I was under the care of several doctors. I only found a new physician because of their retirement, ect. I had hypertension for 25 years and diabetes for 5 years. Never was the chance of my developing this awful stage 3 kidney disease until it was too late. Please tell your fellow physicians to inform there patients of this pogression.

Sorry, I may be missing a point... PTH decreases phosphate reabsorption right? How is increased PTH (as a compensatory mechanism to hypocalcemia) supposed to cause increased phosphate levels?

in regards to aminoglycosides and renal failure/CKD, could you alternatively use amikacin since it is more lipid soluble and is metabolized more by the hepatic system?

Yes. You're right. I should have been clearer here. The CKD limits the ability of the kidney to excrete phosphate and reabsorb calcium, which would be a natural function of PTH. When I say the phosphate can still be absorbed, I'm referring to PTH-mediated absorption of phosphate from the bone.

+Paul Bolin If phosphate is high because of absorption from the bone due to PTH, then why is calcium low? Doesn't PTH also cause calcium release from bone?

+mandron With chronic kidney disease, the calcitriol levels are also low (calcitriol is made in the kidney). Calcitriol promotes calcium absorption in the GI tract. So with CKD you're not getting the same increase in calcium as you would otherwise have if the PTH went up in a healthy person.

Yeah that's right but in ckd phosphate gets constantly accumulated due to failure to exercise.

Because kidney is not able to excrete out phosphate due to kidney failure and hence causes hyperphosphaetemia.. Now because of this hyperphosphatemia fibroblastgrowthfactor gets synthesised from bones which activate the parathyroid to secrete PTH.. And function of parathormone is to absorb calcium and excrete out phosphate.. So the imbalance is somewhat compensated by this mechanism but not fully as the disease progresses 😁

Hi the video is interesting. So if an elderly person say 90 years has a low gfr of 22 should their gp wait and do another blood test to see what's happening? Ie symptoms of rls, headache, weight loss, sleep disturbance, lethargy. Or shod I push to see a nephroligist? It's re my mom. Or would they maybe leave her as ligasposswthout messing.

@@kazbaz8645 be careful/suspicious of this "gp" they are KNOWN and there are stories about them keeping secrets to themselves. heed this warning: if i were you i would take matters into my own hands and start having her drink at LEAST 1L of water a day. because i am beginning to wonder if gfr is something that can go up. DON'T trust this "gp" this person isn't acting in the best interest of the 90yr old person you know. the "gp" is withholding information!

baz don't wonder.....cut down drastically or completely on the sodium. up the water at LEAST 1L a day. the stupid "gp" ALREADY put her on the ropes by not telling her to do this sooner like a yr and a half or 2 years ago.....yet ANOTHER example/story of a USELESS "gp"........(typical shit head "gp's") smh

NOT TRUE I WAS IN STAGE 4 WENT TO STAGE 3 MY NUMBERS HAVE BEEN GOING UP AND DOWN IN MY GF R NUMBERS ALSO MY CREATITINE LEVELS UP AND DOWN WHEN MY GF R WAS 20 MY DOCTOR WAS GOING TO SEND ME TO GET THE FISTULA IN MY ARM FOR DYALISIS BUT HE WANTED TO TEST ME AGAIN MY GF R WAS 44 MY NUMBERS ARE STILL FLOATING MY GF R, NOW IS 31 WHERE AS 2 WEEKS AGO IT WAS 38 I DON'T KNOW IF MY NUMBERS ARE GOING TO GO UP OR DOWN BUT I'M ON THE TRANSPLANT LIST I HAVE TOOK THE DYALISIS CLASS I'M SCARED BUT I'M PREPARED I BELIEVE CKD IS DIFFERENT IN DIFFERENT PATIENTS
