Anti-neutrophilic cytoplasmic antibodies (ANCA) are IgG autoantibodies against the cytoplasm of the neutrophils and monocytes.
This is different from antinuclear antibodies (ANA) which are antibodies against the nucleus.
Anti-neutrophilic cytoplasmic antibodies (ANCA) have several patterns; the most important two being the cytoplasmic ANCA (c-ANCA) and the perinuclear ANCA (p-ANCA).
Anti-neutrophilic cytoplasmic antibodies (ANCA) are associated with vasculitis, such as granulomatosis with polyangiitis (Wegener granulomatosis), microscopic polyangiitis, eosinophilic granulomatosis with polyangiitis (Churg-Strauss), rapidly-progressive glomerulonephritis (RPGN), anti-glomerular basement membrane antibodies (ant-GBM or Goodpasture's syndrome), as well as inflammatory bowel disease-associated. vasculitis.

In pathophysiology Proteinase 3 and mop present in azurophilic granules in cytoplasm of neutrophil and monocyte In that Since monocyte is agranulocyte how it contain azurophilic granules Sir can u xplain it

As far as I know ; monocytes are AGRANULOCYTES ! So how comes they contain AZUROPHILIC granules with their specific target proteins (Proteinase 3 & Myeloperoxidase) ?

I think it is because granules in WBC are clasified as either specific or azurophilic. The specific granules containe cytotoxic molecules and are the ones found in granulocytes such as neutrophyls. On the other hand you have azurophilic granules which mean they are stainable, this can be found in both granulocytes and agranulocytes such as monocites. Therefore the term granulocyte means they posess the specific granules.

@@franciscotapiafernandez5209 hi man, would you help me identify the meaning of granulamatosis? Is it referred to granulocyte i.e neutrophile,basophile,eosinophile before being attacked by auto-antibody or after, so that , as a result they have become as a complex called granuloma? Thank you all

Hi, I've been ill for several years and have gone to so many specialist in several different states with no definitive diagnosis! I have had a positive ATYP P Anca test continuously my Titer is also elevated at 1:40.my rheumatologist said the mpo is negative so she's not sure what it means. I am spilling protein in my urine and have had blood too. Nephrology says it could be from my diabetes and said he'd rather not do a biopsy because it's pretty risky. My A1c is between 7-8. Yes it can be improved but I don't believe this is the only reason for this. I am very fatigued, sob a lot and my insides actually hurt! I also have elevated liver enzymes and had a biopsy they said it was probably fatty liver. If anyone has any ideas please share, I would really appreciate a medical professional to comment! Thank you so much!

I have positive mpo and pr3. Looking for answers on what that could mean and be associated with. Currently in progress of being diagnosed. For now it's considered uctd.

Hi, my pANAC came back positive: Proteninase-3 (Phadia) 5:10 IU/nL. I'm basically a fit guy, with a fair bit of muscle - my only issue was that my Kidneys are not working great, 60 eGFR. However, I have had this same eGFR score now for 8 years, always around 60. Can I be --ANCA positive but not have Vasculitis ? My blood pressure is normal, no blood or protein in my urine. I'm freaking out, this is an awful disease - ive got to wait another 6 weeks before a biopsy.

You'd only be positive for vasculitis if you're positive gbm (I believe)

I was just diagnosed with ANCA + Crescentic Glomerulonephritis - just did my first infusion of Rituxan. Looking for some information to help me understand my prognosis.

Thank you for making these great videos! Just a question at 2:03 discussing the larger titres, the ratios listed are 1:160, 1:320 - is a positive ANA mean a ratio smaller than 1:80 or is it just a typo?

If the serum antibodies do not enter the neutrophils, how are we getting the immunofluorescent staining that is used to characterize P-anca vs C-anca? Is the actual test done to test for ANCA, just done with serum and the immunofluorescent is a demonstration of antigen location our of principle? I guess what I am asking is: what is the point of staining neutrophils with markers (elisa, or immunofluorescence), when we could just be testing the serum for these levels (using elisa i guess)? I totally want to be your friend btw.

Look, I am not a lab scientist, but I think the serology will tell you it’s ANCA (the machine will tell you that), but the best way to actually verify that this is c-ANCA or p-ANCA is to see with your eyes (immunofluorescence)... I guess it’s similar to the difference between automated analyzer and a peripheral smear.

I tested positive for panca past year. I am still waiting for confirmation via biopsy. I have symptoms and he said the test was 100% positive.Am I going to die?

What happened m8? I have just been diagnosed, its awful - I'm fit and healthy, just had some dodgy kidney values - well, went the other day and got told I had this. WTF!!! 1 out of 100k chance in getting it.

Medicosis you are fantastic, but after watching 71 of your videos, you finally said something I do not understand. What's this about hypereosinophilic Syndrome having no IgE???? According to Up-To-Date, HES does have elevated IgE. You're right about it having no ANCA, so why did you even mention it here? For the first time, I am confused. Fantastic job up to now, don't worry, I'm still loyal.
