Or how did Inarius escape his imprisonment by Mephisto? He was tied up in chains with his eyes forced open to look at himself in a mirror for all eternity. As a perpetual reminder of his fall from disgrace. And then he just suddenly bust it out with no clear explanation. Just like the joker from batman. Or where was the Nephalem during the fourth game?
