Sort of ironic, but their conversation was partially to blame for causing the crash. They were too busy BS'ing and not following the preflight checklist and making sure the aircraft was in takeoff configuration.

There are several scary ones including the DHL and Russian plane colliding in mid air and all you hear is wind whipping around the broken up planes. The Japan Airlines crash is haunting too where you hear the captain yell ‘powah powah’ right before it crashes.

I SAW THIS HAPPEN!! I was a kid and my mom and i were driving through the airport...i remember seeing the plane on fire and a bunch of fire trucks and police driving to the field...it happened during take off...crazy

Especially when they go on to say it'll give their wives and children something interesting to listen to. I'll bet! This crash happened due to the pilots fucking around and not taking their jobs seriously. They misconfigured the flaps and the airplane was not able to achieve lift. It flew approximately 22 seconds and crashed. These clowns on the recording survived and were promptly fired. IMO they should be in prison, people died.

The flight attendant you can hear talking to the pilots on the tape is Dixie Dean; the most senior flight attendant for Delta Air Lines at the time. She was killed in the crash.

A year after the crash, Captain Larry Davis, Co-Pilot Carey W. Kirkland, Jr., were all fired from Delta Airlines. So was the flight engineer, Steven Judd. However, Judd was reinstated after it was determined by the NTSB he wasn't involved in the conversations not-related to the aircraft's operation and after he filed suit against Delta for wrongful termination (both sides agreed to reinstate Judd and the suit would be dropped).

I once heard the CVR recording of the Western Airlines DC-10 that crashed in mexico. One of the pilots screamed in terror, far more haunting than this one. Especialy because you know they did not surive where as the chaps in the cockpit on Delta 1141 survived.

1141 Several passengers said that they saw the Flaps WERE deployed . This was a trijet airliner . There is a very strong suspicion that the rear engine was never started , ie it was left dead for the taxi out to save fuel and then because of the lack of cockpit discipline it was not fired up in the time before take off clearance. no thats wrong. 1141 crashed because the pilot forgot to extend the flaps for takeoff, and the cockpit warning sytem failed. In the longer version of this you can hear the piot flirting with a flight attendant right before takeoff. This crash resulted in the "sterile cockpit rule"

Those screams remind me of the CVR of Western Airlines flight 2605, which was quite gutwrenching to listen to. There you could hear one of the pilots shouting something similar "Jesus Christ" then intense screaming till you hear the sound of impact.

No they did not set the flaps correctly as they were distracted with the attendants and did not do a standard check list.... The one in the Micro-burst was another crash in Dallas 4 years earlier of another DELTA L-1011. That one flew into a microburst, and the sky blew it to the ground.

That´s right. The recording available on the internet and therefore here, has been cutted. Either They are talking about "leaving something for our wives and children to listen to" much earlier in the flight, because the crash does not follow direct after the conversation, or it is another flight. It is -for me- the most disturbing record available, because the screams at the end are really gruesome. RIP to the souls lost.

@MrBkey No, this is real, Delta Air Flight 1411 in 1988. The recording's been widely distributed by the media. Besides, there are other CVR recordings out in the open on youtube, and I believe there's an album by the German band Rammstein that includes the last 40 seconds of Japan Airlines flight 123, the worst air crash involving a single airplane back in 1985.

This take is edited. I've heard the whole version in flight training at an airline. It is the passengers screaming at the end as the airplane is breaking up. Tires do not squeel on jets, because all jets have anti-lock brakes. A lot of people died on this crash.

Plane crashes happen for remarkably simple reasons. In this case the pilots simply forgot to lower the flaps for takeoff because they got distracted by this conversation. LAPA flight 3142; a Boeing 737, crashed on takeoff from Buenos Aires in 1999 for the exact same reason. Amazing how history repeats itself!

They have clean cockpit rules now. No chatter is allowed during critical parts of the flight like takeoff and landing. No flight attendants in the cockpit either. This crash was part of the reason for those rules.

I know a Monarch training captain who states that they stick religiously to their proceedure, but according to a BA training captain I know, BA are not so strict about it. I believe his words were "Not so sterile in my experience, nobody really pays attention to it. If you can't talk and fly a plane, you shouldn't be flying a plane"

**There was an engine failure also on takeoff..you can here the warning sound at 0:33 and then the pilot announce it at 0:36..And these pilots were not talking about the dating habits of their stuartesses..they were refering to a crash in Denver that was the same situation actually in which they were killed after not setting their flaps, while talking about one of their flight attendants.

The co-pilot forgot to set the flaps. The pilot applied power too late and didn't lower the nose; and the warning horn that should have sounded because the flaps weren't extended, wasn't working.

That's only partially true. There are many circumstances where a controller gives an ambiguous or erroneous instruction, but the flight crew had all the information available to them to detect the error. The PIC is called that for a reason: he/she has ultimate responsibility for the safety of the aircraft, and has absolute authority to disregard ATC if a safety-of-flight issue is suspected.

Yes they did not follow proper proceedure and have the door closed to the cockpit while doing the standard routine. Having anybody on the flight deck besides the flight crew for preflight and pre-take off is strictly forbidden and this is why they forgot to set the flaps.

I believe two of the three pilots were terminated. The engineer said he had no memory of the accident, (which is possible). He was not fired because of that defense.

The flight attendant's voice heard on the CVR is the voice of the purser flight attendant Mary O'Neill, NOT Dixie Dunn. Dixie was supposed to be the purser for that flight, but she and Mary had changed places and work duties earlier that morning prior to the flight. Mary survived because she was in the front of the plane when it crashed. Dixie was in the rear of the plane with the other flight attendant who perished.

Without proper extension of the flaps, the wings would not be generating sufficient lift for the aircraft to become airborne. "and they didnt catch it when there speed wasnt coming up fast enough?" If the crew believed that the flaps were properly configured, then they would have believed their airspeed to be sufficient.

question was that where the audio recording stopped or did it continue to record the entire crash until the plane stopped moving.. You hear screams then begin to hear the scrapes of metal and then nothing.. just curious as I would think the plane would have to be completely destroyed in to pieces before the electrical systems failed (engines / batteries) before the recorder stopped. Sounds like that was just the initial hit

Why is there a gap in this recording? It goes from the F/O calling 80 kts directly to him saying [en]gine failure. All the interesting stuff happens during that gap.

@nzAVIATORnz First off, his errors are what caused the crash in the first place. He forgot to set the flaps/slats to takeoff. His actions COST 14 lives!

What's crazy is how common it was to pull that damn thing. Pretty scary knowing that your life is in someones hands who is up front fiddling with things just to keep the cockpit a little quieter. Kinda like pulling the seatbelt warning from your car and then having a bad accident.

@mlex169 What are you talking about? They went through a microburst coming out of a t-storm. A gust of wind hits the earth's surface directing it everyway. They were hit buy it resulting in increased airspeed for them so they pulled back the throttle and lowered the nose. When they went through the center they were pushed down by the wind and out the other end now they had a tail wind probably over 100mph. Now they had to add an extreme amount of power to keep air flowing over the wings.

Flight school 101 stuff. Never talk about flight attendants or the cockpit voice recorder before a flight. The "death screams" are what make the recordings so interesting. Some pilots scream like girls, some just curse, and some tell the love of their life they're sorry. The personality of the pilot shows when death is imminent.

This copy probably came from the media. They edited it for broadcast to illustrate the failure of this crew to observe the sterile cockpit rule which in turn led to the pilots forgetting to do the taxi checklist (which includes configuring the flaps & slats) resulting in the crash. The leak of this recording provoked such an huge outcry by the pilots' associations and victims' families that the release of CVR tapes to the public became illegal. That's why we only get partial transcripts now.

It's in fact misinterpretation of a failure. The cause was incorrect flaps and stats settings for takeoff. It's not completely pilot error (the cause is though) but a take off config warning should have gone off but didn't also issues with Delta's training played a role.

He clearly says, "we've got en engine failure! We're not gonna make it!" Dont know mucha bout planes but perhaps the engine failure was caused by the improper takeoff configuration of the flaps.

No the flaps being retracted caused the plane to have no aerodynamic LIFT! Engines were fine!

The more you know about aviation, the more you realize that these scenarios play out over and over in one form or another. Proper CRM and sterile cockpit below 10,000 feet are relatively recent additions to the accident prevention arsenal. I don't think this particular tape is all that informative, if you think of it from the perspective of the people who perished, and the Captain who did'nt.

Pilot-error caused the deaths passengers on this flight. NTSB statistics: from 1984 to 2007, the number of pilot-caused crashes/deaths per airline: United: 0 crashes, 0 deaths. Continental: 1 crash 28 deaths. USAir: 3 crashes, 161 deaths. Northwest: 2 crashes, 164 deaths. Delta: 3 crashes, 198 deaths. American: 5 crashes, 505 deaths. These statistics reflect pilot-error, or pilot-caused crashes, and can be derived from NTSB accident reports.

urm, if you aren't getting off the ground because of your flaps and slats configuration but think you have engine failure, that is a misinterpretation. I'm just reciting what the ntsb report says. As I prefer the best source and the least speculation.

People's perception is distorted between individual deaths vs. a large number at once. Maybe the numbers are inaccurate at times, but that has little effect on the extremely disproportionate ratio. The 04/27/12 NTSB report on US commercial airline deaths for the past 10 years added up to approximately 2 deaths per 100 million. There were 0 reported deaths for '07, '08, '10, '11. By these numbers, you're about 50 times more likely to get struck by lightning than die in a airliner crash.

I worry about missed inspections. A delayed inspection killed 85 people on Alaska Airlines a while ago. Faulty jack screw failed due to lack of lubricant. Threads on the screw just splintered and the tail fin broke loose and the plane nose-dived into the Atlantic killing all aboard. Investigation found that this accident could have been avoided if maintenance had been done at the proper interval. They put off inspection to save money. Money before safety..that's the airlines' motto.

This is edited. First part is the non-essential chatter, regarding events preceding the November 15th, 1987 Continental Flight 1713 that crashed right after take-off due to icing on the wing surfaces. This is not the full Recording of Delta's 1141 the take off roll - I believe it's edited to just after Vr is achieved, and rotation begins - the sound of the plane's tail hitting the runway is heard, i believe due to the improper setting of flaps and slats, and consequent insufficient lift provided, the plane rotated at a higher than normal AOA, causing the tail strike on the runway surface. Is that correct?

@GPMSharpCommand That was a Northwest plane out of DTW back in the 80's. Only survivor was a little girl who was a lap child and was speculated to have been saved by being a lap child and being cusioned by her parent.
