Thank you for posting this. I believe I have this problem and have contacted the doctor that did my meniscus surgery about 4 months ago. Waiting to hear back as this is very painful for me. Some days, I can hardly walk and pressing behind my knee is very painful! I’d rather have the needle and have it aspirated and move on with summer and life!

Great Video!! I am a 72 year old disabled Army Vietnam War veteran who fell down last month on both knees on a concrete sidewalk. I had 7 ER visits at the local VA hospital. During those visits I was hospitalized 3 times with 3 major surgeries on my both legs. Both my legs from the knees down were swelled up terribly big and I was in bad pain. I had the 3rd of the 3 surgeries two weeks ago at the VA hospital by the Orthopedic surgeon. Now, my left knee is swollen, painful, and buckles when standing up. The original Orthopedic diagnosis of my leg problem starting at the knee was a Baker cyst on both my knees. My Baker cyst on the back of my left knee is big. I am afraid of the cyst bursting and affecting my lower leg and foot causing the 3 surgeries I had to have. I wander why my Orthopedic surgeon could not do a procedure like you have in your video. It was never mentioned by my Orthopedic surgeon. I am going to have appointment with him tomorrow and I will show him your great video. I definately need that procedure. I have had one of those for the past 6 months or so. It is completely painless in my case, and I can't even feel it, and I have been able to fully keep doing my exercise routine (so I do try to go easier as I used to). I am thinking about removing it, but I have so much fear of needles (to the point where I can faint on site), and knowing that the cyst can cole back anyday anyway, makes me push back the procedure a lot ( if I ever do it).

Hi guys - great video. My personal experience of having this procedure done (twice) is that unless the root cause is established, the (synovial) fluid will keep coming back and repeating this procedure even once a month isn’t a solution. In my case the fluid returned within a week. I’ve already had arthroscopic surgery to repair my medical meniscus, my surgeon said and I quote “the bakers cyst will heal itself ....”. The last MRI taken a few weeks ago shows the cyst increased in size by 2cm (time between the two MRIs is 3 months) the surgeon now says there’s a significant weakness in the knee which is causing the fluid to collect at the back of the knee. 5 MRI scans later and only now he says there’s a weakness! I’m thinking another procedure to correct the weakness .....

Do you by any chance have an update on this? I've just now been diagnosed with this and what they told me is very simple issue seems to me a much bigger one given the fact, you cannot just drain the fluid as it comes right back.

Update, please? Mine are going crazy... One has popped up above the front of my left knee. It's real hard and interferes with movement. I am on my feet on concrete 10 .5 hours/day. Thinking maybe draining it myself.

Thanks for the comment. Sometimes Dr. Girmann will perform a baker's cyst fenestration if the fluid reaccumulates. You may want to look into that option.

10Q 4 d vid. I developed a ruptured Baker's cyst as well and i can tolerate pain but when the tail of it balloon into the gastronecmius, it was pain +++ ergo on extension and contraction. Knee and four fingers up and down turned red and swollen. And the A & E senile could not diagnose it and wanted to send me home. Hah, the left leg was non weight bearing, pain only on flexion, not on palpation. Finally then they muddled through. It hurts and takes a while to recover. Wonder if there are genetic links...mum has the same problem in the same knee and her father also. My siblings thankfully do not and i am praying for my children it ends with me. The sudden lost of the ability to walk and the wait for MRI exclusion of shadow sinister... It was fun.

For all those looking to do this- try doing 3x5 nordic hamstring curls every other day first before. I couldn’t walk- my leg wouldn’t extend - now I’m back to playing professional basketball. The cyst is still there, but it doesn’t bother me.

After having several knee injuries torn ligaments and my knee bending backwards and sideways starting 2 years ago, the baker's cyst started to get really large and I am not able to bend my left leg even at a 90° angle without extreme pain and I have been experiencing extreme pins and needles all over my legs and feet along with stabbing pains and if I do sit down and my knee will then go for my chair after 10 minutes my left leg is completely numb with pins and needles in my feet. For the last 45 years I have walked six miles every single night so I was also at the gym several hours every other day off and on for the last 40 years. But I was completely cringing I'm watching both of these needles go in so for me I would need to have a general anesthetic tired of having this in office procedure which I am scheduling hopefully this coming week

We're not sure about that, Scott. They may not be trained in ultrasound medicine. Sorry to hear about it. This is something that we do regularly and it is often quite helpful for patients!

I am in Australia and have a Bakers cyst, the thought of this procedure scared me, but after viewing it, doesn't look that bad, I assume the procedure would be the same in Australia. I have been advised and read, that they can be reoccurring, but wow, they cause discomfort and lost sleep

Thanks for the comment. You may want to be sure that the provider is able to perform the procedure with ultrasound guidance. Yes, Baker's cysts can be quite intrusive for many.

Here’s my question… if you get it drained why does it come back? With other cysts there’s usually a sack. If you don’t remove the save won’t it come back like other cysts?

A Baker's cyst is an outpouching of the knee joint capsule, so the term "cyst" is actually a misnomer. The fluid in a Baker's cyst is synovial fluid that your knee will continue to produce no matter how many times it is drained.

They'll num it first and then inject the fluids out. Ask them if you want, to show you the fluids. You be ok. You may need someone to drive you home.

Can this be done after tkr? Or is the risk of infection too great? I am 11 week post op. And having trouble with flexión. My surgeon says cyst will go away by itself but so far has not. Had it before surgery and assumed it would be drained during surgery but no. Thanks!

The docs all say that they’ll go away by themselves and my doctor went on to say that they were painless. I beg to differ,mine hurt like heck and I wanted it out but he wouldn’t. Finally, I had a knee replacement in my 60’s on that leg and it’s gone now. Not sure if it’ll ever come back, but now my little 5 year g’daughter has one on the same left leg but hers doesn’t seem to hurt her so far…so she says. She’ll be seeing an orthopaedic surgeon and we’ll see what he says…

Unfortunately my mother 3000 miles away went to ER 2 times within 48 hours. 1st time no clue 2nd er trip. They said you have a large cyst probably surgically removed. Here’s 2 pain pills She can’t walk. Can’t stand. Very painful. I hope at her age of 80 they just drain it.

Siddhant Khurana my mom had a VERY problematic baker cyst back in 2012 that bothered her for over a year. She got it drained and hasn’t had any problems since. For her the numbing hurt more than the draining.

Perhaps you could say that. A Baker's cyst is essentially an overflow of joint fluid. When the joint accumulates too much synovial fluid, it pushes out the back of the capsular region.

Infection is always a concern but if performed with proper sterile technique, the risk should be very minimal.

I have a huge Baker Cyst at the back of my left knee. Had it drained multiple times but it returns. It very painful and even without weight bearing the pain from the pressure is extremely great. We know the Cyst will continue to return without repair of the mechanical issue. I had a Lateral meniscus tear where about 50% of the cartilage was removed and since that orthroscopy surgery was performed the cyst appeared and continues to return. The 64 thousand dollar question is how can the mechanical issue be repaired? Is there such a procedure as artificial cartilage replacement or is there any other method available to correct what appears to be a chronic unrepairable issue. Perhaps science has now come up with new technology that can finally rectify this problem. My issue began in the 1990's when there was no such thing as a Fix for this. I'm hoping that by now something can be done to help me. I would be so so grateful for any help or knowledge you or anyone can share with me so I can find permanent releif. Thank you kindly. I forgot to mention that the cyst must have ruptured as I feel pressure from the fluid in my calf and above the knee in my thigh. So this has spread throughout my entire leg. The pain and discomfort is 10/10. I'm hoping that something can be done for all of us.

You will find out that doctors are not to smart , 90 % have no clue on curing anything, only give medication's...find a real doctor or specialist who thinks for himself and trys other things to help...

For a runner, after the draining how soon can the pt go back to running? Can you tell me why some orthos would recommend PT? I am seeing an ortho soon for a bakers cyst and I hope he does not recommend PT bc i really do not have time nor $. In my opinion I feel draining and a steroid shot would be the way for someone busy.

You have an underlying injury causing the excess fluid - aspiration is a short term solution. The fluid will return regardless how long you leave it before running again. You need an MRI scan to establish underlying cause.

I had this issue about 15 years ago. Got surgery, needed some recovery time but it was done once and never returned. Not sure if just draining is good enoug because I read it returnes for many people.

Thanks for the question! Sometimes a single aspiration does in f t resolve the baker’s cyst fluid collection. In some cases it can recur and may require an additional draining. Your provider may choose to administer anti inflammatory medication into the knee at the same time to reduce overall inflammation and possibly decrease the change of recurrence. Hopefully that was helpful!

Thanks for the comment. We would definitely not recommend that! There are many important structures (nerves, blood vessels, etc) in close proximity to the baker’s cyst. Additionally, there can be increased risk of infection and other complications. If, however, you see a local provider who regularly performs this procedure, they should be able to take care of it in no time! Best!

I used heat therapy to (1) relax the valve in the fill tube and (2) increase the pressure in the cyst. It was just a layman mechanic thinking but it worked and never returned.
