Do you use your middle finger to hold the pick along with your index when you strum?

Your pick should be held between your index finger and thumb. Good question though, I made that exact mistake in the past.

Index and thumb only if possible, especially for any single note picking. When strumming all the strings it's not too much of an issue... but try and avoid it of possible!
