Of course you said concrete floors. FYI, those earthen plaster exterior walls won't erode, if you coat them with boiled linseed oil.

Can we add quarry dust to lime instead of sand? If yes, what is the ratio and will the wall be breathable?

Could you raise the doorway bottom so that you don't have to dig out the inside floor? Say you just added cob or another filler to raise it, then you would have the height needed for the gravel, vapor barrier if needed, and and cob layers

Yes, there are several different configurations for the foundations. This is simply one example shown.

Concrete, wood, or stone can be used for the lintel beams.

Nope. I like to use a concrete slab and foundation, so no rodents can come through the floor.
