Ambulances have come a long way. They had all those suitcases full of gear stacked back there, imagine having to rummage thru that stack of cases on the go with a critical patient in those cramped quarters with minimal lighting etc. They did have some ready storage for some items and a defib unit handy. There's a pretty good size alternator under the hood to power the lights and accessories they had. Sometime in the 70s they figured out that vans make a better platform for ambulances. Today they are big diesel box trucks with a very efficient layout in back, everything readily accessible, tons of sophisticated gear and electrical power to run it all.

I had an uncle who built his own company starting from very humble beginnings. Back in the early days he had a goal of owning a new Caddy someday and he joked to his wife that if he died before he got his Caddy then she must specify that he take his last ride in a Cadillac hearse. He got his new Caddy in plenty of time though, so that issue never came up. That Cadillac flower sled would make a great last ride for some guy who didn't make his goal in time.
