Knowing Stark he'd have found a way to implement a multitude of solutions. Stark in comics has also mentioned the "redundancies" in his suit. I've only seen it being a problem for him a handful of times. Once in the animated Avengers movie for example. Yes, it does happen but it's extremely rare. It has only killed him once in his entire time as a character that I know of.
