I had this done to me last Thursday for a more 'accurate' set of crown and front teeth fitting as the back molars ALL have to be gold as I suffer from Bruxism and teeth are worn down and they need to 'raise the bite' and with gold / metal it's best to be accurate as harder to adjust I guess. I already had temporary ceramic teeth put in but that didn't require this procedure with the cord only a basic alginate mould. For the cords between the gums I had to have anaesthetic all around the gums and inner mouth and I could still sort of feel it.

@@centerforard yeah no problem, I was just trying to fathom what some other dentist had done to me exactly last week and it is this I'm almost sure. This is the first and only video I was able to find that illustrates this technique. I don't know who pioneered it.

Hi Dr. Steven, do you always do veneers preps and final impressions with cords in or do you put a cord after the preps and before the final impression?

I now place the cord after the veneer preps, prior to the final impression. If you wish to place a size 0 small cord to mark the apical part of the sulcus prior to veneer preparation, that is fine. I used to do that years ago.

How long the cord "sit" in the sulcus dr.steven? Is 10 minutes enough? And for the imoressions, only take out the 2nd cord n leave the first cord in the sulcus?

Yes. The teeth and gingiva are anesthetized prior to tooth preparation.

We should suspend all clinical work that includes the use of handpiece, and only schedule patients that have an emergency, only if it's really neccesary, since we are the most vulnerable workers to get infected of Sars-Cov2 due to the aerosols produced by our practice

My feeling is no one knows how long this covid 19 problem will go on. I would also think this will not be the last of covid 19 once this immediate recovery is taking place and one would think other strains of viruses will come forth in the coming years. We cannot shut down businesses and society every time it happens, and certainly not for months each time. We had better learn how to operate businesses while viruses are present in society or the world economy will collapse.

Use the covid 19 masks, gloves, rubber dams, wipe down surfaces including door handles, do not touch your face with your hands, wash your hands and use Purel cleanser frequently, do not shake hands and follow the covid 19 protocol, just like hospitals. There is a terrific video by a medical doctor working daily in a hospital in NYC that has 1200 beds and only treats covid 19 patients. He follows protocol and says he is not afraid of contracting covid 19. You might want to watch that video on KZbin. It is the best assessment of the covid 19 situation and protocol for not contracting it I have seen.

No, I do not place cord around lower anterior teeth. The tissue around the lower anterior teeth is too fragile. I prepare those tooth just barely into the sulcus, because the sulcus of lower anterior teeth is normally very shallow. I then take the impression without cord being placed. Single cord. I used to use dual cord many years ago to block the identify the apex(bottom) of the gingival sulcus. Now, I just want to open the coronal part of the sulcus to allow the impression material to capture the preparation margin. I do not prepare the tooth for a crown or veneer more than a mm into the sulcus, so there is no worry about preparing into the junctional epithelium, past the apex of the gingival sulcus.

I want a cord that is wide enough to open the sulcus coronally for the impression material. I do not want it to be so wide that it cannot be placed in the sulcus fairly easily or so small that the cord goes to the apical part of the sulcus and does not open the coronal part. I normally only prep into the sulcus 0.5 -1 mm.
