Sunny fights Omori at the end because Sunny doesn’t want to repress what happened anymore. Omori guilt trips Sunny so that he can take control over Sunny’s thoughts once more.

If not mild spoilers: In Blackspace on the OMORI route you fight stranger one last time before basil supposedly commits suicide in the real world. Then you fully suppress sunny and bring Dream world Basil back up to headspace with you. Then Basil is constrained to the picnic basket like Mari so he doesn't die again, basically allowing OMORI to stay in headspace as much as he pleases. Basically Stranger is trying to get you to confront the truth and LEAVE headspace, and Dream world basil wants you to live the fake life with him and STAY in headspace.
