“HER2-Positive” breast cancers are fast growing tumors that more frequently spread beyond the breast to other parts of the body. Because of this, they are a bigger threat to your life than most other types of cancers with different receptor patterns. These cancers are called “HER2-positive” because they have too many HER2 protein receptors on their surface. These tiny proteins act like “light switches” to turn cancer cell growth “on.” Chemotherapy, combined with new, “targeted” drugs, are very effective against HER2-positive breast cancer and are a leap forward in breast cancer care.
Your Breast Surgeon will know your “receptor pattern” within days after your initial breast biopsy. These results are often not communicated to you early on in your decision process. Although only 20% of breast cancers are “HER2-positive,” it is imperative that you specifically ask your surgeon immediately, and well before surgery, “What are my receptor results?” Take our lesson on “My Tumor Receptors” to learn more.
Treated with Chemo and “Targeted Therapy”
HER2 Receptor Positive (HER2+) tumors are incredibly responsive to chemotherapy when paired with new breakthrough drugs that target these cancers, such as Herceptin and Perjeta. The same holds true if a HER2+ tumor is also Estrogen Receptor positive (ER+). HER2+ tumors are more aggressive cancers, but we now can treat them more effectively, than in the past, with chemotherapy and “targeted immunotherapy” drugs that are designed to destroy them. Everyone with a HER2+ tumor larger than 5mm (1/4 inch) and in good health is considered for chemotherapy and targeted therapy.
Unfortunately, studies have shown that many patients are not offered these standard of care, life-saving “targeted drugs” along with chemotherapy. You will make better treatment choices when you are well informed about HER2 therapies before meeting with your medical oncologist. You must inquire about Herceptin, Perjeta, and other “anti-HER2 drugs” that may be of benefit to you.
Ask about the benefits of “Neoadjuvant Chemo”
What is often overlooked are the benefits of offering neoadjuvant chemotherapy for patients with HER2-Positive, “Early-Stage” (1 & 2) breast cancers. There are distinct advantages (listed below) to having chemotherapy before surgery instead of afterwards. The decision to consider neoadjuvant chemotherapy always begins with your breast surgeon. Breast surgeons choose the initial direction of your entire breast cancer treatment plan. You must address this “cutting edge” treatment option well before surgery to benefit from neoadjuvant chemotherapy. Do not be afraid to ask. This is a very important question. The Potential Benefits of Neoadjuvant Chemo:
*Begin life-saving chemotherapy earlier
*Reduce the need for a mastectomy
*Improve lumpectomy cosmetic outcomes
*Reduce the need for an “Axillary Dissection”
*Allows more time for BRCA genetic testing
*More time to think about “lumpectomy vs. mastectomy”
*Shows your cancer team if the chemo is working
*Can possibly eliminate all cancer cells before surgery
*Reduces the need for radiation after a mastectomy
Ask if you might benefit from a Clinical Trial.
New therapies must be studied in clinical trials to make sure they are safe and effective at treating breast cancer. HER2-positive breast cancers are currently the focus of intense clinical research. New drug treatments are rapidly being developed for this aggressive cancer. Less than 5% of all patients with breast cancer participate in clinical trials. Patients with cancer willing to participate in clinical trials are essential for the advancement of breast cancer care. Ask your medical oncologist if they offer or recommend you participate in a clinical trial for your unique breast cancer situation. I was diagnosed with stage 3 Her 2 positive invasive ductal carcinoma at the age of 32. I completed 6 rounds of chemo on Aug 2 (tumor literally disappeared) had my lumpectomy on Sept 3 and pathology came back BENIGN!!! My oncologist had to stop perjeta halfway thru treatment because it got me so sick! But Thankfully I responded to the rest of the treatment extremely well! Chemo sucks! CANCER sucks! To everyone in the fight remain strong and positive and you will get through it and conquer this evil beast! My journey is not over but I can see the light at the end of the tunnel! Prayers to everyone in this battle! 💪🏼🙏🏻💋

@Stacie Jo Miller Hi Stacie! I was diagnosed 3 years ago with the same cancer as you. I did not have any actual "tumors" but had calcification spots and 3 lymph nodes were affected in my armpit. I did chemo, surgery and radiation. I had a complete response from the chemo (no cancer left) and have had a clear mammogram since then. Do not be scared, you will be fine! It will be a year of treatment but it goes very fast once you get in the routine! I worked through the whole process. I had to take a day or two off after the "red devil" chemo infusions but otherwise I think working kept my mind off things and kept me busy. A positive outlook I think helps immensely! MARDI RESAN GOMEZ Iam currently doing extremely well Iam still in treatment I go in every 3 weeks for herceptin and perjeta. I will be done with herceptin next month and perjeta by November. Both are preventive meds for my diagnosis to keep the cancer from coming back.

Stacie Jo Miller I continued to work throughout chemo although you will have to set limits and shorten your days and work week. Don’t push yourself to hard listen to your body and rest rest rest. I did have moments where I couldn’t go to work at all but I was blessed that I had an employer that was amazing throughout the whole process Good luck and stay positive 🙏🏻

I had herz2 and we opted for surgery, lumpectomy and radiation for 4 months that was in 2012 and so far so good.

I think I am going to stay the course with that also. I am HER2 positive as well as estrogen and progesterone positive. I had 16 weeks of targeted radiation, face down, to protect other organs with minimal side effects. I am small breasted so I was borderline but it worked out. Chemo is so terrible toxic. I am very concerned about how it will affect other health issues and side effects. I have cancer elsewhere in remission from immunology treatment that has been around for 40-50 years - BCG for bladder cancer.

I was just recently last 11-13-2023 diagnosed with ohh Her2 positive & Invasive Lobular Carcinoma & Cancer is also in the breast tissue outside of being in the 7cm and 6-6 CM masses . I stay Chemotherapy( stages 2 of the 1-3 # & the Hormone range can’t think of the proper term is should be I believe My Oncologist said the high range is around 30 & mine is at 62 … I am at stage 4 cancer . I haven’t gotta all of the last dr visit report from all of it . I stay chemotherapy , next week . Pet scan - Heart echocardiogram ( Because I have so many serious health issues . I have a heart aneurysm and a hole in my heart ) I have severe spinal diseases and have had 2 4 level spine surges fusions and much more with a year half . ) so NeXT steps for Me 🙏🏻🙏🏻🙏🏻❤ Classes for new cancer patients- Port put in place & begin treatments right away . Get 3 months chemo measure and monitor size of tumor … WHEN shrinks down by the 6 month then I’ll get the first surgery … remove the tumor ,, More chemo ,& radiation .& then remove the lymph nodes all nodules . Then total mastectomy and breast reconstruction . Yes it’s A lot alright and a lot to take in but My Both My surgeon as well as My Oncologist said I was doing great in accepting it and had a great positive and very strong attitude moving in forward in My fight against Cancer. 🙏🏻✝️🙏🏻❤️& Fight I am ! I have A lot to live for ! My Family needs Me .

Thank you Dr. for these brilliant videos. I wish I had known about them when I was diagnosed the week before xmas. I had found a lump myself in my left breast. I immediately went in for a mammogram. They then did an ultrasound and biopsy,and found that I had dcis in my right breast, and a3.2 cm tumor in my left breast. They sent me to their oncologist, who talked for 4 hours straight, even using a whiteboard. By the time we left, our brains were like mush. They wanted to do chemo first before surgery, but only because I have silicone implants, which I have had for 40 years. Both of their surgeons insisted I have the implants removed before they would do a small, simple lumpectomy. I heard of a great surgeon in town, who was supposed to be the best in his field of breast surgery. HR didn't bat an eye about the implants, so I had to change providers so I could use him. Well he removed the tumors, on in side. And even though my new oncologist also wanted to do chemo first, I said No, too much time had passed, and I just wanted the cancer out before it had time to get any bigger. Right after my surgery I found your Breast Cancer School. Even though I still think I made the right decision, I have learned so much about breast cancer that I wish I had known sooner. My daughter just let me know she has breast cancer, 5 lumps in one breast, and I immediately sent her to your videos. Thanks for all your help.

At the age of 58 I was diagnosed with Invasive Ductal Carcinoma stage 2 Her 2+ , I underwent Modified Radical Mastectomy , Chemotherapy 6 cycles, Targeted therapy ( HERCEPTIN) while ongoing there was recurrence- lumpectomy done then finally Radiation done 37 cycles ........ thank God im doing great now Thanks also for my Doctors Also have my maintenance Femara 2.5mg Dok,how is my survival rate? Thanks.....

I was diagnosed 2019 with her2+++, er+, pr+, my biopsy was sent to America for a second opinion, 2020 I had a double mastectomy and only 3months later started with chemotherapy !

I have HER2 triple positive breast cancer (invasive duct cancer). I am on herceptin & perjeta. After the first heavy dose of it and 2 other chemos, the 2.5 cm tumor which was so close to the skin’s surface, u can see the outline of part of it, started to shrink by 1/3 the size. The chemo drugs do work. I will more than likely have a mastectomy because I’m so small chested, a lumpsectomy would look worse. I have been tolerating the chemo pretty good. Just tired of being tired. But it’s only temporary.

Hello from CA, How are you doing? I am just at the beginning... with same diagnosis and 1.9cm tumor. They scheduled me a surgery first then treatment. I’ve been searching a lot on this topic and see that a lot of people did the treatment first.. I will definitely be talking to my surgeon again to discuss all what I’ve learned. If you get a chance, let me know how you doing now and what made your doctors chose the treatment before surgery. Thank you!

Shlerani hello! I am doing well. Ok so here’s the thing when it came to my situation. I found my lump 11/2018. Self exam found it. Then the rollercoaster ride of appointments came. On 2/14/2019 I had my first of 4 surgeries - if you can get the port do it! Chemo will collapse the veins in your arm. My doctors decided to treatment first to shrink the tumor. We were thinking of doing the lumpsectomy after chemo, but decided to do the double mastectomy instead. Now the tumor grew from the nipple upwards, diagonally towards the sternum. After 6 rounds of chemo, we did another MRI and nothing showed. We did the double mastectomy, expanders in, and 3 sentinel nodes and 1 auxiliary node were removed and came bk clean. My mom is 83, has had a lot of surgeries, weak, not healthy, anxiety, brain fog BEFORE any treatment even discussed. She is looking for QUALITY of life. The cancer center suggested lumpectomy, chemotherapy 5 hours a week for year!!!, radiation and they who knows after that. Tamoxifen possibly, I do not believe she can survive this at all. Even a going for chemo sitting in a room for 5 hours would be extremely stressful and traumtizing for her. I was told if we do not follow their protocol they cannot help us. It was very pushy, cold and traumatizing to be pushed through like a factory system 4 hours straight. Not exactly comforting and compassionate experience. She is traumatized.

I was diagnosed with her2 breast cancer in 2019 I went through 5months of chemo. One year after the intial diagnosis I had a mammogram and wound up having a lump removed from my other breast wasn't cancer but said it could turn into cancer.i had a mastectomy on my left breast and reconstructive surgery.they told me that the cancer could be different in the other breast. I have to have another mammogram in October of this year I'm worried about the results.. they told me I'm high risk.

I was diagnosed with Triple + Infiltrate ductile carcinoma in Dec 2018 and went to SIX different oncologist and not one of them sat me down and explained anything in such detail. I was just told I must start chemo ASAP and thrown with gigantic medical terms. Drs really need to take the time to really sit their patients down and counsel them.

It is a racket. This is a gigantic money grab. We spend more on public health than any nation in the world and have some of the worst health. That fact tells me a boatload about the medical establishment. Throw Covid in there and you have corruption on top of it.

I’m invasive ductal carcinoma Her2 positive. My lymph nodes are not effective no evidence of cancer. Her2 positive seem to spread faster than you think. I’m scheduled for double mastectomy in the 10th of January 2023. Then follow up with chemo. I Hope you have gotten treatment before it spreads to the other parts of your body I wish you well

@@mythaichannel9082 I have surgery on the 9th. I did chemo before the surgery. Her2 is aggressive that's why I opted for the double mastectomy. Right now it's only in one area. Surgery will go well and I will do what I need to afterwards. Sending you positive vibes and everything will go well for you.

Met with my breast surgeon yesterday get results back from pathology lymph nodes are Negative for carcinoma, but she said I still need Chemo but not radiation. But that will be oncology doctor discuss and decide hope for the good preparing for the worst. Wish us all luck 🍀

I am trople positive Er+Pr+Her2+ I have 6 rounds 9 chemo followed by surgery followed by rafiation and then 11 rounds of Targeted therapy is my treatment So far have had 4 rounds of Chemo Seeing Breast Surgeon before surgery next month Hopefully the chemo has shrunk or got rid of the tumours

In my case, HER2 was discovered from my mastectomy tissue analysis. So don’t blame your doctor if they didn’t catch it earlier. Just take it one step at a time.

Thank you so much for your teaching Dr Williams. I really like the way you explained your topics and the pace is just right for me to follow especially with so much medical terms. I wish I had seen your video before my treatment, which started 4 months ago. If only my breast surgeon were as patient as you I would have a better understanding of my diagnosis back then. I wonder why my medical team don't provide much details like you do, is it because they don't want us (patient) to ask too many questions which can interfere with their treatment plans. I was diagnosed with DCIS and HER2 breast cancers in August 2022, I had surgery in September and started chemo late October which goes until mid February. After watching your video, I wonder why my breast surgeon decided to go for surgery before chemo, that's a 6 weeks delays from diagnoses... at the time of diagnoses, all these medical terms were new to me and things progressed very quickly leaving not much time to think what's best. My friend does not want to do the chemo that is recommended. She has a lumpectomy. What are the chances of it coming back if the margins are clear & also the lympnodes, stage 1

My protocol was: lumpectomy, mastectomy, chemotherapy and immunotherapy (FEC100 three rounds, Taxol nine rounds, Herceptin 18 rounds), radiotherapy, hormone therapy (Femara I think, seeing the onco in a fortnight). For a 21mm node-negative HER2+ tumour. I've got expanders in both breasts and will be undergoing a DIEP rebuild in March 2019. I eat and drink what I like and I've stopped taking supplements.

Respectively, you should not be eating and drinking what you like. That is just common knowledge by now. Sadly, modern medicine doesn't educate doctors on nutrition and relation to disease and healing. Please do your research. God bless.

@@Shlerani The tumour was operable and didn't need shrinking. But the margins were not clear and it was over 21mm so chemo was automatic.

@@Shlerani Depending on your tumour type and grade, consider mastectomie. Lots of relapses after lumpectomie. Also find out if you have BRCA1 or 2. I have neither. But ask for the maximum treatment. I'm in France so 100% covered by social security.

Mi doctor wants me to get 30pluz radiation seccións after my very agresive 6 sessions of chicoterapy surgury and reconstruccion. But im afraid of my implants to give me problems with all that radiation and me end needing more surgeries in my breast plus all those side effects. What do you think im 42 years old a very active woman i want to enjoy life again not just see it past infron of me. My breast cancer didn't reached my lymph nodes on my arm pit. After surgery for lumpectomy my surgeon and oncologist suggested me to go for chemotherapy treatment. It really makes me weak. My both receptors were positive and her negative. Dr. William what's your opinion about it.

I had Biopsie and 3 weeks latter had appoitement with my Surgent over the phone because of COVID. ALL Ibeen told was " the result showed I have Triple Positives Breast Cancer and requering additionnel test Bone scan , TC , and tumor markers.All I new to ask was my stage ? I am in stage 1 or Ist stage .Week later I doneed Bone scan. Now I am waiting for Med. Oncologist and Radiiatio Oncologist appoitement I dont know resolts ofBone scan and notiing ??? Not fun at all when we dont have any nowledge about our problem ! I may come back in a couple of days and ask you some questions ! GOD BLESS YOU !

I was diagnosed with stage 1 HER2 positive invasive ductal carcinoma at age 50 On 12/22/2022 my healthcare provider decided to do a surgery before chemotherapy. My oncology doctor said the surgery first will help her decide how much medication she should give to me for the treatment she doesn’t want to give too much or too little because of my stage for me whatever doctor says I should take their advice. Doctor know best

It is possible to receive the targeted therapy (herceptin plus chemo) when you have DCIS and you don't know if it is micro-invasive just yet? (several micro-calcifications, 7.5 by 3 cm in extension).

Dear Dr. Williams! Thank you very much for the wonderful presentations on breast cancer topics! If all doctors would be like you, patients would be much happier regardless of the treatment outcomes. The oncologist I saw the other day is against neojuvant therapy even though I am for it. Her argument is that I am 64 years old (no underlying medical conditions) and that I have stage 1 tripple-positive breast cancer with 2 sm and 0.5 sm tumors. She thinks treating me with chemotherapy first would be too aggressive. Does age play a role in deciding who is a good candidate for a neojuvant therapy? Your response would be highly appreciated!

Oncology may give you too much of medicine than you need, I am HER2 positive stage 1 and my oncology agrees to do mastectomy first to find out how bad it was so then decided how much medicine she should give me. She doesn’t want to give too much or too little medicine than I really need

I am Her2 stage 4. Was on a clinical trial for 3 years when tumor in breast started to grow fast. Dont know what drug was administered, Herceptin or Hercules (Mylan study). Still no hair for nearly 3 years and 7 months. Had a mastectomy last November 2017 . On Herceptin but started developing diahorea. Should have received Herceptin yesterday, but did not. I am confused and tired of feeling sick, exhausted most of the time. Had 3 tumors in right breast, lymph nodes, 5 larger tumors in liver, both lungs full of small fast growing tumors. Trail treatment saved my life, but it was pure hell. Side effects... Develop fybromaylgia, sore burning feet and hands, diahorea, constent flu like symptoms. Now I am at a road where I am just scared and confused .... What is the bad side of Herceptin on the long run? No hair, bad diahorea ect .... I am a positive person, but jeez.... You only can take so much. Just tired I guess.

Hey Jackie sorry to her what has been happened to you. There are a lot of natural remedies out there, you can try while still doing your treatment. I just found out my sister in-law has HER2 but we are going to try natural remedies first before she go's with conventional treatment to see if the tumor will shrink. Will try to get her HP over 7 or higher to help the tumor from growing. The funny thing is she eats health but drinks wine and has had a lot of stress in the past which can make the body acidic and we need to get her more alkalized. OK Here go's list: Iodine liquid read up on it doctor Dr. Bob DeMaria Dr. Berg and Bergman. I health Cancer youtube and Iodine , sea salt, ( Baking soda ,raw honey pure bee no crap in it Farms market read labels important ) , rosemary, thyme , sage, bay-leaf garlic ginger, turmeric. these all your friends put herbs in tea's all herbs, build immune system good , eggs , fish Tune good meats, no deli meats high in sodium chemicals good but bad. (chicken has good high in selenium Brazilian nuts 2 very other day). No oil's maybe good pure olive oil little. No processes foods bad sugar and white flour. Dressings Make your own balsamic vinegar squeezed lemon rosemary fresh olive oil little. No starches like pasta soda, rice soy all bad. Drink Mineral water Perrine instead of soda. read or look up anti cancer food fights eat them. I'm also taking Herceptin, did three cycles of FEC 100, nine Taxol and 18 in all of Herceptin (six or seven so far). Next appointment is to start Femara hormone therapy. I hope you're napping after your Herceptin infusion, and eating reasonably well. Stop all food supplements and liver cleansers, stop all essential oils, spices like turmeric, etc. Listen to your oncologist and just cook from scratch and eat what you like. I was told EAT FOR ENJOYMENT. I'm being treated in France. Certain spices and food interact with the drugs, so do your research and don't take anything without showing your onco first. Hey Jackie just wanted to up date on the my sister in law progress no luck with HEMP oil! yet? but she did go with convectional treatment because the fast growth of the tumor. I hoped it would have worked for her, instead of going the other route good luck Jackie. I did speak to my brother on his wife situation they are still waiting for test results and the Hemp Oil they were using was a extracted fruity drink for it tastes. I am now a survivor. I've had lumps since high school they were benign all the way up to the age 39 . I had two cancer's in my left breast and I found out that I was Her2 positive and triple negative. My doctor's put me on the Red devil and then the blue devil. I had 8 rounds of chemo. It sucked and I had to do 28 rounds of radiation. I can't get implants until after 5 years. I took chemo after my lumpectomy. My tumors were very aggressive. I had invasive and ductal carcinoma.

I had my biopsy on 7/05/20 Stage I insitu and IDC 3 mm lesion. The pathology was mixed. ER+ PR+ and equivocal for HER2. The surgeon requested a FISH test. I have already had the lumpectomy and the results are not back regarding HER2 status.

Hi there, My sister had one side of her breast removed few days ago, but the surgeon found no cancer cells in the lymph nodes. The surgeon said the cancer might be at stage 1 or 2A depending on the report from Pathologist. In this case, is chemotherapy needed or not? Thanks in advance for your feedback on this.

I have stage 1 breast IDC cancer, no lymph node involvement but HER2 positive so will be starting chemo in January with Herceptin. It wasn’t diagnosed as HER2 positive till after surgery biopsy result. All previous biopsy showed precancerous cells only.

I am 37, stage 2a her2 positive multicentric breast cancer. I was told mastectomy is the only option for this type and Im currently headed into round 3 out of 6 chemos. I am doing neoadjuvant chemo than surgery than finishing my year of herceptin and perjeta.

I’m 55 and diagnosed with stage 1b multi focal invasive lobular carcinoma with her2 positive. Just finished lumpectomy. The oncologist recommended TH (Taxol + Herceptin 12 weeks) or TCH (Taxotere + carboplatin + Herceptin 18 weeks) for following up systematic treatments. Anyone used either of this regimen? Any pros and cons to share? Thanks!

ADDITIONAL REPORT Histological examination after formalin fixation and paraffin inclusion shows: marked. They have rounded or oval nuclei, irregular contours, with fine chromatin, sometimes 1 An invasive breast carcinoma arranged in clumps, spans and uids. Cells show atypia nucleolated, and surrounded by moderately abundant cytoplasm. The mitotic count objectified the presence 10 mitoses / 10 CFG. The stroma is fibro-inflammatory. Absence of vascular emboli. Absence of carcinoma in situ TILS are estimated at 30%. The nearest lower resection limit is estimated to be 2 mm. The immunohistochemical study of the expression of hormone receptors and overexpression of HER2 (carried out Ventana Benchmark GX controller) shows: -Estrogen receptors (ER, clone SP1): 90% positive tumor cells (positive internal control); intensity CONCLUSION: 1 - Lumpectomy specimen: F22091080 intense expression. -Progesterone receptors (RP, clone IE2): 60% positive tumor cells (positive internal control); intent of expression: moderate. -Proliferation index (estimated using Ki-67, clone 30-9): 50% - HER2 (clone 4B5, ready-to-use): overexpression of HER2 in immunohistochemistry, rated at 3+. 2-Sentinel lymph node: The histological examination confirms the data of the extemporane examination. Page 2-Sentinel lymph node: ON+/2N. Infiltrating breast carcinoma, NOS type, SBR grade III, Luminal B phenotype - HER2 pos Absence of carcinoma in situ.
