.

I agree with both to a certain degree. Once you’re in ketosis and if you have stored fat there’s no reason to take in lots of extra butter or EVOO. But, you also don’t need to count protein macros if you’re eating enough fatty meat to get satiated. Your hormones will guide your appetite after a few weeks to a month of being in fat burning mode. Extra protein intake can and sure break down to sugar, so if you aren’t eating fatty cuts of meat it’s entirely possible to overeat protein and it’ll break down to sugar through gloconeogenesis and what’s not needed will be stored as fat. Fat, on the other hand, does not break down to sugar. So, if you’re just eating animal protein/fat let your hunger be the guide, ands if you’re not losing weight maybe you need to cut back a little on protein. If you don’t have enough energy and staying hungry eat more fat.

I definitely agree with Dr. Westman that protein has to come first. It's fairly well agreed upon that people should eat at least in protein 0.7g/lb or 1.5g/kg of lean mass per day. I haven't seen anyone (not saying it isn't out there, just that I haven't seen it) saying that we should aim to consume at least a certain amount of fat per day. For fat all I hear is "eat enough so you are satiated but not so much that you get loose stools." If someone is carrying excess fat on their body they can eat much leaner and use their own fat stores rather than someone who already is lean who needs dietary fat for energy.

IF your hormones will work. Some need to go high fat to get disregulated hormones "unstuck". It can be one "lever" to get to your goals.

I agree with Doctor Westman because as we get older, we produce less complete amino acids, and we can only find that in red meat. However, I break my fast daily with fat.

I have been on Mounjaro for over two years and love it because it stopped all the food noise in my head, but it took over 18 months to start losing weight. I didn’t start losing weight until I consistently kept my glucose in the 70 thru 99 range on the daily. I sometimes go between 100 - 110 but it’s rare. I do this by eating low carb. I’m not hungry and quickly lost over 40 pounds. I’m still wondering if it’s the low glucose levels or the low carbs. I’m not always in ketosis. I’ve been wearing a continuous glucose monitor for over 2 1/2 years and have really studied what affects my glucose level. Now that I have learned (for my body at least) that if I keep glucose between 70 - 99; that I will lose weight. That has been extremely helpful. I feel I have the key to my success. You really can’t keep your glucose level consistently low without going low carb, but as I mentioned I’m not always in ketosis and if I want a piece of cake once or twice a month I eat it and I find it very easy to keep on track.

55:00 discussion of glp1 injections. Also practicing Internal Med and Obesity Medicine I prescribe them as a tool for as you have said for their appetite regulation and anti food craving and addiction effect. As you said it’s more powerful to have the diet alongside the medication but teaching the diet is hard and takes time and practice and nowadays people want and doctors want the simpler option. Great discussion linking it to addictions

As he says in the first few min, self selection bias is at work. For you a focus on protein works best, for many including my husband that is also true. For me (and many others), that didn't work in terms of weightless. I got plenty of other benefits over 8 yrs but not weightless. It's only when I focused on high fat (literally eating 1/2 stick a day) that I started to lose weight and feel like I had a decent amount of energy. Both approaches work, neither works for everyone.

I'm going to start focusing on fat more for calories and see how that goes, also going to spread out the protein to 3 meals instead of just 2. I'm not really a fan of eating butter but I'm sure i'll grow to like it. Gotta find what works for you.

I love these discussions. But after fussing with GKI, pricking my fingers to test BG and Ketones and obsessing over macros, it was driving me crazy. I am obsessive by nature And the diet mechanics were driving me into a high stress mode. I now say: I eat no carbs, no processed foods, try to stay off alcohol,( my last hurdle). I prioritise protein. Eat when hungry. Deliberately fast as much as I can. Yes, I have still 20 pounds to lose, but I'm not counting. I lost 40 pounds, the rest needs to go as part of my forever way of life. I'm not on a diet.

What an enjoyable conversation! I follow both and watched this "debate" play out over several videos. I personally call it just nuanced perspectives because both are phenomenal. Both have contributed to my own journey of healing ! Reversed every chronic condition , lost 62 lbs , no meds , and I am pursuing SMHP accreditation myself to add to my mental health counseling practice

This was a great dialogue, I like how Dr Westman makes things simple as a 64 year old female I have realized tonns of buuter do not work for me I tried her method and I have gained more than 5 kg my clothes are very tight in less than 2months I will keep and prioritize protein never bother about ketones because I have my energy Dr Westman's course is so reasonable for people like us in South Africa I cannot imagine paying R40k for a 3 week video course its insane that is a year fee in University here in SA Dr Westman Science and experience speaks for itself Thank you both you were amazing it all appreciated ÄDAPT YOUR LIFE

I like both these doctors and my prospective is do the keto plan that works for your body. IMO there is no right and wrong here and both these doctor’s approach will achieve weight loss. My body loves the unlimited protein but doesn’t like too much fat. If I consume too much of even healthy fats, especially in one meal, my stomach gets so upset. Even the smallest amount of MCT oil bothers my stomach. I also start to gain weight so high protein, moderate fats works for my hypothyroidism/menopausal body. Maybe you can use a little of both doctor’s advice to get the right plan for “your” body. God bless both these doctors. They are saving lives everyday! 👍❤️

I think that both of these doctors would agree to this: Try one approach, then adjust it based on your individual results. Neither doctor lives in your body, so only you can know if you’re feeling better and making reasonable progress.

When I got stuck, twice now in almost a year, I've tried different things, like more walking, fasting and so on. The only thing that worked was adding more butter. It was the least expected thing to do but it worked. I started with almost double my normal weight, and not yough either, so not the "steak and butter gal" type of person.

I recently add more meat and fats to my diet and notice my weight started to go down again. Also notice I am pushing more ketones.

Ketone strips aren’t necessary at all, if you’re eating super low carb and not taking in sugar you’re in ketosis, except for short periods when you take in protein, which is fine. In the long run (think lifestyle instead of diet), eating highly nutritious meats and eggs (and low carb/low oxalate vegetables if you feel the need or need more choices) is the key to a healthy metabolism.

I lean more with Westman on this debate. Protein is priority, especially for metabolically damaged people. We have plenty of body fat to use. No need to stuff our mouth with globs of fat. Chasing ketones is not something we do. It can become an obsession. Ketosis happens naturally drifting in and out of it.

Depends if your goal is to lose fat or build protein. Everyone is an individual with different starting points and end goals. I.e. if you want to lose fat, don't keep topping up your fat stores.

The macro percentages for a ketogenic diet were not just pulled out of a hat. People that are new to this generally learn the ratios early on. 75% fat, 20% protein, 5% carbs. It is a lot of fat. They start loosing weight and become healthier. Some do not go that route and go straight carnivore, not ever learning basics and then begin seeing things like high blood sugar, etc. Do some research. There are plenty to books and information on the subject. Learning and measuring macros at first, IMHO, is important to learn the basics.

I thought the exchange was quite professional. Enjoyed it. I have learned a lot from both even though Dr. Westman has been my primary influencer. That said, I'm curious about Dr. Boz's work on improving insulin resistance work. I don't really need to lose more weight. But me and that A1c are going to have a fight. That sucker needs to be below 5. I was at 5.2, but caring for my elderly Parkinson's dad made it hit 5.6. Cortisol is a bastard.

I Agree with Dr. Boz... Fat is the key to AVOIDING.....THE.....HUNGER. Fat magically Kills Hunger. Failure is assured if GNAWING HUNGER is Hounding the patient. No debate here....sufficient fat is KEY to success.

Adequate protein is essential, yes, but I have to be careful how much and how often I eat protein. The only way for me to balance my hormones and my blood sugar (and be in ketosis) is to start my day with fat and end my day with fat. I only eat protein during the afternoon - usually 1 pound or so depending on the day. I eat browned butter bites for breakfast and evening snacks. For lunch, usually a ribeye or lamb chops. If I eat too much protein and eat it too often throughout the day, it causes gluconeogenesis, which spikes my blood sugar and kicks me out of ketosis - and it instigates more food cravings! Focusing on fat for morning and evening and limiting my protein to nothing more than absolute essential amounts (don’t over eat it and don’t eat it all day long), helps keep my diet and hormones and blood sugar under control. That controls my cravings too!

Awesome! The basic principles of the prescription strength Ketogenic diet are available to all. However Some folks need the involvement of frequent measuring to keep the motivation high.

Dr Westman's methods work for the majority of people but for those with severe insulin resistance may need to use Dr Boz methods to increase their ketones and decrease their glucose. I think that both agree that eventually patients can't be eaten a huge amount of fat, but it is extremely useful to do a high fat, moderate protein intermittent fasting diet at the beginning to finally reduce insulin levels. Then fat consumption can be reduced a bit since it becomes easier for the body to get into ketosis. Dr Boz has a lot of data. What another leading doctor in the metabolic space Dr Tro is suggesting that doctors just publish an audit of their patient records to show how effective their interventions are. That's what Dr Unwin in the UK has been doing for years but he was helped by a researcher to help him set up the tables and charts that he needed to use to compile the results

I've been diagnosed with microvascular ischemic disease Apparently early stages I've been on Carnivore diet for the past 3months My knee arthritis has improved I've lost a lot of the weight I wanted to, but should I stay on this diet?

I am on a 80 mg Statin can I quit cold Turkey ? (the medicine) I'm having lots of side effects my Doctor refuses to take me off I would like to wing myself off how can I do that? I am on a ketovore life style 1month in now ..

For the first time, I understand now why I suddenly stopped making ketones (or at least they stopped showing up on my urine strips). Thank you! I was feeling discouraged. Edit: Clarifying that around 6:35, the more excess body fat you have, the easier it is to get into ketosis (if I’m understanding this correctly). And by the way, I got a blood ketonemeter, which is clearly showing nutritional ketosis (0.6 mmol/L) despite my urine test strip showing nothing. Guess I’m getting fat-adapted! 😊

I love both of these doctors, and I practice a low carb lifestyle myself. The question is: what’s the impact on thyroid health long term for some people who remain in ketosis for extended periods of time? Overall, if you’re overweight and have high blood sugars/high blood pressure, moderation is your enemy. You need to zap the carbs.

@linz716 when I eat, I make sure it's 20 grams of protein or less. I aim for around 18 grams. So for breakfast I eat 1 egg, 2 table spoons of butter ,2 slices of cheese and 2ounces of cream cheese. If I eat sometimes and I'm still hungry I add more fat. I eat when ever I want I just keep the protien low each time.

I consume 60 grams of butter and 60 grams of cheese with my breakfast every morning because I found I was constantly losing weight on a low carb diet. I eat Devon butter with sea salt crystals and almost zero carb cheese. I weigh myself every day with a body mass scale which also measures cardiovascular health and I have a score almost nine years younger than my age and a pulse wave velocity that is always optimal and better than ninety per cent of users of my age. I am nearly 69 years old and although I weigh myself at the same time every day my weight is up and down I have heard that is because of more or less water, I am not frail but I do feel less well with too much protein.

@@leticianedilskyj7060 I think you should research this yourself and find out if it can help you I am not receiving anything to promote these things, I also have a cheap smart watch which tells me that my blood sugar spikes three times a day even though I only eat twice a day but it also says I have a good heart range variability index which supports the evidence from the scale.

I've been doing keto, under 25 g's of carbs now for about 5 weeks and my fasting gc has went from around 100 to 115 on average and that's after 16-18 hours of fasting. Staying the course but I'm a little confused at the moment.

My question for you both is : I have SIADH with low Na with no found cause. So how do I safely be in ketosis and keep my Na level up. I do already take sodium chloride 1 gram daily. I am trying to loose 30 lbs Thank you ❤

Excess Proteins break down into sugar so there's that..........I know if I eat more than 10 carbs i'm not losing any weight so 20 carbs would have me in a continuous stall, everyone is different and it's good to hear all sides of the arguments but in the end you have to find what works for you!

Boz has it wrong about the Ancel Keys Minnesota starvation experiment; the subjects were Not fasting. The subjects were eating a low-cal diet of 1,500 daily calories.

I like the “meet people where they are” from Dr Westman. I like that the approach is simple and inexpensive in comparison to Dr Boz’ approach. And what has turned me off from Boz is that she pushes her products. Maybe she’s trying to offer top tier products but they are expensive and it does communicate that unless a person is in her “keto standard” that the ketogenic diet is not worth staying or helpful. Not going to support that.

Can Dr. Westman comment on the Recent ORFAN study in the UK.(Ref LANCET article "Inflammatory risk and cardiovascular events in patients without obstructive coronary artery disease: the ORFAN multicentre, longitudinal cohort study")?. I appreciate that this does not relate to this video ,however, it appears to be a much more accurate predictor of risk than CCTA, calcium score. This applies to both people with CAD and those without CAD (as detected by CCTA). The new study is based on detecting arterial inflammation via the status of fat surrounding Coronary arteries which is a greater predictor of risk than Calcium scores and percentage of arterial blockage.

These are the two people who have played the biggest role in my education. My take is that fat is important to get that sense of satiation among other things but there's a huge amount of calories in a small amount of fat so there is a limit. You can stay under 20 carbs and still not loose weight. I think an emphasis on fat helps in the early stages of transitioning away from carbs to get the ball rolling on becoming fat adapted.
