would be cool if there was a service that would review MRI's with the patient. Doctor's only devote a few minutes to reviewing the MRI with patients.

That's a great idea- the only issue is that the MRI needs to be read with the context of an examination and history to make a good healthcare decision. The imaging on it's own is only part of the equation!

Hi need your help, I in chronic pain in the tharasic are due to an accident I had several years ago, I belive its nerve pain its unbearable, I have had MRI scan and xrays through our NHS but they was unable to find anything, I'm confident they have missed somthing how would I get them scans sent over to you to review in desperate need of help so I can move on with my life just need to know what's going on thanks

@@cavangarside6912 I'm very sorry you are in so much pain. You can request your MRIs images can be send to another radiologist with whom you have previously arranged for a second reading. You might be the one paying for it, but many times is good to have an extra set of 👀 reading it. You need to find a radiologist that specializes on spine MRI's scans. Good luck.

Finally! An MRI interpretation I can understand. Most videos on youtube expect that you have previously taken a radiology course and they go so fast through the explanation that I have to pause and google words. Thank you for the thorough walk-through. I am trying to understand my cervical spine MRI and while I still don't quite understand the axial view (mostly because we patients don't get the split-screen you do here), this definitely helps me understand what the different things in the axial view are so I am trying to match the axial view with the abnormalities I can make out on the sagittal view.

Great explanation of foraminal stenosis. I have severe left moderate right foraminal stenosis C4 C5. Recently had to take time off from work due to a flare up was treated with epidural steroid injections bilateral and RFA injections plus physical therapy. Physical therapist took a look at the mri and due to the severe narrowing and a 4 mm bulging disc with a ap diameter of 9 mm he said it’s a no go on Cervical adjustments as part of my physical therapy treatment!

1:48 thank you for clarifying a lot of videos out there were confusing me saying that axial view is top down and then telling me right and left are on opposite side. I was struggling to understand that because theoretically looking from “top down” was making me imagine looking from the top of the spine downwards. Many videos state axial view as top down but it should always be explained as “bottom up”

I had surgery last July for “moderate to marked” L5-S1 foraminal stenosis. My 8 week post op MRI showed “marked foraminal stenosis. I’m not sure if my surgeon was sleeping while I was sleeping… we both know bones don’t grow back that quickly! I had EMG/MCV last week now showing bilateral sural and peroneal neuropathy, with chronic neuropathy L2-5. The only diagnosis the neurologist left out is “it sucks being you!”. Tomorrow I get a repeat MRI. I am seeing a new surgeon specializing in failed back syndrome. I can’t stand, sit, walk, or drive without pain. My position of comfort was laying down, now that hurts too, yikes! Come on September, that’s when I finally have my appointment, unless I wake up paralyzed or incontinent, then it’s a trip to the ER. And yes, I have symptoms in so many dermatome distributions! I guess that’s why the last surgeon and his PA blew me off, thought I was insane, no, I’m in pain! I’m a disabled PA, I know when medical staff don’t really listen to patient complaints! Thank God I have a few really good providers so I’ve had my labs drawn to r/o metabolic problems, cardiologist has already done my annual testing so I’m good to go! Good informative video! Nice job making it easy for non-medical people to understand! And thanks, I got to blow off some steam! It’s been a long hard road finding another surgeon to look at me with 3 previous back surgeries.

How are you now? I got diagnosed with mild facet diseases stenosis and 3 herniated discs im 24 7 in pain can't sit or stand well I can but it hurts so much..mild! I can't imagine being moderate im going be moderate someday im trying to see what i can do!

@@johnnyrodriguez276 Have you considered the relationship to your bodies ability to heal and diet? A good start would be to stop eating all animal products including dairy. Also, stop eating factory processed oils. Eat only WHOLE plant based foods. Look into the work of Walter Kempner (rice diet) and consider John McDougall's approach to a diet which will allow your body to spontaneously heal and lower inflamation. I'm having severe spinal problems and may need surgery. If so, then eating an extremely clean diet will speed recovery. Even now I see an improvement after 15 weeks of an oil free, whole foods plant based diet.

Radiculopathy is describing the symptoms (numbness, tingling, pain) that occur as a result of stenosis (pinching of the nerve). :)

I have severe neck pain and pain in my trapezoid I was told many years ago I have two mild pinch nerve in my neck and I believe it has gotten a lot worst since then could this be causing stabbing head pains that comes and goes for few seconds? Very curious.

Thank U so much Dr. For responding. I've been suffering from back pains for years, but recently it got so back that it takes me about an hour to be able to straighten my body up in the mornings. I went to the Dr. She prescribed Prednisone and refered me to a specialist. When I saw the specialist they had an X Ray done. He td me that I have a spinal bone out of place. So he had me do an MRI. I have an appointment with him on the 23 rd. Hopefully It will not be so serious because I've known of simple operations and more severe. I would love for U to continue communication with me while I go through this process of my life since your knowledge and wisdom alone brings me much relief. Thank U so much. May God always be with U

What if your T12 is pushing 3mm on your spinal cord? I was in a semi truck accident and my neurologist wants to say my back pain is from a possible auto immune disease that I didn't have before the accident and currently going to be tested for.

Thank you so much for your easy to understand info. I have a question. I was rear ended in a car accident in October 2020 and ever since then I feel weakness in my legs. I was pricked with a needle by my doctor in my lower leg and I didn’t even feel it. I’ve been falling a lot so I am having an MRI done on aug 16th because it feels like things are just getting worse now. Is there anyway to know if it was caused by the accident? That might sound like a dumb question. I’ve always had some back issues. I have scoliosis. I feel like I can’t even live a normal life since the accident because my legs are so weak. My neck was also injured but is finally not hurting constantly. I can’t pick up my granddaughter and carry her anymore though it causes me too much and neck and upper back pain. I did one session of chiropractice. A week later I woke up and my lower back on the lower left side hurt so bad that I could barely walk and I was in agonizing pain. After a couple weeks it felt better, then a few days later I woke up and the pain was back but it was in the middle of my lower back. I just can’t keep living a life where I do barely anything. Before the accident I was more active. I took care of my three grandkids which was really hard by the way lol. That was mentally hard. My back issues are physically and mentally draining. I just want to feel better.

I was having pain in my lower left back area and the MRI revealed severe left foraminal stenosis at L5, I thought I might be eligible for arthroscopic surgery to clear some space but was informed yesterday that the "closure" is due to anterolisthesis, 7mm. So it seems like short of fusion, there is not a lot I can do. The pain occurs when walking mainly, standing as well. Sitting relieves the pain. I recently would feel pins and needles in the bottom of my left foot, but only a couple of times. I am really dejected over the news that I might be stuck with this. I exercise regularly without pain.

Ive had neck pain caused from an old injury. Just had a new MRI and it says my C2-C6 are dehydrated, straightening of the neck curve and a 2.3 mm disc bulge. Ive been waking up every morning with my hand or both hands (depending on how I sleep) on freaking fire!! Can you tell me what disc dehydration means? I'm getting tired of doctors not taking me seriously. I have almost constant neck pain and creptis. I was also just diagnosed with L4/L5 disc herniation and foraminal stensis. It states my disc herniation is 2.5 × 6.9....can you also please explain that size bulge as i cannot finfd any information on the ×6.9 part! Tganks for the very informative video!!

Best options (in the absence of red flags) are to typically start with conservative care such as chiropractic, physical therapy, etc. These types of care can reduce the inflammation and allow better motion. The "success" rate, in terms of reducing pain, is quite high!

I have quite severe stenosis at l3/l4, l4/l5 and l5/s1. It is causing pain/numbness all the way down both legs exactly as you described. What is the most effective treatment, besides surgery? I am currently in PT and it is barely helping at all. Steroid injections will probably be my next option. Are injections really helpful or just a band-aid? I really want to avoid surgery.

Hello, I can't comment specifically on your case. However, in general, research indicates that steroid injections can reduce acute pain short-term, but provide no "healing'. Additionally, they have the potential to deteriorate the joint over time. I'd recommend visiting a local doctor who can help with your specific circumstances!

Typically, movement-based care such as PT or Chiropractic is recommended before injections and is very successful at reducing the symptoms associated with foraminal compression. However, there may be cases when injections are recommended before PT/Chiropractic, although most guidelines are lowering their support for injections due to a lack of long term benefit. I would consult your local doctor to find a plan to help guide you to relief!

Can lumbar stenosis and cervical stenosis cause symptoms like vertigo? I've been dizzy for the past 3 months and doctors rulled out BPPV. MRI of head came out fine. I have a MRI scheduled tomorrow for the spine. I have lower back and upper neck pain. Anytime I lie on my stomach or twist I get dizzy. Neurologist said I can't get dizzy from pinched nerves....? Curious of your thoughts.

Very rarely would stenosis cause vertigo; but stenosis can cause imbalance and altered gait. Be sure to review your spine images with your local treating doctor!

If an MRI is ordered for back pain to ascertain information regarding bulging discs/pinched nerves and other spinal issues why are they performed laying down usually with a pillow under your knees? Most people’s pain is alleviated in this position so why are they not performed standing bearing weight which you would think gives more accurate details of what is going on.....

Ive had a Pinched Nerve for the last 4 years now since the day I got out of bed wondering why I was in so much nasty pain I could not handle,then I collapsed on the bedroom floor,with all my effort and strengh to get up of the floor,After about half an hour,I got to my frontroom,carefully sat down,wondering what the f is going on,The pain was in my left hip,few weeks later went to the Hospital to have my x rays done onboth of my hips,They both where ok,Then I went to physiotherapy another few weeks later after went to the Doctors,They done tests on me,saying ive slipped a few disks in my spine,causing pain to go to my hip,Then went to the Hospital to have an MRI scan done,few weeks later went back to the Doctors,He said I got two bulging discs in my lower part of my back,Been 4 years later now,Still getting Pinched Nerve,Always effecting my left toe on and off,Few days ago,I started getting shooting pain my lower back again

In general, cauda equina is seen as a result of spinal cord stenosis. Also, should be taken as a medical emergency that gets evaluated asap.

In a situation like that… with the forminal stenosis and the encroachment on the descending nerve roots by the disk, is that a situation where decompression would be expected to benefit? Or would it be more likely a surgical outcome that would address the issue? Or other treatment options?

Incredibly video and information. I have a pinched nerve in my left arm and it’s driving me crazy. Almost all throughout the day it hits me. I’ve had two MRIs done arm and shoulder which to my surprise came back negative so i had another done on my neck and back and there was something. Unfortunately my doctor are telling me different things even today i got another theory. The last few months it’s now affected my right arm now which freaked me out. Any advice please would be helpful like you don’t know. Thank you

Can it help me to see a chiropractor for pinched nerves that are causing pounding throbbing pain that extends down to my coccyx bone? I have an inversion table but I heard that if you have osteoporosis that you should not use an inversion table, is that something that you know to be true? Thank you

I can't speak to your specific case (this is not medical advice). In general, the pain/dysfunction associated with stenosis can increase or decrease throughout time depending on the tissue causing the narrowing. For example, stenosis caused by a disc herniation may eventually disappear is the disc reabsorbs. It's important to have a doctors advice who can provide a thorough examination and evaluation to determine the best course of care and potential outcomes.

I was diagnosed with Spondylolisthesis 2 months ago. Started with leg/foot tingling in both sides. Burning as well. Glutes and thighs tender at times. They tell me the L5 is compressing the nerves. Not sure if surgery would provide relief. These doctors do not really want to spend much time with anyone. It’s like “on to the next patient, get out” every time I go.

I've had an MRI on my neck and was told I have a few bones spurs, and C6-C7 I have severe foraminal stenosis. I've had a Nerve test that showed multi layer pinched nerves in cervical and lumbar spine. Neurosurgeon said surgery isn't necessary on neck. Had an MRI on Lumbar spine and waiting for those results. I have a lot of pain in my neck and low back, but only symptoms are numbness and tingling in my hands and feet. Any suggestions as to what's causing this?

Hi Brian- impossible for me to give medical/health advice online- but typically foraminal stenosis is caused by either disc, bone, or ligament pressing on the nerves as they exit the spine.

I've been diagnosed with a pinched nerve due to a twisting injury in my Lower back I had an accident at work January 23. I been seriously suffering left lower back all the way down my left leg to and included my foot. I'm waiting for tests I also have left buttock blader failed and waiting for bowel investigations the latter two symptoms started the day of the accident and bowels the next day. Hello….I have a partial footdrop caused by herniated disc…..3/5 strength. My MRI is similar in showing no white space near L5. I have no pain just numb toe and the footdrop. Surgeon can’t make up mind whether to perform an opp as he is not certain it will resolve the footdrop. Tried steroid injections and no difference. Can physio help or I am stuck with footdrop or the risk of an operation which may not work? It’s been 11 weeks and footdrop not getting worse but also no better ?

Good question - I would continue to work with your local healthcare team as they know your history, evaluation, and imaging. As a generality (this is not a diagnosis or recommendation for your particular case), foot drop should be addressed sooner rather than later through either conservative care (if improvement is gained) or through more aggressive treatment.

Hi! Two months ago I woke up with pain in my right calf which quickly became pain in my front right thigh. The pain in my outer half front thigh has not left since, and I have also had temporary pain in my hips, buttocks, and left front thigh. I went to a chiropractor and had an xray. He said I needed an mri, but I think he said I may have an issue with L45 or something. A couple of message boards are online with people having the same issues as me, but they don't have an answer. Can Foraminal Stenosis on C3-C4 cause chest pain? I am going through long undiagnosed left chest pain which comes and goes and the pain level varies. Recent Cervical MRI says "right uncinate spurring which causes moderate narrowing of the right neural foramen". I found in some journals my symptoms sound more like "Cervical Angina". Is it possible?

Hello Kishan, its not possible for me to speak about your case without a complete history and examination. What I can say is that chest, or thorax pain, can occur when there is compressions of the exiting nerves from the thoracic spine but I havent heard of it coming from the cervical spine.

Im 43 and this is my 1st experience with back pain of any sort really but had mri Thrusday saying "Multilevel disc dengeneration" C3-c7 saying on C5-C6 "Disc osteophyte complex and uncovertabral hypertrophy resulting in moderate to severe canal stenosis and slight effacement of the ventral cord .Severe bilateral neural foraminal narrowing." the other 3 disc say similar things that one being probably most severe. Really havent had chance for anyone to go over this with me ive been referred to spine specialists through a email. I really dont even know what it means other than the.pain ive been in last 6 weeks. Any advice would be greatly appreciated!

Hello, it's impossible for me to comment on your case. However, in general, degenerative changes are a normal part of the aging process. Images are only part of the equation- your pain level and functional ability is what make the imaging findings "matter". We all have plenty of "not perfects" on images, the goal of the doctor is to sort through them and discover the "problem".

Also in right arm n shoulder similar issues where i have lost strength and in my right hand very noticeable loss in thickness and muscles in hand to the point where even though im right handed and always my right hand being slightly bigger than the left to now my left hand is thicker now. The lateral you used was not equal! There is obviously a scoliosis since the T13-L3 show a whole vert body and the L3-5 show a partial body the IVF's are not equal because of the scoliosis and can NOT be equated! You must go sideways a slice or two to get equal views of the IVFs. On axial go up or down to view the whole IVF because the nerve normally leaves in the upper portion of an IVF and looks pinched in the lower portion of the IVF leading to false conclusions. The white in the spinal canal is fat pad and the grey is the actual nerves of the filum terminale after the cord covering has stopped and the remaining few nerves are preparing to exit the IVFs. Not much "cord" exists at that level. MRIs and CTs are limited by 2 dimensions just like plain films so you have to look at the torsions and bends in the spine to get the proper interpretations. Too simplistic! Most irritation at this level is NOT from discs but from "Hyperimbrication" of the zygapophseal joints, or 'jamming together of the facet joints' and not from disc bulges. Herniations with extrusions will put pressure on nerves coming out of fixed sacral nerve outlets but Not affect the sciatic nerve.
