I see many young males in my practice being unnecessarily prescribed testosterone by online companies, often without proper workup or monitoring, which is truly concerning. The issue with testing for testosterone, the "normal range" is 300-1200. The range is extreme. This range doesn't even look at age. So 300 for a 20 year old and 300 for a 65 year old are both considered normal.

Yes because age changes the number, not the sensitivity. Meaning the range is pretty accurate for all ages. A 20 year old at 220 and an 80 year old at 220 are both hypogonadal. However what's more important is symptomology in general. You might have one guy (regardless of age) at 290 that feels fine and has no symptoms because his sensitivity is higher than normal. You can also have guys at 350, who are having all the symptoms because thier receptor density or sensitivity are abnormally low. That's part of why the range is so broad to begin with, since a lot of guys with "low normal" are usually highly sensitive, and guys with "high normal" levels are less sensitive, since guys with 400 and 900 are basically indiscernible from each other.

I don't see how you're supposed to build data on this and determine what is actually normal, when doctors are refusing to do testosterone tests and no-one is gathering the data on a population level and your doctor hasn't got your previous checks in your medical records so they can say, 'Look, your T was 500 at 19, 485 at 30, 470 at 45 and this reduction isn't due to a hormonal problem, but because this chart matches up perfectly with your muscle mass loss which I also have the data for.' I really don't think doctors get big data or what it's for yet. There's a company that's edging in on using heart rate to accurately pin down your blood glucose which would democratise continuous blood glucose monitors since it'll be a one time purchase. They can only do that with huge amounts of data, which Dr Mike has said before he's opposed to gathering because the general public can't interpret it. Well... neither can scientists if they aren't allowed to gather it.

@@taylorhillard4868 usually people get ordered a hormone panel through their primary care physician and they ask for symptomology and interpret current T levels in the context of patient history. If T is borderline low, there are lifestyle interventions that can nearly double the testosterone, especially in younger men. Having more sensitive and specific tests and adjusting the ranges for age might be a good thing, but the problem that ‘some people who would benefit from TRT are being missed because of these rigid and outdated reference values’ is just not existent, because even if the T is low - lifestyle first, medications later. And not that many people maximize their T FOR SURE because majority of them are (a third of the male population-and that’s a conservative estimate) sedentary and don’t sleep enough. Drugs are cool but they don’t cure the underlying issue of WHY the T was relatively low. Sorry if it sounds like i made a couple of strawmen here, my point wasn’t really orthogonal to yours but kinda adjacent 😅

The problem is that these sort of "normal range"s are based on medical affects without considering life style affects. A doctor might tell a patient they are "healthy" then they go home and kill themselves, the problem is this seperation.

Had an issue with pelvic floor dysfunction a few years back. Painful/burning urination, sex etc. Did pelvic floor therapy and it was a life saver. Never knew it was a thing but I am so thankful for it. Thank you Dr. Mike and Dr. Malik for this interview. Hope it helps a lot of guys that went through or are going through the same thing I did.

He comes from a very different world and different perspective. I would just love to hear a conversation in the West about his work, like "The Burnout Society", or his recent book the "Crisis of Narrative". I think speaks English, maybe just German and Korean, but yeah the world needs to know about actual smart alternatives from other places than the US.

A friend once told me that if you are grinding at your work in your 20's to 40's, not doing low dose HRT is sub optimal. Assuming you had the cash from said grinding, getting a low dose HRT would essentially be spending hitpoints(fraction of your life currency for non gamers) to create a barrier that is worth 3-5x the worth of your life spent. The problem is that high test makes you want to do EVEN MORE and burn yourself out, so if you don't have ultra high restraint, the high test will end up costing more of your life through increased risk taking and an actual hunger for inflicting self damage to speed up progress being greater than the added health currency to spend.

Hey Dr Mike! Was just wondering if you would ever consider having Dr John Campbell on for an interview. It would be very interesting, as, if you have heard of him and followed him, he began on one side of the aisle regarding COVID and had since moved to the other. Of course, there is more nuance to it, and I still really find his videos interesting, giving me another perspective on things that I don't necessarily agree with at all, as he has made it his personal and professional mantra to "Follow the evidence, wherever it leads". I understand things grow ever more polarizing, so I just wanted to suggest that perhaps it would be most helpful now to refocus on the realistic nuance in health and science specifically as well as applied globally. Thanks for always providing interesting and informative content through your podcasts and videos!

Sitting in waiting rooms with mostly older men and feeling really odd, I always wondered why there are no 'male specific' doctors like there are for women (=ob/gyn) and why don't we normalize visiting one - again like we do for women? For both physical health and sexual health?

Yo, it's not just about NNN. the most overtime is done in 11th month of the year .but about the ED alot of vitamin D zinc, and nitro maybe shrinkage and bruising from overusage plays a factor
I've tried a lot of supplements and although I know this is anecdotal experience, Tongkat Ali really felt like it made a noticeable difference to my performance in the gym. I've never felt stronger or had better pumps than when I was using Tongkat Ali.

For many years I was really into weightlifting and light bodybuilding and many trainers offered you testosterone pills like it was candy, inside the gyms. It was crazy, like a blackmarket but at plain sight.

you should react to more painful basketball injuries, you barely scratched the surface with the last one, react to Haywards injury, Kevin wares, Paul George's 2012 USA injury, Shaun Livingston's etc.

Hey Dr. Mike, I just found out about a pediatrician Dr. Bob Sears and I feel like he is a source of vaccine hesitancy. He has been around for awhile but think it would be informative if you could get him on the podcast or a video about some of his claims. Keely up the great content!
