Welcome to Psychiatry Simplified.
Have you ever pondered over the distinct disparities between amphetamine and methamphetamine and their intricate repercussions on the human body?
Key points in the video:
00:00: Exploring the Core Differences: How do amphetamine and methamphetamine bring about distinct effects within the body?
02:04: Structural Uniqueness: Discover the specific characteristic that sets methamphetamine apart from its counterpart, amphetamine.
04:07: Balancing Risk and Benefit: Understanding the link between the speed of action and bioavailability of methamphetamine and how it contrasts with the safety profile of amphetamine.
06:03: Dosage Insights: Unveiling the typical dosage ranges associated with methamphetamine usage.
07:44: Neurochemical Mysteries: Delve into the complex pharmacological impact of methamphetamine and amphetamine on the brain while deciphering why methamphetamine's allure lies in its heightened euphoria and addictive nature.
If so, brace yourself for an enthralling expedition as we explore the very essence that sets these two compounds apart.
Join me, Sanil Rege, a consultant psychiatrist, on this episode of "Psychiatry Simplified," where we unravel the multifaceted distinctions between amphetamine and methamphetamine.
Amphetamine and methamphetamine, two potent stimulants, possess intriguingly diverse chemical compositions and wield varying influences on our bodies.
As we delve into the heart of this video, we will intricately dissect five pivotal differentiators between amphetamine and methamphetamine.
From their unique chemical structures to their contrasting routes of administration, potency levels, effects on the central nervous system, and long-term health implications, we will navigate through this intricate maze to illuminate the distinct risks associated with methamphetamine usage.
Methamphetamine, in particular, has earned its reputation as a ticking time bomb due to its highly addictive nature and its potential to inflict profound physical and mental harm on its users. Its potent euphoric effects can swiftly transform into a destructive addiction, unleashing a cascade of behavioural, psychological, and physiological challenges. Our exploration will provide insight into why methamphetamine poses such a significant peril to individuals and communities alike.
Moreover, we will plunge into the dramatically dangerous territory that methamphetamine treads. From its devastating impact on brain function and cognitive faculties to its capacity to catalyse violence and criminal behaviour, this substance stands as a menacing force that demands our understanding and attention.

In my professional experience working with addicts and lived experience, psychiatrists are seeing significant progress and lower rates of relapse by prescribing dexamphetamine/ dextroamphetamine to patients with history of methamphetamine abuse/ addiction because of the similar chemical makeup. Harm reduction works.

levmetamfetamine (the spelling is correct) is the active ingredient in generic inhalers which are still available at Dollar General, Meijer, CVS Pharmacy and Walgreens. Vick's inhalers are now non-medicated. It's levo-methamphetamine, the weaker "left handed" isomer.

And then later in life Daniel Junior got Adderall back then he started doing crystal meth and I don't think he died from that I think it was the drug dealer but I will never know cuz nobody cared at the Batavia New York Police Department they said they assume he died from drugs.... that's what these drugs can do for your kids and it's not just me my friends kids we're on that kind of medicine and they started going off when they took them off at the age of 18 doing drugs I think it makes you an addict.. I already know I'm an addict but I don't do drugs and when I did the rock it was for one day and I hated myself... and unlike what they tell you it's not exactly wonderful for your sex life either if you're a guy anyway I don't know about women?? If a kid goes in and they're hyperactive or something they get them addicted to this s*** that's where it is f****** garbage excuse my French but I freaking don't like what it did to my son he's gone forever and I hate myself for not being able to save him everyday I called him you got to stop doing that you're going to die and he did.. I have one leg now I called my son Jimmy I said I haven't heard from Daniel can you go check on him while he was dead that's what these drugs can do for you and I'm thinking the Cymbalta to make me happy and the Seroquel to make me sleep and the Vicodin is in helping me either I think cannabis helps me more and gummy bears 50 mg CBD but they don't want you to know it can help people they want you on the f***** up drugs.. I'm 62 years old I drank for 20 years wasted my life away and now I have nothing really and I quit drinking a lot when I was 38 and had children.... well it keeps me from drinking to know I can drink someday but I get in trouble so I Drink Alone just like George Thorogood LOL there's a song I Drink Alone cuz I can only stand to be by myself but the reason I wrote this if it helps anyone it is good...

If you stack Amphetamines with Tropoflavin and low dose Selegiline and Memantine, there's no neurotoxicity or excitotoxicity. Also, you can cycle Amphetamine, Methamphetamine, and Ethylamphetamine with Modafinil and Adipex. And if you want to quit amphetamine or methamphetamine, use Kratom and Khat plants. Propranolol helps to not use alcohol all the time.

its seems that the addition of a methly group to the amphetamine molecule effectively doubles the potency, this is very similar to the what happens with the methylation of morphine to heroin where the addition of the methly group effectively doubles the potency of morphine.

Methamphetamine and amphetamine psychosis is a product of lack of sleep and not due to any inherent properties of the substance. This effect can be seen in patients that deal with insomnia. This should be referred to as sleep deprivation psychosis. The psychosis will subside after regular sleep cycles resume. It takes anywhere from 24 to 72 hours without sleep to start noticing reduced mental function. Cases of insomnia rarely last more than a week but insomnia caused by drug use can go on for weeks or even months with further mental deterioration. People with longer lasting insomnia that is not caused by substance use will also experience similar mental deterioration. There are likely residual effects over time in both cases. This is a case where correlation doesn't equal causation. Failure to acknowledge or study this common link is a disservice to the scientific method.
@PsychiatrySimplified
@PsychiatrySimplified 9 ай бұрын
It is due to the inherent property to release a significant level of DA , NA & Serotonin. These directly impact sleep as the sleep milieu requires at various stages a complete silencing of NA DA & 5HT. Severe insomnia is part of agitation and is a risk factor for psychosis. But Methamphetamine directly increases DA significantly which results in aberrant salience - the salience network is responsible for the genesis of psychosis. This is also the reason why a D2 antagonist treats the psychosis. One can’t simply say that psychosis is due to sleep deprivation only. We see several patients who are sleeping feasibly ok that can have relapse of psychosis post use.

@@PsychiatrySimplifiedexactly, this is also seen lot more in flakka which is like methamphetamine. Since flakka likely overloads the nervous system lot more than meth, the psychosis from flakka is almost always 30mins later with D2 dopaminergic receptors overload. Having said that, you said it right that it's never simple & whether it's flakka or meth, dose and biochemistry play huge role. In addition, it's true that stimulants especially amphetamines usage even in ww2 has insomnia & thus acute sleep deprivation is observed in various people leading to cognitive decline but this too is dependent upon stress and environmental issues. Thus no wonder that a war like situation coupled with amphetamines might lead to higher psychosis rate but this alone doesn't mean that amphetamines alone lead to psychosis yet amphetamines (especially unregulated street ones) may also trigger psychosis in compactible individuals especially those with schizophrenia/bipolar history or worse cause perception distortion syndromes.

@@PsychiatrySimplifiedi dont want to convolute things but how does that neurochemistry relate to psychosis purely bor of sleep deprivation - point is i think you and OP are correct - further and concerningly, it's prob sone rough overlapping negative feedback loops ie. drug induced psychosis as you suggest - result is inability to sleep - skeep deprivstion induced psychosis - fear response etc making it harder to sleep - SD pyschosis - take more drugs to stay awake - DI psychosis Its not about right and wrong answers - its about both are correct but partial, heres how they fit and here is more to add to the infinite picture Integral Medicine is putting these things together - Ken wilber has the underlying model and it can be apllied to all fields and then integrate those also

I don't agree with that. I am sorry but through personal experience, I have seen those people that have slept and as soon as they intake the drug they immediately start seeing and hearing things that are not there. I do agree there are those that do start having the effects once they have lost many hours of sleep. Someone without drug use can see and hear things after so many hours of no sleep. However, there are the ones that have them from intake.

I can say first hand my ADHD meds DO NOT cause me any type of euphoria, and have solved my insomnia. If you compared an oral extended release formulation of methamphetamine at an equivalent dose to oral extended release amphetamine, they would likely be very similar. As someone that has taken both on several different occasions, I can tell you that there are no noticeable distinctions between the two highs. They're identical. Crack was a short but powerful rush. Powder cocaine, depending on the quality was shorter acting and less intense. Meth, again depending on purity is intense and LONG lasting but comes with more paranoia and makes the thought of eating nauseating and leaves you feeling very unclean and contaminated. Adderall always gave the same feeling as meth. It took longer to take effect but then it was the same long lasting high that made eating and sleeping impossible. This was an awesome video! The first many times I snorted methamphetamine, it was in combination with drinking lot of alcohol, so it wasn’t really until I tried it without drinking alcohol that I fully experienced what it could do for me… Since then, I’ve taken A LOT of it! It made me calm, far less aggressive, and more capable of being "present", less clumsy, better at conversations, and also at studying. But it also often ended in taking far more I needed to achieve the benefit, so long periods without sleep, long periods where I made a living of selling it and similar substances, also meant I tended to end up at Monday night parties that had started on a Wednesday…. I also used it a lot while I was in the Army, and it’s freaking wild that I never got caught! The few times I’ve been tested, in sports and in the Army, was always when I hadn’t taken any for a while! Two of the tests in the Army I got told they wanted to test me because I seemed more aggressive, easily distracted, had problems staying on topic when talking, had failed some of my responsibilities, made mistakes I didn’t usually make, forgot things more often than usual, would interrupt/speak out of turn also when addressing people above my rank, I had gotten a couple of stupid traffic tickets by the Military Police (and they had noted my behavior was "suspicious"), and at times they felt I seemed almost erratic…. But both times, it was several weeks since I had taken anything. Long after I left the Army and around a year after I had left "the substance use environment" behind, I had an appointment with a Psychiatrist! I had already been diagnosed with PTSD when I left the Army, and Depression shortly after that! But after a few consultations, he picked a book on the shelf, told me to listen carefully and tell me if anything of what he read out loud sounded familiar - and apparently he had a book that was written about me with the title ADHD! This was in the 00's and I was in my 30's - and he started me up on Ritalin and later on he combined it with Strattera. What a difference that made - it made it much easier for me to study and get an education within the health and healthcare sector. But after years on medicine and psychoeducation, which helped a lot on some of my issues, it was like other symptoms got stronger and then I met this woman who knew a lot about ASD in adults and some of her friends had a lot of insight into this as well - and after having been asked a couple of times about how long ago I had been diagnosed with ASD and telling them that I didn’t have ASD, and on both occasions they had just looked confused at me, and one of them had asked me "are you absolutely sure?", and with the other she had just smiled and then said "you’re definitely autist"! So, I once again ended up at a Psychiatrist, for new evaluation! And it turned out that I have ASD as well - so with the BPH (I probably got from taking the Strattera), and various comorbidities, I got more than enough letters to sustain myself on Alphabet Soup in case SHTF! But my last Psychiatrist replaced my Ritalin and Strattera with Attentin, Aduvanz/Elvanse, and Intuniv! I get fairly high doses, but Blood Tests has shown that the biological half-life (or is it the plasma half-life they measure?), is pretty fast - the same goes for the Duloxetin I take as well, so I wonder if it’s due to the same mechanisms - anyone knows anything about that? I also got very high Dosis of Ritalin when I was on that - so maybe I just burn through it faster? Can this be due to the huge amount of methamphetamine I used to take when I was younger? I’m also a big bastard, Scandinavian, the Thor type you see in God of War Ragnarok, but just more blond, can any of these factors play a role in why I need higher doses for it to have the needed effect? Adderall isn’t approved in my country, but I can’t help wonder if the mix of amphetamine salts in Adderall works better than the combination I get from dexamphetamine and lisdexamphetamine - anyone know anything about that?

Thanks for sharing your interesting story. There is evidence that individuals with substance use especially Meth need higher doses of stimulants - this is mainly due to the changes in dopaminergic pathways with previous use. While IR and XR can be combined it’s important to recognise the targets being treated . Of cognition and activity are targets then before increasing doses it’s important to look at sleep and arousal - addressing this becomes important as it allows for the doses of others to remain lower and can be more effective . Something to look at.

Thanks for the comparison. I've been prescribed Amphetamines for over 30 years for a few different reasons. Although, It seems to work slightly different for me than the average person. While people claim to get "euphoria" even from Amphetamines, I do not. I only remember "feeling" anything the first few times long ago but, wouldn't call it euphoric. Being that I've only ever taken it orally might be the reason for that. At this point, the dosage I take daily is considered fairly high. I've never had Methamphetamine and never will so, I can't personally compare the two.

Could you make a video around harm reduction for methamphetamine. Because realistically speaking while these videos are concerning they don't help people like myself with quitting the drug because of the difficulty giving up the addiction. So approaching the point from a more realistic point of view it would help to know what someone using could do to counteract the oxiditive stress and harmful effects. For example taking a specific supplement or drug that may have neuroprotective effects against methamphetamine or suggesting something that could serve as an anti-oxident.

Yes I understand. Perhaps todays video release will help. Should be released in a few hours . I understand where you are coming from - the harm reduction aspects. I’m 55 and I’ve been prescribed Adderall for about 17 years now. When I was in my early 20s, I was a big meth head. It was really unhealthy. I stopped for around 15 years before I went to my psychiatrist. When I started on Adderall, I kept feeling like I wanted to fall asleep during the day. I had good concentration, better than when I wasn’t medicated. My doctor prescribed what I now see as a whopping 150 mg per day. It was predictably uncomfortable. Currently, I’m down to 60 mg per day. I still get sleepy at times, so more isn’t better. I always take it with food because I don’t enjoy the “rush“. I get more of a kick from a cup of coffee. I can concentrate without hyperfocus. yet I’m more curious about life in the world. That’s what I do it for.

@ChardeeMacdennis339
@ChardeeMacdennis339 3 ай бұрын
Hi! I wouldn’t recommend combining the two. Adderall contains amphetamine salts directly while Vyvanse uses lisdexamfetamine as a “prodrug,” which gets converted into an active form in the body before it has any effects. Adderall will start taking effect faster because it gets metabolized in the stomach. Vyvanse starts taking effect in your intestines. They are both amphetamines. Just different formulations. Vyvanse is typically “smoother” and lasts longer than adderall. I have taken both for adhd. Anyway talk to your doctor. Don’t take both at the same time because are essentially two sides of the same coin. I can’t imagine any doctor advising you to take both at once. Good luck.

Methamphetamine has weak effects on serotonin, at least compared to MDMA. MDMA has the meth chemical scaffold, but it’s much, much more focused on serotonin. Long-term meth use in high doses can deplete serotonin, but using MDMA once can leave you with barely any serotonin for days. It all comes down to frequency of use, really. Amphetamine itself has negligible effects on serotonin.

One last thing, there are plenty of people who do not become psychotic on crystal meth. Everyone I knew who did crystal meth, back when I did it, had zero psychosis using it. I did form psychosis while using crystal meth, at one point, as a result of staying up too late. Then, I stopped staying up too late and didn't experience any psychosis on the drug after getting proper sleep. Someone I used with who knew Chemistry and was a pharmacy technician, and used crystal meth for over a decade without experiencing any psychosis, told me I needed to get proper sleep if I was to use crystal meth with him. We took breaks, where we would not use for 3 days and then use again. People need to learn harm reduction when using any drug, including crystal meth. Crystal Meth, for most people, will not be a problem if people get proper sleep, eat well, take care of their hygiene, and don't use too much. All of that is possible to do while using crystal meth, and if people do that then they won't be discovered for their meth use. You can only tell if someone is using crystal meth if that person stays up too late, does way too much of it, and neglects to take care of their personal health. That is a small minority of crystal meth users. I only saw one person in my life, back when I used crystal meth, who fit the popular stereotype. You are not helping people by pushing misinformation. I have studied drugs, studied addiction, and I have also the experience of doing these drugs and being around people doing these drugs to know better. There is a difference Drinking it in a cup of coffee doesn't give you much of a rush. Snorting gives you a rush but not very intense. Smoking it instead gives you a really good intense rush. Running it gives you an extremely intense rush . The rush can vary in intensity and they last pretty much the same amount of time. And the overall high ends up about the same. And length of time. The only real difference is the intensity of the rush.

Great video, as always! I have a question: If someone chronically abuses methamphetamine or amphetamine, leading to issues such as cardiomyopathy or blood circulation problems, are these conditions reversible? Some doctors are afraid to prescribe amphetamines; maybe they are afraid of these conditions developing. I'd appreciate your input, Dr.
@PsychiatrySimplified

Abuse at high doses can result in longer term issues - but this depend on the severity. In terms of reversibility I wouldn't be able to give a definitive answer as such cases would be seen in greater portion by a cardiologist who may have a better idea. Amphetamines in therapeutic doses are safe

Creepy crawlies are not my thing, but my roommate has asthma and was prescribed some pretty sketchy stuff back in the day. He actually used “speed” to control his asthma when he didn’t have health insurance. I don’t know what the difference is between speed and meth, but I am curious if you could explain it.

Sharing my own personal experiences here. I do not have ADHD. I do have other mental disorders that were triggered by past drug usage & childhood trauma. My body does not respond well to heavy stimulants. I experienced psychosis with past meth abuse & I've also experienced psychosis & extreme anxiety from Adderall that was prescribed to me. I was misdiagnosed. My body just doesn't like what I would call extreme stimulants. I can handle a few cups of coffee. Beyond 2 cups I have issues.

As a current meth addict, one part of this video stuck out for me, and that was when you said that methamphetamine caused a ~2000% increase in dopamine transmission. Something that users will all talk about that was not mentioned here or in other videos that I have watched is that both for Heroin and Methamphetamine the very first significant dosage of either drug is unlike any other dose that follows. The intensity and duration of the first significant dose *_CANNOT_* be understated or ignored. It is transformative. Much like the graphic you used with the red liquid almost filling up the human body outline (seen @ 2:38), the first use feels like this, but imagine that instead of red liquid, you are being filled with an absolutely euphoric feeling of joy and happiness. Every fiber of your being is just buzzing, and literally looking at your own hand seems amazing beyond belief. Sex is a joke compared to that feeling, and that is how/why most people get hooked as long term addicts. After that high, your thought is that all the other times you use meth will be like that, (because you'd never done it before) but in reality you only feel, at most, 20% of that same degree of high after that first use, no matter how long of a break you take. This used to be called "Chasing the Dragon" ( the dragon being the first dose high), and as old of an idea as it is, it's worth understanding that it is easily one of the most significant factors that causes people to continue usage after a first dose. If my first dose of heroin or meth had felt like my 3rd or 4th use, then I would have had no problem at all in keeping myself from continuing usage, because I've always had a strong sense of self-discipline and self control, but those first doses just break you. Nothing can prepare you for what I can only imagine is decades of built up, reserve dopamine that is just continuously released like a firehose into your system for hours on end.

I used to do meth, and holy sheet was is absolutely insane the first time I did it. I got a big fat clear crystal in Vegas for my first time, probably came from a superlab in Mexico. It really is unbelievable just how high you get… It’s scary when you’re still out of your mind high after 6 hours, and your buddy tells you that you still have a few more hours to go. 🫥 I have also been clinically diagnosed with ADHD a few times starting at childhood. Real ADHD, where it can be proven by brain scans and neurological mapping. I can tell you as a matter of fact that the two (methyl-amphetamine and the other amphetamine salts in adderall) are definitely different. I can’t take 20mg of adderall without feeling awful, whereas with meth I could smoke a pipe full and be super euphoric. Pisses me off when someone says they’re the same exact thing. Hopefully you can eventually quit the meth though. It’s fun for a bit, but it ain’t helping you make money… I don’t have access to a big portion of my memory now because of it. What a waste of potential…. 😞

As a 40 year taker of Methamphetamine (Narcolepsy/catalepsy), I would never even consider taking more than 20 Mg in a single day. In me, a dose of 10-15 Mg lasts a good 18 hours. The one time I took 20, I had the jitters so bad, I couldn't function, much less understand how anyone would want to feel like that. Related, because I don't take enough for the euphoric effect to kick in, I have never become addicted to it. I stop from time to time to make sure I am not. Now, due to US regulators, the prescription form has been unavailable, and as such I have had to do without for nearly a year now. The result has been the loss of about two hours of productive time for work each day. Fortunately, I have an understanding boss. It will be interesting to see if I get the same relief once I start taking it again.
