It's well organized!! here, in my workplace, nurses like to hand over starting introduction to pt's name, dx, dnr. then past hx, how's lab data and radiology report, what med using now, ending in assessment.

Thank you for being honest! It IS a lot of information! As nurses we manage ALL aspects of pt care...we have to know what social work is doing, what PT did, what OT did, what speech did, what medications are given, what labs are drawn, the family members involved, etc!

Awesome Job! I wish nurses would give report this way. I work in acute care and I often have to ask many questions in order to get the full picture. Enjoyed this report for sure.

Ashley. I need your advice!! I always feel like I am pretty thorough, but I often find myself in report not being able to answer some of the questions the oncoming nurse has. I am still a new grad, but I feel like I need to get better at knowing the “why’s” about my patient’s admission, rather than just the “what’s”, if you know what I mean. What advice do you have on getting a better “big picture” on the patient other than reading the progress notes! Thank you 💕 love your channel xo 😘

Hey Isabel, I am a new grad as well. I am pretty sure you got better at knowing the "why" aspect by now. My preceptor always said keep their diagnosis in mind. that helps me to see the big picture. This person has COPD exacerbation and is on 2 l Oxygen now. He was on 1 L at home. So a plan for this guy could be see if we can lower it to 1 L oxygen and see if his O2 stat remains good. And keep a close eye on his SOB / skin tear / and potassium/ and wait for another X ray result. Look at the current care plan and see if it is helping the diagnosis :) If you have any other tips that you use, let me know that as well

Can you do a new report example for a complicated ICU patient? I just started out in ICU and I'm really struggling with organizing my report. I did make a report sheet that I use religiously, although I feel like there is just so much to say that I can't write all down or relay the information without stumbling through. It would be great to see what you've been doing!

thanks just started clinical & when the nurse is giving me report she talks sooo fast & i dont get half of what she says . i end up going into the chart & getting info myself . this was helpful thanks
