Cervical Radiculopathy or Pinched Nerve in the Neck, is most often causes by either a bone spur or disc herniation. Posterior Foraminotomy, Anterior Cervical Discectomy & Fusion (ACDF), and Cervical Disc Replacement (CDR), all proceedures are designed to take the pressure off the pinched nerve. I've been dealing with cervical ridiculopathy for years. So many different PTs, chiropractors, injections, etc. I've done a ton of research on this, and no one has ever explained the pathology or treatment options more clearly and directly then you in this series. Amazing work. Thank you.

Great video series!! I just got diagnosed with cervical radiculopathy and the pain is intense. I have watched probably a dozen videos this weekend trying to get some insight for options. Your videos are far and away better than any of the other videos I watched. You make it easy to understand and I learned a lot. I wish you were in Arizona to treat me!

I did solve this by correct exercises and patience, + sleep and nutrition is important as well, but it can take 2-12 months before it disappears, I know what to do, but since I don't have uni in physio nobody trusts me, so until I get one, it will take 5 years 😊

So awesome that you show the options risks and benefits of all of these options. So many times a patient can have a laminectomy or forminatomy and avoida fusion but too often a doctor will just do fusions because they are quicker and pay more. They don’t really care about the longer term risks and patients results.

Thank you, sir! This video series has been extremely helpful for me. My local VA hospital is very good, but so far they have given me very little information. They told me to try ibuprofen, but they never said how much. I wasn't taking enough and I wasn't taking it regularly. I am now, thanks to you. Great videos!

Hello hope you're all doing well tonight. Thank You Dr. Brian S. This too is a video that is very important to me. Why? Because you've touched a topic that my sister and I are going through. My sister has had several neck injections and still suffers from her pain, as for myself I have sharp pains from my neck I can't turn my head around without pain I also have migraines brought on by my neck pain. Thank you for this series. It has helped my understanding of the condition I have. I had a herniated disc C7 T1 and arthritis, stenosis, and bone spurs with left sided radiculopathy and numbness in my 2 outer fingers on my left hand. I’m mostly healed at 12 weeks out, but still some radiculopathy with the pain in the elbow. My physiatrist ordered a nerve study to rule out impingement of the elna nerve. I will be scheduled for a steroid injection C7 to calm the nerve down. I’m so glad to find out most cases resolve with time. I’m hoping I don’t need surgery.

Thankyou ,I've struggled with this in the UK for 32yrs. So informative and clearly explained. I just seem to get reviewed by my consultant and never am free of chronic pain issues.Im unsure why surgery has not been offered?

I have been offered an operation on my neck, the doctors have decided to do this after much deliberation, I have already a plate in my neck. They have informed me the op may not relieve the pins and needles in my fingers, they are going to perform the op to reduce degeneration. I am also having a cortisone injection in my lower back, the doctors explained there is not much room to operate there, and it could be a very risky op.

What about a procedure done in Florida? Cervical endoscopic laser discetomy..surgeon goes through disc front small incision cleans out herinated disc bone spurs no fusion.

Thank you for your demonstration. 👏 I had my ACDF C6/7 in 2021 I suffer with degenerative disc disease and had cervical Myelopathy. Unfortunately I have been left with permanent nerve damage. They will be going in again to decompress C6 I asked what that means, I was told they will go in deeper. I wasn’t sure what that meant. I was told they had left some of the cervical myelopathy in the disc for whatever reason. I am so worried as I have new symptoms constantly including pins and needles and other sensations 🙏👍

Thanks a lot for further enlightenment Dr. Brian! Ur a heaven-sent NeuroSurgeon Doctor! I am having nerve stenosis and have a C2-C3, C4-C5 & C6-C7 problem.I also have bulged-discs per my MRI report .And I have a cist-like thing that grew on the left side of my neck. Is this cist-like thing just a spasm? Currently I am using a traction device of different brand but I am intending to replace it with Saunder's traction device, one driven thru a pump. I still drive a vehicle ( is this advisable activity)?

Doc you’ve been awesome explaining it in a simple way for people like like us who are not doctors. I have bone spurs and foraminal stenosis , it’s going to the cord and my lighthead and tinnitus are my symptoms I’m going crazy I feel like I got a hangover every single day , I haven’t heard you talking about my symptoms , are my symptoms normal or could it be something else ? Dr. Su, 2hrn I rotate my neck it sounds terrible. Like grinding and pop corn popping . Not good. I had a severe injury to my neck when I want 21 years old. My now deceased Husband pulled up on my head trying to kill me and separated my vertebrae and I had to get Chiropractic Care for about one year. Now my neck sounds terrible. Also I have major injuries in my lower spine.

Doctor question This information makes me feel good, I have c4-c5 a disk protrusion mild right neural foraminal narrowing. Disk deformity 1.5mm. C5-C6 Disk protrusion mild spinal narrowing, annular fissure is identified, disk deformity 2.6 and the same thing with Disk C6-C7 I'm just wondering if this can heal itself? I just feel mussel pain on the neck is frustrating please advise😔

Thanks for the information. I’m 6 yrs post a c3-c7 acdf. Before the acdf I’d had a stroke in my mid pons. My symptoms are worse. I’ve now been diagnosed with Cervical Dystonia and Occipital Neuralgia. Could my body not be tolerant of the fusion? Could my radiculopathy be worse because of the brain injury. I need help in peeling back the layers to these questions. I’d love to set up a phone conversation somehow to explore ideas of diagnostic tools and treatment. Thank you 🙏

I'm sorry you are not feeling well. There could be many reasons for persistent pain after an anterior cervical fusion. I would certainly start with a CT scan to document the fusion as well as an MRI to look for persistence stenosis. You can always make a zoom appointment with me by calling 415-927-5300. Thank you for taking the time out of your busy schedule to read this. I'm trying to understand why they're not doing anything about my neck other than injections in my neck, but they only last around 3 to 4 months. I have 3 pinched nerves in my neck, and they're not bone spurs. I had a double lung transplant 5 years ago, and the medicine from that had caused osteoporosis. My back has been messed up for about 23 to 26 years. It has gotten worse also. My doctor wants to stay away from surgery for now, but what I don't understand is why. It's not going to get better. My balance is horrible, I'm constantly running into things, and I try to do balance exercise, and I can't. I had to get reviewed by disability and it had to be done by one of their doctors, and he diffently saw what was going on with everything. If you can, please help me understand. Thank you

Hi sir, I’m 35 years old female in 2020 I started to get pain in my elbow afterwards went to see doctor they prescribed anti inflammatory for months nothing get changed in my pain went PT also nothing changed but little by little the pain started to elevated like from behind the neck and shoulder blade .. numbness.. tingling feeling in wrist and little and ring finger.. burning feeling in my behind the neck started.. they gave me shots in my elbow and shoulder.. for few weeks it felt good later started again … they did nerve conduct study it gave results last oct C8 Radiculopathy … I took ibuprofen with Tylenol.. they proscribed some sort of muscle relief medicine… nothing changed… still If I bent my elbow longer it started to aches and weakness feeling.. couldn’t carry big Stuffs in that hand ., even if I continue to write longer it’s getting worse pain.. can’t sleep that right side.. if I do fist tight it’s shooting pain goes out little finger to elbow. I’m using ice and hot massage… I don’t know what to do?? All the treatment are given by my pcp.. which department doctor do I need to see? Please help me out.. last two years this pain been there… if I don’t work or don’t do anything in that hand it was little relief but when I started it’s coming back..

Until now, doctors have done nothing but give me pain killers to manage the pain. I am wondering one thing, at what stage dose it becomes serious for a operation? I have been diagnosed with cervical stenosis. I seen a neurosurgeon at Audie Murphy VA hospital. After reviewing my Mri, Ct scans and x - rays.. He said my C3 is over lapping my C4.. I'm having major chronic nerve pain my neck, shoulder, arm and in my back. Numbness the whole 9 yards... He told me if my symptoms progress to go to the ER... My question for you is. When should someone with cervical stenosis go to the ER...

Hello. I am 68 years old. Diagnosed (MRI) as suffering CERVICAL SPINAL STENOSIS - significant at C5-C6 and medium at C4-C5. The main issue for me is numbness in my hands (palms, fingers), especially my left hand. In addition, I have some weakness in my left arm (compared to my right arm). Is this likely to be Radiculopathy or a Pinched Nerve in the Neck?

Oh I wish you were in Ireland.. got a new MRI and had review with nurosurgeon Monday after 16 months with pain specialist and rhizotomy.. MRI shows C5/6/7 issues and pressure on cord however he was reluctant to jump into surgery and is have a case conference regarding my old and new MRI.. I'm even more confused as it's been a really bad three years..

When two vertebrae are fused after a disc is removed, what does the relevant pair of nerves use for a foramen; how do the nerves branch out of the spinal canal?

If you take the disc out and do a fusion the exiting nerve still exits at that level there is just no motion there

Do you have a video on a Myelagram? I had a spinal fusion 3 months ago. I have recently began to have numbness in my thumb and halfway up my forearm in my left hand and arm. My Neurosurgeon has ordered a CT/Myelagram.

Patients after they fuse or have a decompression on their neck can return to full sport.

one question , once I have foratonomony would that prevent me to have a successful ADR placed at that level, would it affect the stability of the Artificial disc, or cause instability? Thanks a lot for the video

In theory it's reasonable but we don't have long term data on that.

@@DrBrianSuTheSpineGuy thanks for the prompt reply , I am confused as I have 3 level adr with M6c after 4 years c56 has a posterior bone spur protruding into spinal canal and one of my consults suggested it but if it would cause instability down the road I assume anterior revision is probably the more consistent way to go. However since my placement of the disc is still perfect on the scans I am not sure if the risk reward is there to remove the artificial disc . So either option does not look great at this point I guess. Thanks for the reply I appreciate it and love the videos

I have it all, so if i have a choice i pick chisel out the bone spur at the back of my neck so not to disrupt from part of neck. Just chisel out that damn spur and dissolve it flush it out or vacuum it out of my neck please.

Sir, are surgeries, where only herniatied part of disk is removed, popular in USA? I am thinking to undergo such a in Poland. Thank you very much for your contribution to make patients familiar with ale possible ways to treat their spine related pain. Something that was so scary in the past seems to expand in ways of treatment so much that it becomes more and more common and patient friendly.

Dear doctor,is the timing of the surgery matter? Can I postpone my dad's surgery for years or will it cause some damage to the nerve or so? Also, according to your experiences, What is the likelihood for someone to recover themselves given that person is 70+. Thank you and your explanation is brilliant. :)

That's a good question. If there is significant weakness less than gravity there is some benefit with earlier surgery rather than later. Otherwise you should not have surgery unless the pain is severely affecting your quality of life. As a disclaimer please consult your spine surgeon.

Hi. I'm currently 28 and have had CHRONIC (every. single. day) pain since starting when I was 17. That's 11 years of absolute hell! It started from what I remember around my left nipple (like underneath it, inside) and quickly spread throughout my entire left chest, into my upper back, and up the entire left side of my neck within about 2 years. Luckily (or unluckily) it has stayed stagnant since then and has not spread anywhere else. Unfortunately it affects pretty much everything for me. My breathing is impacted (it hurts to breathe on my left side, feels like my ribs are broken). It hurts to talk (constant pain in my throat and neck on left side). This pain has also caused me to develop massive anxiety. I am pretty much scared to death of people. I feel like they can see my pain and can tell I'm "different" even though I know they can't. I've been to pretty much every specialist. Neurologist. Pulmonologist. Cardiologist. General chest specialist.I've done 3 rounds of physical therapy. I've tried many different diets. Exercise almost every day (even though I feel like I'm gonna die when I do). Staying active. Stretching. Needling. Still after 11 years no doctor has ever given me a definitive diagnosis. I pretty much always get looked at like I'm a crazy drug addict psycho. Well just the other day I was feeling sorta fine and then I turned my head to the left and boom! my pain intensified tenfolds and remained the same throughout the rest of the night. This strongly leads me to believe that it is a pinched nerve somewhere. I'm making an appointment with an Orthopedic surgeon (I'm hoping this is the correct specialist to see, if not then I will ask him/her where to go) and I'm demanding an MRI. I'm guessing this sort of thing is diagnosed through an MRI. If not then please someone correct me. It's mind boggling to say the least that I have never been ordered one. Again, most doctors have always written me off for being "too young" or crazy. Well this time I'm getting one. Not taking no for an answer anymore!!!! Hopefully I'm close to getting rid of this shit so I can actually live again.

Not only do I hope I can get rid of this pain but I hope this is what it actually is (a pinched nerve). Honestly the not knowing part is almost worse than the pain part. My mind is always racing trying to think of what this pain is and where its coming from. The not knowing REALLY sucks!

@@escape2064 I’m 31 and I have early onset DDD from genetics since I was 18 and I have to get a multi level fusion from osteophyte complex and complete loss of disks bone spurs pressing on the spinal column and cervical nerves. The pain has just been debilitating. It’s unfortunate that a lot of doctors I’ve come across doesn’t take our pain seriously because “we’re too young” it’s just muscle strain 😒. You’re not alone! If you need to, go to the ER and demand a CT or MRI. Good luck to you

I'm sorry to hear you're going through that, I hope it turns around for you one day :) And I know tell me about it, I've been in and out of doctors offices for the past 11 years since I was 17. My favorite is when I get written off as a drug seeker, when I have deliberately suffering through my chronic pain for the past decade almost entirely med-free because I am terrified of becoming addicted to pills :) I don't even drink alcohol. I actually have an MRI on my chest soon (it took me 6 months to get one). We'll see.... I just hope it shows something even if it's not fixable!

@@escape2064 I always tell them I don’t want pain meds. That pain meds doesn’t fix the issue. I have tons of tattoos so believe me I can handle pain, it’s how long the pain lasts that I have trouble with. They have me up to 600 mg of gabapentin, supposed to help decrease inflammation of the nerves but when it comes to structural issues, no medication can fix it. And MRIs definitely show more then CT, let me know how it goes! Because my MRI didn’t physically show spinal column stenosis, and I’ve had all the symptoms of a pinched spinal cord, they are supposed to treat the symptoms, not the MRI. However, don’t lose hope if it doesn’t go exactly as you hope. You’re your own best advocate and don’t take no for an answer.

That is a marketing gimmick and is no different than using a small burr. Both remove Spurs successfully

@@DrBrianSuTheSpineGuy The demonstration by Dr. Cantor if the Cantor Spins Center in Florida would counter that. According to their video the burr cutter can cause damage if it nicks the nerve where the ultrasonic cutter does not. Very interesting to see conflicting views from spine surgeons.

Why does the disc need to be replaced if its a bone spur causing the issue?

It doesn't always have to be treated from the front by taking the disc out. Usually the disc and bone spur is the offending agent. Sometimes we can consider doing something posteriorly

I am wearing a Bone Stimulator. Apparently my bone didn’t start growing after the Fusion.(5,6 &7). On my last visit my Dr. Said it has started to grow, but, he did not show me the X-ray , like he had every other visit. Also, the top Titanium ring has begun to tilt down. He said he is not worried , if the bone begins to grow, it will cover it. I’ve played guitar for 65 years! And the numbness is beginning to affect my playing. How do I get your office phone number? What town is it in?

I did watch it. I had Ablation done on my Lumbar spine 3 yrs ago. It worked well for a yr. Now my pain DR wants to do the Cerviical spine. I had the Median Branch blocks done AFTER doing physical therapy. At PT they told me it hadn't worked but warned me, about Ablations. They said each time I had it done when the nerve does grow back there will be more nerves with it and eventually take up all the space ? Your opinion on that would be most appreciated! I am somewhat of an enigma patient. 11 yrs ago I was diagnosed with Renal Cell Carcinoma IV. They told me I had 5 other areas with lesions. I let them take my left kidney and did nothing else. 1 yr later, no lesions. I was a passenger in a car at 14 when the car was struck by a train. 1967 I could not tell my parents. I went on to lead a normal life with race horses at all the CA Tracks, lived in San Mateo at Bay Meadows. At 36 I had my only child and 1 yr layer had my first back surgery. Dr's wear thinking my spine looked like it was tipping off my pelvis. After surgery they told me my spine had already fused itself and wasn't going anywhere. At age 51 I was rear ended and had another back surgery. I am blessed to have had good docs, but I will be 70 this year and would love to live another10! < pain free? Any advice would be appreciated and I can surely find Marin County if need be. Thank you for ALL your great videos and your time. Near Seattle, Jackie Rogers

@@DrBrianSuTheSpineGuy I have a feeling I’m just continuing the activity that caused it so it’s getting worse, not better. It’s called making a living lol Do bone spurs ever get reabsorbed if the pressure/cause that caused them goes? I had another pinched nerve (affected other side) that hasn’t come back in spite of this opp side developing the cervical radiculopathy. I would think they would be both sending searing pain down my arm, and throbbing into upper back n base of skull-because I’m doing things again that strained my neck. I’m so tired of inability and PAIN
