In terms of onset, according to Harrison's p.719, CAD is characteristically a chronic disease. For me this arises a question since you mentioned polyclonal antibodies are related to acute onset, and the patient in this vid's case presented acute onset as well. I agree with what you mentioned by polyclonal antibodies show in acute situations as in infections, but this might not be case. I'd appreciate it if you could clarify if CAD is chronic or acute. Thanks!! I really love your videos :)

Cryoglobulins - Cryoglobulins are antibodies that form immune complexes in the cold; they generally do not interact with RBCs.So no hemolysis.

think you very much for all these lactures and I would ask you about a case of patient with Sickle cell disease came for blood transfusion but all the blood are incompetible and the coomb's test is positive .what is cause of this combination and how can treat this case(HB=5.0g/dl)

@@MedicosisPerfectionalis of course its real patient 7yr aged and its stable for long time with Hb between 7_and 8g/dl suddenly his HB dropped to 5g/dl. blood film show marked agglutination,sikle cells ++ and few sphercyes. reticulocyte more than10%

I am literally preparing that video right now 😁😊...In brief, the direct Coombs test detects antibodies (or complement) bound to the RBCs surface, while the indirect Coombs test detects antibodies that are free in the plasma. In cases of warm or cold autoimmune hemolysis, the antibodies (IgG or IgM respectively) are bound to the surface of RBCs, that’s why we use the direct Coombs test...Hope it helps!

Why is it important not to treat with Steroids, Reason I ask is that my Mother has CAD and was on 80mg Prednisone and was lowered to 40mg. She is having issues with her O2 sat and the cause is presumed to be from the Prednisone. We have asked to be taken off of this but were told she needs to stay on it.

i think cold agglutinin disease causes extravascular hemolysis. its cold hemolysin disease(PCH) that cause intravascular hemolysis
