Are you alright instead of saying Hello how are you is different.. You only use : are you alright when something is wrong happened, i.e. the kid tripped over, car accidents, sickness , damaged something.......and so on....

I think the "going" in "How's it going ?", loses its individual word stress on the first "g" n the last "g" becomes stressed for the "?" . The entire sentence of "How's it going ?" becomes 1 word with the word stress on "H" n the secondary stress on the very last "g" " 'Howsitgoin'g? ". ' in front of a letter indicates stress.
