Do you reach the alar ligament & the transverse atlantal ligament with prolotherapy injections? Because i have them both injured, confirmed by mri.

Hi Lifepool x, We do not inject the alar ligament. Good question and we will have some more published on this topic in the future as Dr. Hauser has dedicated a lot of time to researching the cervical ligaments. The majority of the cases we see where the alar and transverse ligaments mentioned in the report, it is a finding but not the primary culprit causing a person’s symptoms. Overall, there are more important ligament structures that need to be addressed along the posterior spine, and that these are not addressed through more invasive procedures that focus primarily on the alar ligament. So, we do understand that when imaging techniques report on the alar and transverse ligaments that they gain attention, but we typically see it as only part of the bigger picture versus the primary cause, because for most patients with complex cervical instability issues, their case usually goes beyond the alar and transverse atlantal ligaments by themselves. @@mapex311 Yes, you're correct it's much more invasive to reach those ligaments. We have found over 28 years that it is not necessary for most cases and focusing on the posterior ligaments is the primary determinant of successful outcomes.

Did you inject the alar ligament through the mouth? I have read that the alar ligament can’t be reached through a posterior injection. If you guys did the posterior ligaments, which ligaments helped treat these symptoms? Also, what is he best way to diagnose alar ligament injury, is it through MRI or the DMX? I am having severe cervical instability causing face pain and occipital neuralgia and trying to learn more about what I can do to get a more specific diagnosis.

Hi Champions of Odin, We do not inject the alar ligament- you are correct about it not being treated posteriorly. The point is in cases like hers is that the alar ligament is not actually the primary issue. We do look at the cervical spine in multiple views on Digital Motion X-ray, as it shows joint instability and ligament laxity issues better than MRI. However, alar ligament laxity is also indicative of other ligaments along the posterior neck also being lax which can be easily treated safely in our offices. Giving a potential explanation for how posterior ligament disfunction impacts anterior ligaments, and why only treating the posterior ligaments helps to solve the overall condition.

@@mehdikhemiri1418 I was diagnosed with instability by dr sunil j Patel at MUSC south carolina. There are other doctors faniliar with this problem who can get you the correct diagnosis. Dr Bolognese Dr. Frasier Henderson and there are more. Get a stand up flexion extension mri asap and find a neuro familiar with clivo axial angle, or basian axial interval measurement. They can confirm your issue and maybe even get it covered by insurance. I am suffering with you brother and pray you find a way through this

Its amazing u can go to dr and they say they dont think anything is wrong. She is so good at explaining it. From what i know prolotherapy is over $1000 and multiple treatment and not covered by insurance. Thank you for helping her im glad someone got relief

Can these cervical ligaments be tightened only with prolotherapy? What about exercises? I have clicking and grinding in my c7 and have had 5 lots of dextrose prolotherapy here in uk with only minor improvement

Hi cambio123, Good question. Yes, Prolotherapy is the treatment for tightening ligaments. Exercise/PT would be for muscles. They can both help each other but often people do a lot of PT/exercise yet it doesn't resolve the symptoms and only holds them at bay them for a short time, or a person has to do exercises, stretches, and/or massage multiple times every day to try and manage the pain. That is one of the signs that point to an underlying ligament laxity issue. We also find that various bracing or curve correction is helpful as well for improving outcomes with Prolotherapy, so we have a chiropractor here who specializes in that aspect and works along with our Prolotherapists to analyze a patient's curve under Digital Motion X-ray and do adjustments and individualize various wearable weight combinations to work on restoring the ideal curve. That way, we are addressing the vertebrae, muscles, and ligaments, along with monitoring nerve input and functioning as it relates to the patient's specific symptoms. Sorry that you are still experiencing grinding in the neck and other symptoms. We do not know if there is anyone in the UK doing upper cervical Prolotherapy or about how many injections are being done and what type of solutions. But one of the primary reasons why we see patients who travel here from the UK is for upper cervical Prolotherapy and a more comprehensive treatment technique overall. No neck complain here in east africa, low land of Ethiopia bcs it is the hottest place on earth. So heat radiation ,specifically infrared is detoxifying and anti inflammatory ,but in Highland, coldest place of Ethiopia, there is many pt complain cci. So the reason is iflamation due to exotocsin like vegetables oils, lectins etc..... there fore go carnivore anf live hotest place, u will see the difference within a month

If you chose to get treated, we will provide you with a detailed invoice that can be submitted to a private, non-HMO insurance company for possible reimbursement at the “out of network” rate. We cannot, however, guarantee any insurance reimbursement because most insurance company policies only offer pain medication and cortisone, or some physical therapy, for chronic pain treatments.

It's a common finding that patients are told they have (like Donna in this video) but when you dig deeper into the case, it's not exactly the primary cause of concern when you look at the other structures in the neck and things like blood flow through the neck to/from the brain, vagus nerve size, and other functions being affected by the structures related to the posterior ligaments more comprehensively.

I ask because both my chiros tell me I have CCI, most likely caused by a compromised alar (based on an x-ray showing lateral shift of c1 when I bend my head to the right.) I've been hesitant to investigate prolo/prp since alars can't be treated (aside from an experimental procedure out in Colorado.) But it sounds like there's a chance alar isn't the main culprit? Anyway, thanks for the response. I appreciate the insight. It's hard to figure out what the right move is when my Neuro and Spine/Pain Docs seem to have no interest in exploring the CCI angle. Cheers

Then they can let you know if they feel you are a candidate for what else is offered and what to expect, or if they feel that you are on course based on their experience with cases like yours. I've already sent an email a couple of days ago and I was informed that I was a good candidate. I've watched all your videos regarding CCI and I'm pretty sure I fit into the story of CREEP - I was unaware of my posture and 80% of my day involves working in front of a computer. I don't have EDS and I don't wanna undergo surgery but you guys are just so far away :'(

I have hypermobile Ehlers Danlos syndrome, c1 c2 instability and Chiari-Malformation. I have not been decompressed because I was told it would make my instability worse. I would like to avoid fusion surgery, so I would like to try prolotherapy. My question is will prolotherapy still work for me if I don't have decompression surgery for my chiari first? Thank you!

I sadly understand her h*ll. I've had a headache for 26+ years that's incredibly worse since 9/21/21 due to side effects from the stupid J&J shot. My brain is being crushed. My scalp and neck burn. My neck muscles are locked up all the time. I am going for a consultation on Tuesday with a local doctor who does PRP injects and I pray he can help.

I had 3 months of $$$ treatments and had to quit as I can't afford it anymore. :-( He told me I'd probably need 6 injection sessions but I also saw on his site that some people have needed 8-12 months at $975 each. I didn't notice any improvement after 3 sessions and lost hope. I do worry that I gave up too soon, but I'm really financially stressed right now. :-( Dr. Jonas Skardis IS a wonderful man and I feel like I let him down by quitting.

In our 25+ years of treating upper cervical cases, even when alar ligament injury is noted on a report, it is not necessarily the determining factor for a treatment that will ultimately resolve symptoms and restore function. It becomes, in many cases, just a finding. You cannot have alar ligament damage without damaging other ligaments in the posterior neck - these are much safer to access with a comprehensive technique of Prolotherapy. We talk more about this in other some other videos and research. Her case is representative of a lot of cases we see where the alar ligament injury was a finding but is not the true underlying reason for the condition. Hope that helps. @@shark61111 Good question and would need to be addressed on an individual basis. Most of the time a cervical fusion is the absolute last resort because of the long-term known side effects of adjacent segment problems that occur after the fusion is done.

Hi Lifepool x, Usually they are done between 2-8 weeks apart, depending on the solutions used and the level of instability and disability that the patient is suffering from upon the initial exam.
