Thank you for this video. Personally, aftwr following Dr. Vanita Rattan and switching my actives to night time, I have seen a dramatic difference in my skin. I have been using sunscreen daily since I was 24. I am now 47 with no lines or wrinkles but struggled with sensitivity after sun exposure. I switched my actives including Vitamin c to night time two months ago, voila, total clearing of my melasma and sensitivity.

Hey!!! Yesss glad you’re seeing improvement! Definitely a smart choice since you have melasma. Melasma can be quite the player hater and is VERY tricky when it comes to sun exposure. You can be fine one day and then not fine the next 🤦🏾♀️ 😩

I am definitely going to switch to night time for my Actives. I started two weeks ago wearing Alpha arbutin +Ascorbic acid, and Azelaic acid during the day, then sunscreen on top. I am not sure if its working for my melasma. Can Azelaic acid be used with retinol? Also can azaleic acid be used with mandelic acid. Right now am trying to use these 3 ingredients separately two days a week for each

Wow, I am so glad I found this video. I promise you that you two ladies as well as Dr. V always provide great information and teach people of color a wealth of infomattion on how to treat their skin. I am a 47 year old woman that dose not have break outs anymore but I have a great deal of old hyperpigmentation. I have been trying for years to get rid of them and now I have hope.

It’s so sad that dr Alexis had to say ‘most of the products are not formulated with people of color in mind’ it’s so true! People of color are often left out of medical school books (including that for dermatology) so the next generation of doctors and cosmetic chemists don’t even get a change to learn about the specific concerns of people of color. Eliminating fragrance from my skincare has been a total game changer! I first learned about the potentially irritating effects of fragrance from Dr. Dray and decided to try out her fragrance-free suggestion for my *stubborn AF* PIH. I have tried hydroquinone on my bikini line and found it effective. I have been using a vitamin C serum with hyaluronic acid, ALWAYS using sunscreen and/or wide brimmed hats. I think results are seen best with proper sun protection! I wish Dr. Stephens had harped on that. Still a great show and discussion. Thanks! Also, HOW ARE YOU 39?!?! I am 29 and I was watching this thinking you were my age. That melanin really works wonders. ✊🏿

I had dark marks under my arms from years of shaving, using antiperspirant, and getting ingrown hairs. I stopped using antiperspirant 10 years ago, and recently got laser hair removal. I started using Oxy Maximum Strength pads and The Ordinary’s 10% Mandelic acid under my arms daily and it has significantly lightened my pits. I make sure to use shea butter afterwards to keep my skin from being dry. Some days I don’t even wear deodorant because the acids prohibit bacteria growth. I can even do a strenuous workout and not smell. Added to that I am also a vegetarian and have a relatively good diet.

My biggest regret was being off of Retin-A for monthsss and when I finally started using it again, I was using .1% EVERY night. My texture came back, face looks darker than my body, etc. I learned my lesson big time!

She didn’t just say most of these were not formulated for skin of color. That part was referring to the *percentage of actives* in formulation and why she mentioned starting with the lower percentage and seeing how your skin reacts before moving up to a higher percentage. And doing patch tests before starting a new product. This helps mitigate possible irritation or a reaction that can cause hyperpigmentation.

It’d be great if derms could clarify how long ‘long term’ use is for HQ (even with the breaks) is it 4 months, 6? Are you still using it for the under eye area? How do you plan to maintain the great results once you go off it. The non-HQ ingredients cant be applied in this area right?

Thank you so much for this information. I have been experiencing burning following application of Azelaic Acid. Mind you this has been after I have put on Trentinoin Cream 0.05%. I will try to separate use as suggested and see if that avoids irritation. Also, I substituted The Inkey List Niacinamide for The Ordinary brand and that irritation stopped. This was based on Dr. Stephens' recommendation of this brand. I hope this helps someone else. I was beginning to think that I could not uses these products.

I wonder what Dr. Stephens opinion is about layering some of these ingredients. I seen a video by Dr. Hani that recommended layering a mandelic acid toner, alpha arbutin serum, kojic acid serum (1-2%), ascorbic acid serum, azelaic acid (10-20%) to reduce hyperpigmentation and to introduce a serum each week to figure out if you truly need to use all 4 or just a combination of 2-3 of them. She also stated that results would been seen in 16-18 weeks.

I fail to understand advising people to alternate/separate vitamin a derivatives and azelaic acid. They don’t oxidize, degrade, weaken or otherwise contradict each other when used concurrently Azelaic acid is anti-inflammatory and unlike strong retinoids it has a high tolerability profile especially at OTC concentrations And pH can’t be the issue because dermatology providers actually compound Tretinoin/Retinoic acid and Azelaic Acid together in the same vehicle cream in order to improve the acne treatment &/or hyperpigmentation treatment being prescribed to the patient It would of helped if she explained her rational

Listen, I got prescribed Tretenoin ... I can't rmbr the % but that was way way way to strong. At first I thought it wasn't working ... Who knows but based on everything I've been learning just think it was too strong. Now I've been using the CeraVe retinol with licorice root

I thought I replied to this but my comment is gone 😩 Anyway, I was saying tretinoin can be very strong especially when you first get on it and you have to be very careful. If you use too much of it and use it too often in the beginning (typically the first 1-3 mos but varies per person) it can make the skin extremely sensitive. It’s basically making the skin turnover faster so you have to be gentle. You also shouldn’t use a lot of other actives in the beginning stage. I had to use the products every other day so Tretinoin Monday La Roche Posay Tuesday...etc. So that was 3 - 4 times a week. I found it - It was Tretinoin 0.025. But my face was clearly not having that much. Tbh I know I should have gone back to the dermatologist, and I was going to and I had tonnes of questions for her but I didnt and I am now treating myself.

Hey! Wow your entire state doesn’t have a Black derm? There are non-black derms who have experience treating black skin. You need to seek one out who has plenty of receipts that show they know what they’re doing when it comes to treating Black skin. If you can’t even find that, then traveling might be an option for you.

I often read online that these exfoliators mandelic acid, vitamine c, alpha arbutin etc are melanin inhibitors. So they stop your melanin production. Im really cautious of bleach effect or something that takes my color away, but i would loce my face to glow again. It looks so ashy and dry (post accutane effect) Help??

Those actives are not strong enough to take your color away. They help reduce excess melanin production-the excess melanin is what causes hyperpigmentation.

Hey Danielle, This was Awsome! I luv vitamin c and i like to use it during the day. I also use glycolic acid at night (its a toner) with my night time routine. I try to keep my steps simple and consistent.

Glycolic Acid is an AHA and exfoliatior. Even if it says toner on the bottle. Do your own research.

You can-but just go easy if you’ve never used a retinol before. You may want to start out by alternating usage (use AA on some nights and R on others). Dr Stephens has a video on alpha arbutin I think-that may not be the title but I know she has videos on actives and a few hyperpigmentation videos. Check those out!

In general, no. But if you personally notice any reactions to using it, then yes. A lot of products have a mix of acids in the formula-that doesn’t mean they’re bad. It just means to patch test before you try anything new. The point is to mitigate any irritation that can lead to issues that can lead to hyperpigmentation.
