Something that slightly bothers me is that (if I remember it correctly) earlier in the episode Chucks dad tests if he still has the Intersect by throwing something at him and Chuck has the time to flash and catch it even though he is caught off guard. But in this scene he already flashed and is highly focused on fighting but he cant react to the knife being thrown at him

I guess he can’t do two things at once. I think once he’s already flashes he’s focused on whatever it is he’s flashed on. So a flash mid battle wouldn’t work

I think if he needed to he would, he flashed when it got really close to him. For all we know he did flash. There are scenes I believe where you can see him flash about different things one after another so it is possible.

Tbh who knows if he was about to duck the knife or dodge it before Sarah stopped it

He didn't see the attacking motion, he had just turned around, there was no time to have the flash and block the move (bc he would need a different flash than the cqc he was using) that's why. Maybe he was about to flash, but i guess it would be very close if not to late

Also, when Sarah opened the door she let a lot of the gas out thereby reducing it's intensity in the room.
