Cervical Myelopathy
Cervical spine myelopathy can occur due to compression of the cervical spinal cord at the cervical spine level. Cervical spine myelopathy is caused by spondylosis, cervical stenosis, cervical disc herniation, tumors, or ossification of the posterior longitudinal ligament (OPLL). OPLL can occur more in people of Asian ancestry. C4-C6 is the most frequently involved level and can be seen on CT scans. Exacerbation of symptoms followed by a long period of static or deteriorating function (stepwise pattern). 75% of patients will have long periods of stable neurologic function, which may be for years. In the majority of patients, the condition deteriorates between quiescent streaks. The condition does not improve without surgery. Pain is absent or poorly defined with vague sensory and motor changes. The patient will have progressive gait and balance disturbance. Broad based ataxic gait and unsteady gait. There will also be intrinsic muscle weakness with difficulty in buttoning the shirt and performing fine manual activities (weakness of grip strength). Poor hand fine motor function and dexterity occurs early. Pathologic long tract signs will be seen consisting of ankle clonus. Clonus sign is a non-voluntary sustained movement of the ankle muscles with firm, passive, continuous stretch. Pathologic long tract signs will be seen consisting of positive Hoffman’s sign in about 80% of patients. The Hoffman’s sign is done by flicking the nail of the middle or ring finger to produce flexion of the index finger to the thumb. Pathologic long tract signs will be seen consisting of bilateral outgoing toes by a positive Babinski reflex.The Nurick classification deals with walking ability and gait. It ranges from normal to wheelchair bound. When you have cervical myelopathy, you need to get an MRI. MRI is the best study, it shows the compression and the changes in the spinal cord (look for bright signal in T2 MRI). In patients with low back pain and gait disturbance, look into the spinal cord, especially into the cervical spine. Some of these patients will have lumbar spinal stenosis and they come to the doctor with an MRI of the lumbar spine that shows lumbar stenosis, but the patient also has gait disturbance. The patient examination may be confusing because they will come to the doctor with low back pain and a positive MRI for lumbar spinal stenosis. Lumbar stenosis and cervical spine stenosis can coexist together in about 20% of the patients. Cervical myelopathy can have a differential diagnosis of amyotrophic lateral sclerosis (ALS), syrngomyelia, spinal cord tumor, or multiple sclerosis. ALS is a motor neuron disease affecting both upper and lower extremities with no sensory changes, and will lead to progressive weakness, muscle atrophy fasciculation and spasticity, in addition to dysphagia and respiratory compromise. In multiple sclerosis, the patient will have cranial nerve involvement, and the jaw jerk test is positive. Cervical myelopathy is progressive, and it rarely improves with nonoperative modalities. Decompression and fusion in case the symptoms are progressive or severe. Patients with rheumatoid arthritis are going to surgery for symptomatic cervical myelopathy. The neurological recovery after decompression is best predicted by pre-surgical posterior atlantodens interval exceeding 13 mm. with more than 10 mm space for the spinal cord, the patient will usually have improved neurological function. The patient will die earlier if the patient does not have surgery. Surgery is usually done anteriorly with decompression and fusion. One to two level disease, do anterior cervical decompression and fusion, especially with a fixed cervical kyphosis to more than 10o. Multilevel involvement of three or more disc spaces is easier to be managed by posterior approach if there is no fixed kyphosis. Lateral x-ray will show if there is any kyphotic alignment. You will do multilevel posterior decompression and fusion for multiple levels of involvement. Posterior approach has a higher incidence of would infection. You should not do posterior fusion if there is a cervical kyphosis. The residual kyphotic posture of the cervical spine will result in persistent spinal cord compression. For fixed kyphosis more than 10 degrees, you will need to go anteriorly (don’t go posteriorly if more than 10 degrees kyphosis exists. For multilevel cervical spondylitic myelopathy, the most common adverse post-operative complication is loss of range of motion up to 50%. Laminoplasty is used instead of Laminectomy to prevent progressive kyphosis. Laminoplasty is done by decompression of the cervical spine by widening of the spinal canal. The patient may get nerve root palsy (C5 nerve root is usually the most common involved nerve root).

Dr. Nabil Ebraheim, in 9 minutes you have concisely and completely explained my spinal status perfectly. For the very first time and so clearly that most doctors should be able to understand. I thank you as I am going to provide a link to this to my GP and neurologist (in Aug., 2021). Blessings upon you.

This video described everything way more clearly than the Neuro Surgeon I visited. The Dr. I saw just rushed through trying to explain what is wrong with my neck and how he would perform the surgery. He seemed so rushed, and acted like he had a million other patients to see and had only 5 minutes(or less) of time to speak with me.

Dear Doc, Thank you for your explanation of these issues. I only wish all Neurological surgeons would be as educated as you, because they are assuming they know things when in reality they are many times doing surgeries when the problem is in a different area as your mentioned. Cervical issues can be causing lumbar like symptoms. I have had a few surgeries and I am serious when I say I wish you were my Doctor and able to diagnose and help me. I struggle with balance and pain and have issues in my cervical and Lumbar areas of my spine.

Dr. Ebraheim. I had to have Anterior Cervical Disketomy and Fusion (ACDF) in the summer of 2017. I was -to put it mildly- quite concerned. Many of the neurological symptoms you discuss in this video were going on, and what the surgeon explained, though it made sense, was hard to visualize. This video is spot on explaining everything in a succinct manner. I very much wish i had had this resource available then. Thank you for putting this together.

I am a 68 yr old male who was diagnosed 5 yrs ago with cervical myelopathy (confirmed by MRI).. The neurosurgeon explained the risks of operating, but Dr. Ebraheim fails to mention the greatest risk in this video. It was explained to me that because it is such a delicate operation, there is a 15-16% chance of quadriplegia associated with this procedure. I would have opted for this operation if it was a 15% chance of death, but the specter of life as a quadriplegic was just too frightening. I've opted to take my chances for as long as I have left.

Dr. Ebraheim is a gift to humanity. I have seen 7 spine doctors in four different countries with the MRIs of my lumbar and cervical regions. Most of them had opined that the pain in my lower back was coming from the lumbar. After embarking on a lumber decompression surgery (Laminectomy), the pain was still there. In fact, it became worse. I was lucky to recently find a doctor that believes, like Dr Ebraheim, that the problem resides in the cervical region! This video has now given me more reason why I should proceed with a neck surgery. Thank you Dr. Ebraheim

Hi Nabil. I gone through your total VDO. I had also gone through the cervical surgery in March 18. My nerve was also pinched in C5-6-7 in MRI. In my case Legs got affected first hence walking got affected. Then @1 month later my right hand lost the hold/grip. In between these two incidents there was huge tingling which starts from neck & goes to fingures of both hand & legs. However later on right hand fingures lost hold. As of today my fingers gain the hold @80-90% but some times it gets cramped. But legs are still not recovered that much. Means tingling & numbness totally removed after surgery but legs still not. I can move, walk etc but not as natural. It's just like a video shown. While walking, working it gives pain & very irritating. One thing also cleared that the problem in legs is due to cervical Spondylitis. Since improvement in legs taking time & there is pain in lumber region while movement I thought there may be additional problem in lumber. Hence could you explain which medicine, treatment, yoga etc to be adopted to overcome this problem. Many thanks

Thank you doctor for an excellent dissertation and explanation of these conditions. I have stenosis in C and lumbar spine. Neck myelopathy with weakness and tingling in both arms worse on the left. My gait is affected, my bladder is affected although the 6 surgeons i have seen indicated that no surgery for me. Maybe !!!!! In the C spine at C4 to C7 if the pain and stiffness become unbearble. Ive been to Mt. Sinai, NY Columbia Pres , and other surgeons for opinions for 2 years. Non surgical spinal decompression has helped a bit. One surgeon said that not only would surgery not help me but could make me Worse. I experience constant temporal pain , TMJ syndrome, tinnitus, a d also have facet joing arthritis. All of this including 25 degree dextroscoliosis from a 1 inch shorter right leg after a femur fracture at age 15. 52 years ago when the body cast was removed, the fractured right leg did not grow, but the left leg did. Nothing was recommended to my parents to pursue such as a shoe adjustment for life. Any advice and direction is appreciated. Ive tried acupuncture, any sessions of PT, CHIRO and just recentky the aforementioned spinal decompression.

Range of motion (ROM) loss of 50%. So basically, if the patient could move one direction all the way when healthy, just think of it as half of that.

NO, you must get surgery its the only cure, once the nerves are decompressed your body and your nerves have an amazing ability to heal

U need to understand what’s going on, conservative treatment is basically like taking pain killers for a tooth that’s infected and needs a root canal treatment. Your spinal cord is being pinched which if untreated for a long time can paralysis you. When your nerves are pinched the signals between nerve points starts to disfunction and that’s why u get pain, just the nerve is up pinched then you basically reversed that process. if you take strong painkillers for this or and injected to numb the nerve, what your doing is allowing the condition to deteriorate further with less pain until the pain killers were off. Conservative treatment. Is a failed treatment but they offer it first if normally they have a criteria of who should have surgery, normally it’s patients that have this issue for more then 6 months to a year. I’d I was an orthopaedic surgeon. If I find any pinching in the spinal cord I will straight away offer surgery

Hi Anirudh! Both and degenerative arthritis throughout my body, was in shock because I had no prior symptoms, Except vertigo, Which debilitate me for several weeks and cannot move the trunk of my body, is it too late for me to have the surgery? Had several mri’s over all these years, Definitely progressing😳

I too had no real clear symptoms until this summer when my neck and shoulders were unbearable. Mine is quite advanced without any pain for years. I read that 75% of people who have this condition don't have pain for years, yet it continues to get worse. I'm meeting with my surgeon again to discuss the surgery.

I'm talking to Sonospine as we speak. It's a surgery that doesn't involve fusion. The technique should be more known to people. Why it's not baffles me. Hopefully I'm eligible for it cause I do not want to go they a fusion at all. Pray for me people and look into this. Maybe it will help you out.

@citizencain2383
@citizencain2383 2 жыл бұрын
@@AdventuresInLiving The method is similar to this. Sonospine has a even smaller tool that does the same thing. My operation was minimally evasive, he went thru the back of my neck and within 2 hours I was back at my hotel. Next day eating food and walking around with my wife. I'm very happy I didn't get a fusion. I would do it all over again if had to. Can't say if I will ever need another operation but I will try to do the things necessary to stay healthy this time around.
