Everyone fears to go againstvthe system as it will mean facing punishment. The regime wants everyone to see how bloody and relentless it is towards people it considers its enemies. If you are considered a problem or even suspects to be a problem you live in constant fear of brutal punishment (torture and slow and painful death).
