I was working as an ER physician in a VERY rural ER in the Ozarks of Arkansas just a few years ago and had an elderly man come in with this. He had just been discharged home after CABG a few days prior and the patient and his wife just couldn't understand why I had to load him on an ambulance and send him over 100 miles back to his CV surgeon.
