To be fair to Holt he has a point, the word doctor has nothing to do with medicine, it’s origin in latin and french mean to teach or a learned person. It literally describes a level of learning which is what a doctorate for any profession describes. Just nowadays we associate doctor with medicine…though you’re probably right that his husband being a non medicine doctor probably added to his anger since he earned his right to be called Dr.

It was used to indicate _any_ type of teacher, regardless of their field and degree, even personal trainers. The word for later used for the Apostles and by the third century it was generally used for Christian preachers until the 12th century, when the degrees of MD (medicine), ThD (theology) and PhD ("philosophy", as in science and humanities) were established in a collaboration between the pope and some univesities, making the three types of doctorate equally old. But when churchgoers talk with a priest they don't call him by his academic degree, they call him Father. When students talk to their university teacher they don't call him by his academic degree, they call him Professor. And when patients talk with their physician they don't call him by his academic degree, they just call him Doctor. And so the names stuck for centuries, occasionally shifting to include Doctors of the Church, specific academic degrees that change from country to country and all medics, respectively. Plus, PhDs are as old as ThDs and MDs, but have started to widespread only recently (partially due to a Berlin university in the 19th century). Of the three types of doctor, no one is "more doctor" than the others, they are just different and they are all doctors. I don't put my kidneys in the hands of a historian just like I don't send my oculist to teach history.

A lot of your examples are very old and you said that it is now used to describe PhDs, MDs and ThDs, which are all obtained after a certain level of education is reached. So the word doctor is used by those who have a doctorate regardless of their field. Of course not every profession uses them but it is never incorrect for someone with a doctorate in art history to be referred to as a doctor. So even though in common discourse the name of doctor is associated with medicine, in reality due to the origin of the word that you described, it is about education, not profession. As an extra interesting fact that you might find interesting cause you really seem to like the use of words, the word doctor is other languages is used even more broadly. For example in Spanish “Doctor” can be used to also refer to a lawyer even if they don’t have a doctorate.
