Imho the permament license revoke is too much. I mean there has been no fatal accident at all, just a lot of fear and disappointment. This due to 9/11 that, for a lot of person has not really been "an accident" so...

I think a better approach would have been to suspend their licences for maybe 3 months, have them complete additional distraction training, renew their licences and then come back to the airline and continue as normal. They recovered safely and nobody was hurt.

Yeah, as far as I know there isn't a way to auto land the plane or remotely set it to auto land. jets get sent out to stop planes from killing thousands by dropping it in the middle of nowhere. If you end up on a plane with guys and guns, fight. it's your best chance.

In ATC speak, "Company" refers to the airline a pilot is flying for. IE: An ATC may tell a pilot to line up and wait behind company on Alpha. That means that they're supposed to wait behind the plane of the same airline they are on the Alpha taxiway.

One time I was flying on an Air Force C-130 Hercules. I was sitting in the cargo compartment and was plugged into the intercom system and listening to the crew talking up front in the cockpit. They had a copy of Playboy and were looking at the pictures in the magazine. All of a sudden someone up there said something to the effect, "Hey, we overflew our last way point. We should have turned 100 miles ago." Suddenly the 130 banked over hard left and began a very sharp turn. Everyone in the back was looking at each other with a puzzled look on their faces. Except me.

Also at that time, some companies allowed one pilot to rest in the pilot's quarters, while one stayed in the cockpit. Probably the one resting overslept, while the other pilot who was suppose to be alert, fell into deep sleep in the cockpit... Other than something like that, it start's getting fishy!?

Phil Chill Actually, there is never a solo pilot for any commercial airline regardless of rest rigs. The FAA requires 2 designated persons in the flight deck at all times. Even flight atts enter the flight deck for restroom breaks to ensure no 1 person is solely in charge. It’s an FAA requirement since 911, 2001. This happened in 2009.

After watching this full episode of Terror in the Skies, the pilots were actually on their laptops discussing their work schedules in the cockpit. They didn't even know they had overflown their destination until a flight attendant called into the cockpit to ask if she should start preparing the passengers for landing.

Not excusing the pilots but 150 miles really isn't that far in a plane. If they are traveling at 500 mph that's only 18 mins and at 400 mph that's only 22 mins. I know they don't use mph for the speed of an aircraft but that's relatable to most people. Radio silence I have no idea why they didn't respond for an hour though.

78 minutes with zero contact. That’s a lot worse than 15 minutes. It’s hundreds if not thousands in wasted fuel. And what if they ran low and had to declare emergency. Also let’s not forget that now 150+ passengers are stranded two states away

After takeoff, one of the pilots wanted to take a nap and told the other to fly the plane and talk to ATC. Then the other pilot was too tired and also fell asleep... then they wake up and realize they went past their destination. Might be what happened.

Airplane pilots that "get distracted" are likely either sleeping or playing poker among the crew, or something else equally important. There's a VERY simple (and almost free) solution to this. Set an alarm. Have a computer play a random, annoying song that forces attention to return to reality by simple accident of interruption. The alarm is randomly set between 45 minutes and an hour and a quarter to sing a song to the crew, alerting them to the fact that they're in an airplane and in control. Another is fixed at a certain time before an approach course is set. It's hard being bored for a long truck drive, it's murder on attention. It must be many times harder as a pilot. It's not sleep, but daydream. Humans aren't designed for boredom. Ask a trucker.

When I was 12 I was on a southwest flight from Denver to San Diego and the pilot did a loop around the Grand Canyon this was a year before 9-11 it was the best flight I was ever on I'm pretty sure it wasn't planed

You can't fucking say you got bored and/or distracted when you have hundreds of souls in your hand. If you are flying all by yourself, with no passengers, go ahead and fly off the face of earth. I don't care if that's how you wanna go...but being a pilot comes with responsibility. One of those fucking responsibility is staring at the sky if need be. They're a human too is a weak excuse. They are human with expertise. They are pilot. They trained to land that shit where it is supposed to land. Oh I just got distracted...company problems...some bs. shit could've gone horribly wrong if they were distracted forever.

Oh man, how did the supervisors not get you for that? One time I flew a PMDG 777 to Munich from Dallas late at night and was so exhausted when I taxied into the gate that I simply slumped over as GSX was deboarding cargo and fell asleep. Woke up to find myself kicked off the network and an email from a supervisor. 1 week network ban. FML.

***** Keep in mind we're talking about a flight simulator. For Vatsim, if you're a long hauler, yes. I fly for Fedex Virtual Air Cargo and man, let me tell you, alarm clocks are a long hauler's best friend. I have a bad habit of sleeping during long flights, and the supervisors know me very well. I'm pretty convinced that they look for me flying long routes just to catch me and kick me off the network. Oh well, It's really the only way I can tolerate long hauls is if I sleep through it. Vatsim supervisors seem to think that pilots have the time to sit in front of a monitor and watch a virtual plane fly for 10 hours. Out of 10 hour flight, I'm most likely only actively at my computer for 1/5th of that time.

This is more common in our skies today than we wish. I spoke to a pilot recently from a German company who told me, these issues get even worse when flying long haul at night. Even more since the company installed a system ( i forgot the name) that automatically reports the position of the aircraft that makes radio un necessary..

Something very interesting that I saw in Japan was the manner in which the conductors of high speed trains operate. Since most of it is automated as well, the conductors are instructed to point out all safety signs they see on their train ride. This shows that the conductors are paying attention and are aware of their surroundings.

I looked up details about this. The pilots were so distracted, the only reason why they snapped out of it was because one of the flight attendants phoned the flight deck and asked why the weren’t landing yet.

That's why I always set my alarm on my watch. Need to be awake to land. Boredom sure but toss in being exhausted. Especially if you've got a quick overnight turn. Sure you have 8 or 9 hours off on paper. That doesn't mean you're sleeping. The wx. Is bad the hotel van is late. You have check in, triple S it, maybe grab something to eat, talk with your kids etc. Great it's 1AM and if I'm asleep right now I can get 4 hours. But I'm not asleep. Another night with 2 hours of sleep.
