the reason so many people in the bleach verse get cut like that is because that is the way samurai usually cut with katanas, as they were known to cut between the shoulder and the neck in a diagonal fashion.

Remember that Aizen was literally ruling over Soul society and hueco mundo, he killed central 46 and used his perfect hypnosis to impersonate them, he ordered Rukia to go to kurakura town at the same time as the ruler of hueco mundo he send the Hollow that attacked Ichigo's house and force him to become a substitute Shinigami. As for why Aizen did all of this you'll see.....
