I was at my annual CPAP review with my sleep specialist. They told me my pressure is set for a variable from 12 to 20, with an actualized average pressure of 17.2. They said that during my last overnight sleep study I was having an average of 93 breathing “events” (stoppages) per hour and none were recorded in the last year (per the CPAP data). Unlike many people, I don’t mind my CPAP. I have bad sealing masks, louder machines, poor fitting head harnesses, etc… I have gotten to the point that I can’t sleep without my CPAP. The difference it has made is truly amazing. No more feeling dead tired all the time, fighting to stay awake watching TV or shaking my head violently to keep myself awake/aware while driving.

I have the ResMed 11 and I swear by the EPR setting. For me it makes breathing so much more effortless. I completely agree with EVERYTHING else you have stated in all your videos (I watch them all). Thanks for the help. You have made a positive difference in my life.

Well, you have to raise your minimum pressure if you're using EPR but the delay is still there :(

I’ve been told off today for messing with my settings, I have my machine given to me by the NHS and they said if I continue going into the clinical menu they will take the CPAP off me! Couldn’t believe they’d rather me not be interested and continue with therapy settings that weren’t working until my next appointment that are every 6 months! I’m looking into buying my own kit now and going it alone

Sounds like you're over in the UK yeah? You can probably pick up a Lowenstein Prisma 20A for around $600 pound and it's worth every penny (although not compatible with SleepHQ just yet) A few of our SleepHQ pro members have been picking up the AirSense10 card to cloud for $499USD (400 pound) and freight forwarding it to the UK so that's an option also. Cpap is a medical device….if a dr is in charge of your care and then you change settings, then something bad happens…a patient could still try to hold the physician for the problem. …and you would rather take advice from youtube cpap techs or whatever they are? changing the clinical settings is like prescribing your own medicine. i understand your frustration, but i wouldnt mess with settings that you may not know all the ins and outs of.

I started CPAP therapy 2 weeks ago. I haven't been able to fall asleep with it since i started. Until last night when i turned off auto CPAP. Auto mode have me the feeling of gasping for air and it stressed me out. The inspiration was too slow at the beginning and I felt like I wasn't getting enough air and taking long drawn out inspirations. Turning off auto allowed me to fall asleep for the first time with it. I have the resmed 11.

You know what, thankyou for the info, I actually turned my epr to 3 and for some reason I have been getting worse sleep lately, pressure is pressure no matter what, this actually make sense losing the desired pressure wont be back for awhile, last night scared me, because I was choking on my dream and when I woke up from it, I was actually choking, I will change my settings today to what it was Update june 18. It was indeed the resmed EPR, I stopped choking, on the 1st day of turning it off, I was still waking up like every 2 hrs, 7 hrs total, following day it was gone, manage to sleep for more than 8 hrs, felt so alive and energetic, once again thankyou for this info, you're a godsend, 3 days later I am loving my cpap again. mine is Airsense 10 elite.

sometimes notbeing able to breathe out fully during the therapy can cause problems as well, bilevel is better then EPR as it changes the pressure faster.

For a long time I was trying to transition from EPR 3 toward going straight CPAP. I started to shift it to EPR 2. A couple of nights I tried EPR of 1, and slept horribly. So kept it at 2 for a long time. I got used to exhaling against the increased pressure, but never fully used to it, and definitely not to the point of being able to comfortably switch to EPR 1. Since getting sick last week, I switched it back to EPR 3 after having been at 2 for a long time, and it's so much more comfortable. I think my sleep might be a bit better, as well.

Confused! You just compared 2 different machines with different settings, not different settings of the same machine. If the ultimate goal is to achieve better AHI, you just made the case for using EPR as a good thing. What's the downside? Love the channel. Lots of good advice/help. Imho the settings the doctor gave me for my cpap was too low. During my test, it says I need 9 but today I need 14 when using auto mode. It also depends on masks. With Phillips I only need 12, but resmed mask I need 14. Your weight also determines the pressure

Been consistently having an AHI of 11-14 and sure enough, it was on, gonna try this to see if it improves things. Just found this channel after a couple months of middling success on my ResMed 11 (was having upwards of 60 events an hour beforehand, so it is at least helping) and enjoying the videos a lot, hopefully this channel will be a help!

Thank goodness it hasn’t really bothered me to my knowledge. I reduced my pressure a few years ago on my own which helped with mask leaks and discomfort. I just got a resmed 11 with EPR. I may play around with adjusting it and see what my results are. Thanks for sharing all this

I have a ResMed AiirSense 11 and when I was struggling to get used to the machine (my first CPAP) the EPR was the key to success. I can't speak for anyone else but I've had zero problems with the setting (I use EPR level 3) and in any case I'd have no problems making a little tweak to the primary pressure if that were required. Not sure what the fuss is about but I guess everyone's experience is different.

Resmed 11. Poor communication between tech and Drs causing delays and problems. This interesting 🤔 Hhhm, after pressure of 11-12, got rid of obstruction but CA went sky high. So, they increased pressure up to 16 to reverse conversion to central which only caused an air pressure wound from pillow mask. Then decreased to 7 and with different and same mask, Obstruction and CA went sky high, couldn’t breath and exhausted. Have changed to a range with continued problems with both. Turned off pressure relief- dramatic decrease in both- obstruction almost controlled, CA still too high, but much better… THank YoU! May still need another sleep study and different machine, but some relief for months b4 this can happen better than the bs that was happening…

Not sure if you saw his video about soft vs standard setting. He said with soft you can get a lot of issues with swings in narrow breathing. So you may want to try Standard instead of Soft. As well as upping your max pressure level 2 or 3. So like from 12 to 14 or 15. Hope you were able to figure it out.

So I'm confused.. I have a ResMed 10, and my AHI was around 4 with EPR off. My provider turned it on (full time at level 3) and my AHI immediately (and constantly) dropped to 1.4 on average. Does this mean it's worked for me? Or are my AHI readings false? Cheers!

Thank Chris for very interesting comparison. I believe that clinical blind study with say 100 voluntary patients between a) different C-PAP brands b) various settings would yield so much new data re. brands and EPR's importance to treatment results. Maybe even that golden key to Pandora's box? I am from Finland and we cannot tweak our ResMed or other CPAP machines (yeah, really true!) due our 🤬😤 government healthcare legislation. Patients need to contact to "authorized" public healthcare (long response time) or private specialist apnea clinic (€€€) in order to ask them to tweak or do any changes (and they charge you roughly 50€ for every change). This is absurd law is resulting many people to drop out from CPAP treatment Anyway, your channel and videos are a lifeline to me and other Finns to learn more on treatment! So huge thank you for all your great information

If the machines are set on the same pressure, isn't part of the "secret sauce" of the softPAP that it in fact goes to a higher pressure? I use my airsense10 only because the "auto on" seems to work much better than my Dreamstation, perhaps I should give that another go. Thanks for the video.

@@Healinghands829 Great question and millions of cpap users would also like to know that. In 1993 they eradicated my pineal gland along with the tumor. They thought it did nothing, turns out it regulates your sleep and deep sleep by emitting melatonin. I'm 44 and before I turned 40 I had 3 strokes. Sleep depravation was most like the culprit. I'm a 30 year brain tumor survivor now. 30 years of exhaustion. It just hits me and I have to sleep. Part of why I'm on disability along with brain damage and many other things.

I totally agree! Count me confused too. After the first 70% of the video I thought "oh dear, I'd better turn off EPR on my AirSense 10". Then at the end, presumably to support his case, Nick shows just one example....but EPR helped!!!

Nicko have you had a go on the Lowenstein? I've got a Prisma 20A on a 4 week trial - I found that the Softpap gave the inhalation part of the cycle a slight delay and a fair old whoosh of air. I turned it off last night and found the machine a bit more comfortable but still not as comfortable as the Fisher & Paykel (which had everything turned on). I've had the max pressure on the Prisma lowered after consultation with the shop (from 20 to 12 I think). I had a trial on the Resmed Autoset 10 and had the pressure relief turned on - if I knew then what I know now I would have turned it off. I thought that machine was a bit agro and now I know why!

Pandoras box my friend. It really depends on what is the cause of the airflow resistance. Is it up in your nasal cavity or perhaps the nasopharynx or even lower perhaps. 'Flow limitation' is a broad categorisation. Could be a massive booga in your nose

I tried the EPR off and woke up half way through the night with pain in my abdomen. Without the EPR I somehow eat the air and get abdominal pain. Honestly felt so bad I thought I was dying. So I keep mine on now. 😂

Well, that explains a lot. I thought I was going nuts when I first noticed a different airflow with EPR turned on versus off... Sometimes it feels like a damn hurricane in my mask. Fortunately I'm only EPR 1 and was planning to wean off it over time anyways.

yeah that's correct. An increase in EPR will result in a large influx of airflow into your mask during inhalation. This can cause a mask and mouth leaks and also some other problems. It also increases noise levels. Good luck

That's interesting. My sleep WatchPat One gave me an AHI of 40. My very first night with resmed 11 airsense auto gave me an AHI of 1.56 (my lowest AHI to date [new user, currently only one week]), I turned on EPR and my AHI the very second night was 3.19. I suspected it was EPR set to 3 so I lowered it to 2. Since then it has been 2.06 and lower. I think I'll try EPR 1 and even turning it off.

Thnx for explaination - I've noticed the airflow characteristics you've mentioned with my AIRSENSE 10 with ERP set to 3. How do the other EPR settings on Resmed compare? I remember there are 3 levels of EPR available. Might one of the other EPR settings improve apnea performance? FYI my AIRSENSE 10 is in CPAP mode (no auto adjustments other than EPR).

Very timely, have my new resmed 11 for less than a week, coming from a dreamstation 1, two nights ago I turned off EPR and my AHI went down almost by 1 per hour. Thanks for some explanation. Thinking I'll just leave mine off. My mental model was keeping my airways blown up like a ballon was likely a good idea, it would help with stoping things from shifting around and causing obstructions.

Weird... My original Phillips (yeh that one) did a great job. After i got the Phillips refun, i purchased air sense 10. Never been as good, same setting. Did another hospital sleep center test.. Nope nothing changed.. Still setting of 12. Either AS10 not as good or sleep center missed something.

Hey mate, this is a really comment and something i should have mentioned in the vid. So most clinics should have pressure relief off when titrating a patients pressure levels but from my experience in the lab it's a bit of a mixed bag. Let's say you visit the lab, they have EPR off and they prescribe you 12cm of pressure, you go home, switch EPR on 3 and now your only getting 10cm of pressure and your now far from the optimal pressure. I think this is a really valid point and it's disappointing that ResMed doesn't provide information and training to inform patients that EPR is not just a comfort setting. It's a very powerful clinical setting that can reduce apnea control in a big way. Cheers brother. Nicko

My mask has never stayed on consistently, my sleep doctor has dismissed it everytime i've mentioned it so i've been just going on my own research for 4 months now. I have a resmed and i've tried changing mask sizes, humidity level, air temp, mask tighteness, a large amount of different sleep supplements and recently started messing with my own pressures because when I called and asked if they could tweak it for me i was told i would have to wait for my next appointment with at the time was 3 months away. Havent tried EPR settings i wonder if it dropping off pressure is making me rip it off in my sleep. Hope this works tired of not sleeping at night

I turned my EPR to a one and turned my machine to just CPAP use. To see if maybe that’ll help. I can’t stand the face masks so I’m gonna try the nasal one. My pressure is automatic so it’s between 4-6 since I turned that off to CPAP I’m hoping it helps. Every time I used it in the past I couldn’t sleep at all. So I’m hoping with this I do. Do you think that’ll help? My EPR was a 3 My pressure was automatic 4-6 My humidity level was 6 Tube was 72 degrees I turned the humidity off too.

Started Apap therapy 2 weeks ago. I'm a healthy, gym going 40yo dude, with no health issue. BUT, my throat closes on its own the second i fall asleep! I've been dead tired for years! Now, i feel like i'm 20 again! Cpap for the win.. health is more important than comfort

Hi, I am new to CPAP therapy. I started about two weeks ago, and I have been having such a difficult time with it. I have the Airsense 11 and have my pressure set on 5 to 15. I would wake up with air in my stomach, even my ribs hurt! It was bad. And also I would feel as if I am suffocating in the middle of the night I wake up gasping for air. And the pressure feels as it is too high. I keep messing with the settings. Trying to find my comfort level. My sleep has been worse since starting therapy.. but I am going to try some of your tips tonight and hope this helps! I’ve changed my pressure and will turn off EPR.

@@Healinghands829 I am no longer using the CPAP machine, the doctor didn’t do much, he just changed the pressure settings to the lowest but it didn’t work. I’m going to try the mouth piece, you put in your teeth. Hopefully that helps.

@@BlondieHappyGuy I was using the pillows and the full face mask(airfit F20). The pillows was more comfortable but I definitely took in a lot more air, I believe because I would open my mouth.

I have an awful time at the beginning of inhalation. It seems I have to wait for the machine to start blowing air, Mandel I'm coving trying to bread in. Horrible experience not sure what to do.

The secret "Sauce" of the pressure increasing on the end of expiration is usually what causes people to not tolerate CPAP at the beginning, P flex is similar and is VERY comfortable... While i agree with every single thing your saying here.... graphs dont show comfort..... and yea they are less comfortable... sure my AHI might be slighty increased.... but thats RARE....and if compliance improves then thats whats most important...

Good morning Nick, watching the video, I have had the EPR turned on, but the EPR was at 3. I adjusted EPR from 3 down to 2... OMG... the difference was incredible,,,, no leaks and I actually slept better all night. I was thinking of lowering the EPR down to 1 from 2, just to see if the results are better. BTW, I have a ResMed 10..... thank you so much for your video's, I have learned so much but still continue to learn.

Does the sudden surge in flow for Resmed EPR also affect their bilevel machines? Or is it more subtle? I tried EPR 3 last night, and it felt like a fan was blowing on my face every time I took a breath. I need to switch to a bilevel and am leery of getting another Resmed for this (and other) reason.

I'm new to therapy and don't have a device that's compatible with sleep HQ. What I've noticed since turning on the EPR on my airsense 11 is that i don't wake up with my lungs full of air, feeling like I can't breath, and needing to exhale through my mouth. Needing to do this is distressing to me, because I'm a nose breather and I've never needed to open my mouth while I sleep unless I had a sinus infection. My therapy does still seem to be working - i have many nights where my AHI is less than 1 according to the myair app (prior to turning on the EPR, I frequently had an AHI of 8-16. I'm wondering if maybe I ought to have bought a BiPAP. I just did an at home sleep study (due to the probative cost of testing and therapy with the sad excuse for health insurance provided by my employer - I'm in the US by the way) and I'm questioning if I might have mixed sleep apnea rather than just OSA. Like i said, I'm a nose breather and I'm not a loud sleeper either. The pulmonologist said that women can tend to snore quieter than men, and that I'm probably still snoring it just probably sounds like heavy breathing. I think my therapy is still working, at least to some extent. My primary reason for being tested for sleep apnea is because I had secondary polycythemia (excessive red blood cell production - presumably due to being in a hypoxic state for extended periods of time). Since adding another asthma medication and starting CPAP therapy, my polycythemia has resolved! However, I would like to also reduce my risk of apnea related conditions (dementia, heart disease, obesity, etc). My questions are: (1) is it possible for ResMeds EPR to help with mixed sleep apnea or central sleep apnea even though it tends to be ineffective/increase the pressure necessary for treatment for patients with obstructive sleep apnea? And (2) how is central sleep apnea or mix sleep apnea diagnosed within a clinical setting - should I attempt to have an in clinic sleep study?

Thank you . First i work out if needs to be treated and it certainly helps if there's O2 data but you can also tell from the flow rate if it's causing sleep disruptions. If it is an issue and it's a ResMed users it's worth enabling EPR because that can often help as i showed in the video. Increasing the CPAP pressure (ipap & epap) can also help. It really is trial and error when it comes to airflow limitation as it's hard to know exactly the anatomical cause. Cheers

Does what was said about EPR also apply in APAP mode? I have had Resmed devices for about 15 years. Up until the 9 series, it was a big improvement every time. Since the 10 series, I don't feel the devices are better, they are just playing around with the design. With the 10s the modem did not work, with the 11s the modem now works but Bluetooth is an imposition, does not run. In addition, the humidifier solution is getting worse and the air purification filters are a laughing stock since the 11 device. You can just as well leave them out. I will now get myself air purification filters and cut them to the necessary size. Are the current devices from Löwenstein medical (I think that was the German company you mentioned in the video) now so good that they can be a reasonable alternative to Resmed? At that time I already needed the APAP version. Meanwhile the ratio of obstructive apneas to central apneas has turned around and I have over 75% central events. Perhaps I need a different device?

Im confused. Resmed Epr is bad as the graphs indicate,… but real results show better results with the only difference being epr on? Huh. My testing showed Aerophagia only with epr off.

Oh no😬. Great insight and thank you for sharing. I tried with the EPR off or even level 2 and couldn’t breathe with the nasal pillows. Resmed have turned me into an EPR Junkie! 😂😂

Anytime mate, thanks for sharing your experience. I also can't use my P10 without EPR on. That mask is very small and there's a lot of resistance with the mesh exhalation port so exhalation is extra hard with that one.

@@CPAPReviews even with regular cleaning with cheap vevor ultrasonic bath it clogs easily, I had to put a few small holes in the mesh with the tip of a dart and it’s been good on EPR3😉

I turned off EPR years ago. It was never comfortable for me. My AHI over the past year is 0.4 - 1.4. Turned off the "soft" setting also! I just ordered another Res Med A-10 Auto-Set Card to Cloud model. Good prices on them now. What is your "go to" settings for this machine?

I use a resmed airsense 10 i have gone back to a set pressure of 16.6 with EPR set to one. My expectation is that there is little to no adjusting pressure adjustment - Am I correct in this assumption?

The first thing that I turn off for every machine. They think they are being clever and helpful, but the C in CPAP stands for Continuous. I like to think that this means constant, not variable.

I am 57 and I do have severe sleep apnea. If I lay on my back of course it makes it worse but if I lay on my side, it does decrease the apnea. quite a bit through the night, but my pressure is so high I can never get a good seal… do you suggest a mask that with thick foam, and something that I can pull tight without the marks I’ve gone through seven different mask, trying to find the right one to sell my size head..😡😡😡🙏🙏🙏 thanking about the Rez med air fit f30-40

I am using ResMed 11 and struggling with to much pressures with it,in the past used ResMed S9 & 10 for Her. My EPR is set at 3. Should I lower EPR to 2 or 1 to see if it will drop the pressure, or maybe turn it off? My pressures with my older Resmed was staying around 7.6 to 8.4. For the first time I am really struggling with side effects.

Interesting stuff. Can you do one like this with comparing these against the same brands' bipap machines? I'm curious if the same brands end up doing better with that was well. Like would a ResMed bipap end up not going back to the inhalation pressure in time, just like with its CPAP EPR?

That's a very interesting idea! With Bilevel it's different because a lot of the time you want that epap to stay low so it helps with ventilation etc. Thanks for the awesome suggestion

I'm new to CPAP therapy nd I have an Airsense 11 Auto set. Pressures re set to 4-20. The clinician turned Epr on today to 3 full time. It sounds like I should just turn it off? I tried turning it on myself a few weeks ago because I was curious what it felt like. I noticed it felt more comfortable but I was able to snore where when epr was off I literally could not snore if I wanted.

I'm confused about the number settings of EPR. I have the ResMed 10 and while I don't see an EPR setting, I do see Pressure Relief, which is On (no numbers). Is this the same thing as EPR?

You need to access "Clinical Settings" by pressing both the click wheel and home button simultaneously for a few seconds.
