If you want to see the massive difference, take a trip to one of the many hospitals who cater for medical tourism. We got a full exam and I mean FULL medical exam - less than $250! KL is considered quite small capital city compared to the neighbouring countries like Bangkok, Jakarta, Manila or Singapore with only 2 millions population and equal demographic 26% Malays 24% Chinese Malaysians, 15% Indian Malaysians and the rest are foreign workers and expats included Indonesians, Bangladeshis, Nepalese, Burmese, Pakistanis, Arabs, Iranians , Koreans, Japanese, Europeans, Brits, Americans, Canadians, Aussies, Africans, etc. Very cosmopolitant. However Greater KL is much bigger population nearly 8 -10 millions or 1/3 of total Malaysian populatiom where most people who work in KL live.
