Great question! Would love to hear Ben's opinion on this. The tibialis anterior is one of the antagonists of the gastrocnemius en soleus muscles. The tibialis creates dorsiflexion aka pointing your toes to your shins. I think it is important to maintain a balance between these muscles to prevent the gastrocnemius en soleus from becoming too tight. An example is the squat: as your knees travel forward you need to have good dorsiflexion or else your heels will come of the ground and the weight is shifted on to the ball of your foot. You can train the tibialis during calve raises by really focusing on stretching at the bottom en consciously contracting the tibialis. Same as only training your triceps and no work for your biceps. That would create an imbalance in the elbow joint and possibly in the shoulder. I'm no expert on this so I may be wrong.

Where is the optimal part of the foot to place on the bar/pad? - Here your feet rest on the pad all the way behind the balls of your feet. This could lead to the feet pivoting and not really getting maximum tension? Behind/On/In Front of the balls of the feet?

You're right but he clearly says "this pad is perpendicular to this one" not that the pad is perpendicuar to the force of the weight. He probably meant what you said, though.
