Struggling with aging in your under-eye area? With so many different anti-aging surgical operations available, it can be difficult to decide what is best for your aging concerns. For someone in their mid 30's who isn't seeing the fat loss or excess skin but still has dark circles, how can I tell the issue and the solution? I stare at a computer screen all day for work and I have been told by my peers (aka non-experts) it may be thin skin where you can see the veins underneath, but again, they are not experts so I don't know...

I wish the before and after pics had the same amount of makeup. It’s hard to tell what’s going on when the after pics have the brows and lashes done, but the before pics do not

Thank you for the comment. It is impossible to dictate if a patient wears makeup or not when she shares her after photos. But, makeup does not change facial shape or hide laxity, otherwise no one would ever have surgery.

I've shown your videos to my "Facial Team" As a Hairstylist, I see so many women with work done. My goal is to always look like me...just better, fresher. My Nurse Injector has told me "NO" to more of things I've discussed with her. Communication and trust between you and your Facial Team is essential!

Dr Karam this has been the most informative video I've watched in a very long time and I watch videos on anti-aging procedures all the time. I often notice that celebrities start looking tired in their 40s but they still look beautiful so I couldn't quite understand what this tired look was due to, I only knew it had something to do with the eye area. Subtle changes due to ageing are hard to pin point so it's great to have you explain what to look out for

As someone who has photos from childhood with lower eyelid bags I appreciate this video so much. I never got to have that moment of just youthful eyes people have asked if I’m tired since elementary school 😮‍💨 I’m now 31 and will be getting surgery in the next 2 years I hope now that I know it can be fixed and I can afford it.

I live 20 min from his office!!!! He is so expensive, but I’m seriously considering shelling out for a consultation. I’m turning 50 next month and I have had a brow lift and a blepharoplasty. With a different surgeon. But now my lower face needs some help!

Agree completely. I thought all of them looked 10+ years younger at least… some even more. Very cool! I’ve been wanting to to have my eyes done as I’m 63 and have fatty pads that have created puffy under eye bags and pretty sure it’s genetic bc my mom developed the same in her later years and so has my sister. I’m just excited that there’s a solution as I’ve stopped spending $ on trying to reduce the puffiness. It’s nearly impossible without having a skilled plastic surgeon. I’m signing up for a consult soon as I live within an hour of San Diego. Lucky Me!!! Thank you Dr. Karam for this excellent video!🙏🏻❤️🙏🏻❤️

Everything you say always makes sense. I’m coming to you when I’m ready to do my eyes (if it’s not cost prohibitive like Dr Kao). You do beautiful work ❤. I just had a facelift done in Korea. I wanted my eyes done too but they said they couldn’t because my eyelid skin was too thick. Not sure what that means but I think it was a blessing in disguise. Because now I can have you work on my eyes.

I did bleparoplasty operation just a few months ago, but I seen already my puffy eyes (bag eyes) again, I am 52 yo and don't understand why it came back so fast, is it connected with my hypertension or ...? What should I do then? My doctor said I have to massage or compress with cold water 🙏

I am so impressed by this information. I so need this. I am a person that has always struggled with under eye bags. I went with my 40 yr old sister when she got her transcongective blepho.. and the Dr refused to take me in . I was 27 back in 1995 . Said I was too young for that procedure. I am now 54 and all that you have to offer is what I have wished for since then. Thank you for educating me. I so want to go to San Diego to get this done but I am way out here in Texas. And I feel I could never trust anyone else but you who takes time to study the person's unique features. God bless you and your practice. I will be curious to get a consult sometime soon. This was awesome! I went for a consult recently and he only wanted to do a lower bleph fat removal. I clearly have mostly hollowness issues so this seemed weird to me. Great explanation! Do you also do surgical brow lift in concert with these lower lid procedures?

I'm 46, and I always follow your advice because I want to look like myself as I age. I also inform my own audience to be careful about any procedure and do their research because it can look very abnormal if you start messing with your face too much! I really appreciate how you break down the aging process, making the mechanisms to aging easy to understand. It's important to know these aspects when making a decision about something so critical.

@@sabrinarenataditaranto4960, I know you are asking him, but I love to make sure I use most of my actives on my eyes as well, I use Snail Mucin undereye cream over that, and at night I apply tretinoin to my undereyes. I am fighting the battle, but all of these have helped tremendously!

@@realist8052, surgery is permanent for this, but that doesn't mean the skin won't start sagging again or that the fat will not leak into the area again. I'm not a plastic surgeon, though, so I don't know for certain how long that takes. I'm assuming it's like a face-lift, which lasts around 10-15 years before sagging starts again.

I came across your channel recently and have learnt so much from your videos. I'll search for a video that you've done on the upper eyelid too. I applied foundation instead of eye shadow a few times last year, and it seems to have sucked up most of the fluid (whatever makes the upper eye lid look plump) from my upper eyelids. The upper eyelids also got puffy in the mornings as well. Now most of the morning puffiness is gone leaving me with excess skin and sunken eyes. Not sure what to do. Ps. I like the new wall design 👍

The more I dive into aesthetic medicine, the more interested I am in becoming an aesthetic nurse! Would love it if you did a summary of how to be an ethical nurse injector and use filler responsibly (how filler can help and hurt, what nurses should do in their scope of practice, and when they should refer out to a physician vs. trying to overcorrect with filler).

The more I learn on this subject, the more I feel I don't want any of these interventions. Just wear sunscreen and apply good quality creams (retinoids, Vitamin C and E, Glycolic Acid--The Ordinary is a great, affordable line) eat well, drink lots of water, don't smoke or over drink alcohol, exercise, get enough sleep, do yoga and don't stress. Learn how to apply make-up to suit your skins needs and age gracefully!

Amazing advice, as per usual, Dr Karam! Very wise, I am in my late 30s now and thinking of possible fat transfer, but for now using a bit of fillers and botox. I still look very well, but perhaps will consider the transfer next in a few years!

Thank you for taking the time to explain so much on plastic surgery . Just 2 weeks ago I had a face lift and took all your advice on board and I am very happy with the results. I am now looking at just adding either the fat transfer or a very little filler under the eyes and this video has helped me decide what I need and how best to discuss it with my surgeon . Under eyes, do not do fat grafting. It's very unpredictable. Lumpy and bumpy. Only surgically removed

Thank you so much for explaining all these choices... you have likely saved me from bad experiences with your advice (such as laser resurfacing or other energy treatments!). Suggestion - the before pictures are without make up; but the after pictures are with makeup. I know that makeup can hide all types of 'sins'. May I suggest that the pictures (especially the after) are shown without makeup? Or include three pictures (before, after, and with makeup)

My mother had surgery on her eyes over 25 years ago. After seeing her results I decided I never wanted a plastic surgeon to touch my eyes. The eyes are so important to the "essence" of a person, as you say. After her surgery, her eyes made her look different. Not to mention, her eyelids sort of drooped and didn't touch her eyeball anymore. I am open to the idea again after watch this video though. @@melinaPesakit is not the same technique. In the same way once upon a time keyhole surgery didn’t exist and if people needed to have their appendix removed, it was one large incision. Now they do it keyhole and it’s practically invisible. Same thing goes for this field of medicine. They develop and learn new processes all the time.

What I like so much about Dr. Karam is that he’s able to put out these informative videos on a regular basis, while running a successful practice - AND still not look exhausted. It worries me when plastic surgeons with popular KZbin channels look like they don’t get the rest they need. My reasons are selfish. Basically, it would be impossible for me to have any confidence in a doctor who has both a surgical practice and a booming KZbin channel, if it means it’s taking a destructive toll on his sleep. I would not want anyone like that going near my face with a knife. Another reason I respect Dr. Karam’s channel as much as I do is because he’s not doing silly skits that (while being entertaining) cause me to wonder whether he would rather be a performer in the entertainment industry than a plastic surgeon. It’s reassuring to see how seriously and confidently Dr. Karam takes his craft. Finally, the before and after photos are “amazing” (such an overused word, and yet it applies perfectly here). Now, if only he wasn’t on the opposite coast from me. It doesn’t seem like the high altitudes on a plane would bode well in the healing process. 😂

@@recoveringbaptist2023 I am 46 years old and for the past 26 years, I have tried all there is to try. No home remedies work. No creams, oils or any other concoctions work long term. By the way, I like Dr. Rajani and I watch his videos all the time. I do ice my eyes often and it does lessen the dark circles for a couple of hours. I am more so looking for a more permanent fix rather than having to do a home remedy twice a day.

Another awesome video!! So much great detail. I definitely want and need a lower eyelid surgery. My concern is how does a person know if it’s the fat pad bulging forward or if it’s “fluid” from allergies? I’m guessing I might have both. So maybe in my case I would get improvement but not as much as others might? In my book it would still be a win, I think.

I also have allergies , my lower eyelids have always been puffy. I think that's a good question for a surgeon in the initial consult. Nothing I have done has ever changed the puffiness, Only on a couple of times the puffiness seemed to be gone( for a day or two) but I had folds of skin next to the cheek bones. I’d be scared to get filler under my eye because I see that the skin under the eye is very delicate and might not be firm enough to hold the filler and end up looking more like bloating, which I hate more than hollowing. Some things are better left alone and for me this is one of them.

I wouldn't get filler under the eyes. Fillers tends to migrate, getting lumpy, and is extremely hard, if not impossible to dissolve. Get PRF injections.

I just want to say (and I haven’t watched the video yet, I’m about to) I’m in my mid forties and for quite a few years I had dark hollows under my eyes that looked like bags. I tried so hard to get rid of them and hide them. I used to have a glass of wine every night. On a separate note, I decided to stop having wine every night and wow…..my skin , especially under my eyes looked younger again! I discovered alcohol was dehydrating my skin and working against me all that time. Now I just have one or two glasses every couple of weeks, if I choose. And my skin is much more hydrated.

I had a plastic surgeon ‘grant’ my request for filler under my eyes when they appeared hollow only to be fully disappointed when there was the appearance of bubbles under the skin , following a follow up COVID vaccination . Apparently they was a known reaction . It looked terrible and I ended up having another dermatologist have to correct it by dissolving some of the filler with some injection and making it look smoother . What’s crazy is I was never really consulted on what procedure was appropriate for me , she just followed my request .. but I’m no plastic surgeon 😂

Radio frequency has helped my eyelids as has Microcurrent & LED therapy!! In combo with VitC , Glycolic acid serum, Grapeseed Oil and Safflower oil have helped immensely with either tightening, discoloration and volume. Results can be achieved without injectables or Surgery. Oralift has also had significant results on people's facial shape, hollowing lifting and volume, so there are other types of therapies that just take frequent consistency.

Dr. Karam, PLEASE tell why you don’t like lower lid fat transposition. Several prominent oculoplastic surgeons have told me they believe fat transposition is the gold standard because the fat being redraped into the hollow retains its own native blood supply.

I had my first undereye filler at 52 , half syringe on each side, it was life changing and now I do it every 3 years but I'm worried about fillers there because it does last a long time. I'm trying to look into something more natural, but I live in Canada and don't see many options, if anyone in Toronto knows a good practitioner I would appreciate it.

I did my tear troughs twice, and never needed to do them again. I recently had a “Micro-tox” treatment and ended up with full-on festoons for over a week because the filler grabbed all the saline. It was a really hard week emotionally, but it’s since settled in and looks great. Don’t redo your filler unless you have to, and be careful of any Botox in conjunction.

Thank you doctor Karam for great educational videos! 🙏🏻 Regarding CO2-laser under the eyes, it seems to me that many surgeons prefer this to a skin pinch, and that some even do both for optimal results. Their claim is that the laser will not thin the skin but rather make it thicker and better. When you say skin will become thinner and more fragile after laser resurfacing, do you then see this as a permanent result, or is it a temporary issue? And could laser tightening also result in changing the eye shape? I struggle with vertical lines right under my eyes that go down my cheeks and therefore can not be successfully treated with Botox.. At the age og 46 I dont have much other ageing issues yet. Have seen different surgeons, but most are sceptical towards the skin pinch saying it will make the eye rounder in shape, and that its very easy to take too much off so the lower rim shows.. Makes me very confused as to which procedure to choose, as everyone has got great before and after pictures to show…

I did a PRF treatment under my eyes and saw absolutely zero improvement. In fact, the serum ended up sagging over the days before it absorbed, and I think in the end it made things worse. They then said I'd need to get it done 2 or 3 times more (at $800/per) to see the best effects. I declined and kissed my $800 goodbye.

My father is in his 70s, has excess skin below his eyes, and is on heart medication after having a heart attack about 10 years ago. He had a consult to get the eyes fixed and they said he would have to come off the heart medicine for a week. My mom is right: she said get something like this fixed sooner while you are healthy. He opted against the surgery because of the risks associated with stopping the heart medicine.

thank you doctor. I have a lower blepharoplasty scheduled in May with a high skilled Oculoplastic surgeon. His recommendation is to do transconjunctival lower blepharoplasty along with a C02 laser. The reason for the C02 laser is to to tighten the under eyes and smooth out the texture and lines under the eyes. In listening to your video and hearing about the skin being so fragile and thin (my under eyes skin is definitely thin), I am now very concerned about doing the C02 laser and having complications down the road. Based on this dr's recommendation, do you think the C02 laser is still necessary? Your thoughts and options would be very much appreciated. thank you, Roseanne

I had the same procedure done and had great results for 1 - 2 yrs then the hollowness set in. Fillers don’t last and are lumpy in the area so do what this dr suggests and be sure to get a fat transfer as hollowed eyes look terrible and are impossible to treat. Would ask surgeons for a video testimony of a patient 4 years out to see real results. Photos can easily be doctored.

I definitely think fat transfer would be the best choice over fillers but this would only be if you get hollowness and I don't believe everyone experiences this. I also think that there are only so many doctors doing fat transfers because its a very specific kind of surgery and it really requires someone with expertise and someone who does it regularly.

I took your advice and went to famous Beverly Hills Occuplastic surgeon to get a fat transfer because you were too busy and had a year waiting list. Now I have a dent in my leg where he took the fat and my eyes look horrible. He put 2.5 cc under each eye. I have tons of pressure now under each eye and bulging eyes totally puffy 18 months later. I am so depressed. I saw 26 surgeons now and can't get help. Nobody wants to take on someone else's mistake. I finally found one surgeon willing to take it out, but he wants to charge me over $50,000 just for lower eyes and he will not take insurance. I am totally screwed suffering emotional damage as well as lid margin roll and exposure caused by the excess fat stretching things out for almost 2 years. I already spent over $10,000 on consultation fees and can't find anyone to help me in this area. This is the worst nightmare. Do you have any eye surgeons you recommend?

I also got fat transfer for my face and have a horrible dent at the side of my left thigh. Also it puffed the corners of my top eyelids and I can feel a bobble under my eyelid. Sometimes it seems better to age naturally as most surgeons, and beauty salons do not think properly, just do what they are taught and that one size fits all. the majority are not up to any standard.
