To clear up any confusion with the upward deflection of T wave. So the repolarization vector we said is moving upward and rightward basically the exact opposite direction of the mean QRS vector. If you remember the mean QRS vector was positively charged because it was a depolarization wave and we said that when positive charges approach the positive electrode of lead II it causes an upward deflection. Well in the same way the repolarization vector is moving upward and right ward towards the negative electrode of lead II. The repolarization vector is a negative vector and it’s approaching the negative electrode of lead II so if that’s the case negative charge approaching negative electrode causes an upward deflection just like the positive charge moving towards positive electrode do. That is how you get the upward facing T-wave. Hope that helps Ninja’s!!

Thank you so much for the clarification ! I'm just concerned about the propagation of action potential from the AV node down to the His- Purkinje fibers . Actually I have 3 questions ! First : Does the depolarization wave start symmetrically & simultaneously on both the anterior ventricular wall & the posterior ventricular wall ? Second : Does the repolarization wave follow the same course of the depolarization wave in the ventricles ? Third : If the QRS complex of the ventricular depolarization overlaps the atrial repolarization wave ; why the atrial repolarization wave does NOT show in the EKG in case of 3rd degree heart block ?

I am a first year intern in emergency medicine. I some how got by in medical school just by memorizing EKG tips and tricks. As an intern now I know I needed a deeper understanding and you just blew every previous attempt of learning this out of the water. I'm not a doctor or med student, I just had an EKG today as a "routine" of being inducted into Medicare and officially being classified "old". I am a nerd who knows electronics and wanted to know more about how the graph was created and what it was depicting. I certainly learned all about how the pulses are detected and why there are so many wires, thank you for the well done explanation of why you hook someone up just so. It now makes me think about why the machine had so much noise that they had to run it 4 or 5 times and it apparently indicated that I was having a heart attack - well, I have my non-professional opinion about a bunch of long leads running around you, inside a typical commercial building complex, that are trying to detect pretty faint signals. The apology for the length of the video was unnecessary, it covered a heckuva lot!

I just want you to know that I watched this entire thing and when I showed up to class the next day I blew my professor and classmates minds when I was able to explain where the depolarization occurs with each wave. He never even taught us why there are negative and positive deflections. Our textbooks barely even goes into this much detail unfortunately. Without you, I would've never known. I work as a RN on a CV IMU. Been trying to understand the Leads and precordial leads and what they really mean in relation to the flow of the impulse. Never made sense for me and I attended a 3-day EKG course at my organization. Zach breaks down and explains the foundation of EKGs. Having been an instructor in the Navy, I am absolutely in awe of his teaching ability. Wish I had watched his videos sooner. Plan on going over the videos again and mastering EKGs before I take my CV certification. Zach, thank you. You are a blessing.

Basically my notes, hope I did well xD When negative moves to positive, there is a positive deflection (QRS complex) When positive moves to negative, there is a negative deflection (Inverted QRS) The flatline that comes immediately after the P wave is when the electrical current hits the AV node AKA PR Interval which holds the impulse for 0.1sec. The dip after the flatline, right before the QRS complex is a negative deflection (Q wave). The bundle branches depolarize from left to right. Essentially because of the anatomy of the heart, the conductivity is moving top to bottom or negative to positive AKA positive deflection, then from bottom left (positive) to right and up or positive to negative AKA negative deflection. When it reached the apex, it ended at a positive point. Now it's moving from a positive point (Apex) back to the right side and up a little which is a negative point, hence the positive to negative. This negative deflection is the Q wave or septal depolarization. The purkinje fibers are depolarized which then causes the myocardium to contract, making the ventricles contract or a big positive deflection AKA R wave. The R wave is depolarization at the APEX of the heart. Lead 2 is located at the left foot. The net conduction points south east, if our body was a compass. The LV generates more conduction because it's thicker than the RV. Therefore, the net conduction leans a little left instead of the middle. As stimulation is moved outward at the R wave, the current must go up, and when we go from down to up thats a positive to negative AKA negative deflection. This negative deflection is the S wave which is the plummet after the R wave on an EKG. The S wave is the depolarization at the BASE of the heart. ST segment is the time at which the ventricles are still depolarized(no extra movement=isoelectric), it hasn’t repolarized yet. The negative charge is now moving back up and away from the positive Apex, it causes a positive deflection AKA the T wave. 12-Lead-EKG gives a 3D view of the heart on a 2D plane. 3-Leads are bipolar limb leads: Lead I Lead II Lead III - (Have negative and positive electrode for each) | Looks at the heart in a frontal plane | Coronal view or front and back view 3-Augmented Unipolar Limb (AUL): Leads AVF(Foot) AVR AVL - Lead I is a neg node on the right shoulder and a pos node on the left shoulder, that imaginary line that connects the 2 is Lead I - Looking from the left/lateral heart. Lead II another neg node on the right shoulder and a pos node on the left leg. The imaginary line is from the right shoulder to the left leg AKA Lead II. - Looking from inferior to superior. Lead III another pos node on the left leg and a neg node on the left shoulder. - Looking from right/lat heart/inferior. Einthoven’s Law: Lead I + Lead III = Lead II The AUL has one node per part of the body. What the AUL does is switch the charges on the nodes to see what’s happening on one side of the heart and computes it on the EKG. AVR shows what’s happening on the right side -> creates a positive charge on the right node/arm while the left arm and left leg are negative. AVF shows what's happening from the inferior view -> creates a positive charge on the left leg node while the left arm and the left arm are negative. AVL shows what’s happening on the left/lateral side -> creates a positive charge on the left node/arm while the right arm and left leg are negative. 6- Precordial Chest Leads: Lead I Lead II Lead III Lead IV Lead V Lead VI - Transverse or horizontal section Right 4th intercostal space V1 Left 4th intercostal space V2 Left 5th Intercostal space mid clavicular V4 V3 goes between V2 and V4 Left armpit (anterior axillary line) 5th intercostal V5 Left middle armpit (mid axillary line) 5th intercostal V6 Precordial Chest Leads don’t need negative charge leads with the positive ones, just positive leads (unipolar) V1/V2 picks up interventricular septum conduction/posterior V3/V4 picks up anterior ventricular wall conduction V5/V6 picks up left lateral part of the heart conduction V7/V8/V9 for posterior

Again a massive thanks for quite "simplistic" approach to such a very complex topic. Being IT professional and not a medical student or professional, it was very helpful to understand the mechanics of ECG and how wide and exhausting the "view" is, by using just some electrodes and all the work done on the ECG machine side to express in a form of wave that can be quickly and effectively analysed by a professional. Zach, you are an amazing teacher. I’m a pharmacist with two best friends that are PA’s so I have a special place in my heart for PA’s. But, watching your videos, you should’ve gone to medical school. Wow, thanks so much for all your great information,explaining everything easily. I. wish you were one of my instructors when I was learning Biochemistry or Anatomy & Physiology. I work in a EP/Cath lab. I've been a cardiac RN for 18 years and I've taken SO many Ecg classes. You just gave the best instruction of this material I've ever seen!!

In 10:03 it’s actually the PR segment, not interval. Segments are just the straight lines, while the PR interval includes also the P wave.

interval in the sense of actual delay in conduction no recording in the paper just a straight line - it makes sense in his order and your's also makes a point like in the books
