I just wanted to say thank you for this video. I am an RN and work inpatient psych for Active Duty Military. I suspected for years that I had ADHD. But didn’t really in-depth knowledge of ADHD, even as a nurse. MH is always just brushed aside. Well, I fully understand it and now know in-depth knowledge of MH. With that said, my daughter was dx with ADHD. Naturally as a nurse and curious, she gave me two Ritalin pills for 2 days. I was absolutely gobsmacked when I realized THIS is how normal people feel and then came the my God, how have I been struggling like this all my life? Moment. I finally saw someone and dx. The NP gave me Strattera (No Bueno). Didn’t work. Had another appt and mind you, I told them my daughter has it & takes Ritalin due to familial genetics with meds. They rx Buspar and Wellbutrin. This particular NP was very dismissive. Went to a GP with documented dx, he gave me Vraylar for 7 days. Did nothing. I was actually pissed bc I do have knowledge and trying to talk to them. I don’t need the Vraylar. I was actually not being listened too and being in the medical field, I already knew what he was thinking. I faithfully tried all the meds. Went to the 7-day f/u from the Vraylar. Told him the truth. I purposefully brought my sister to articulate for me bc I was flustered bc he asked me, “you’ve tried everything, what do you want” out of his frustration. Idk how I did it, but I told him, “everybody is trying to play these f-ck, f-ck games experimenting with meds they and I know won’t work”. “I want my life to be normal”. I want the first-line, gold standard medicine that works for ADHD. “I want my f-ing life normal so I can function. Told him I want Ritalin or Adderall (which my colleague psychiatrist recommended). I apologized for cussing. I’m a 47 year old woman, I’m not crazy, I’m not gonna use/abuse. I’m not a drug seeker. His whole thing about not rxing is bc it’s a schedule II drug. Told him I didn’t care. I’m telling you, it was an absolute fight with many appts just to get Concerta. I will try it due to basically same as Ritalin. I mean like pulling teeth to get the right meds just for ADHD. And I’m a nurse!! I can’t even imagine what non medical people go through. I just wanted to thank you and share my story with anyone who reads it… please keep fighting and advocating for yourself and bring a trusted person with you to advocate and articulate for you, if needed. The Concerta won’t be ready for a week. So, when I get it in a week, I’m confident my life will be changed for the good and functional. ❤❤🫡🫡

I completely understand how you feel. I understand what meds I need. But I don't want it to seem like I'm out drug seeking. For the last 25 years I've been misdiagnosed. Everyone has put my on these $hit depression drugs that have had no effect. It's like these people don't care. Every depression medication to me has felt like a placebo. Some people understand and can do there own research on what their problems are. The drs should listen and give them what they believe would help the most and monitor it. They just like to d!ck around with people's lives and have them wait for decades to find the correct medication. It's unbelievably frustrating to listen to them say oh just give it another month to kick in. It's been 20+ years.

My Concerta withdrawal was TERRIBLE. I had a doctor leave the state and I couldn’t get anyone to fill my prescription for a month.

@@marshallstrander3922 It was a while ago, but I was easily agitated and would suddenly jump from calm to upset, either super rage or emotional weeping. This was at work. It was not a fun time for me or my coworkers, lol.

As a 40yr old just starting my journey, thanks for being an open minded parent. My life would have been much different if I had had adults like you around when I grew up. I can tell you having had a good time in my younger years, my experience with methylphenidate is as follows... If you have ADHD it's not a party thing. Too much doesn't feel good. The right amount for me is low, I don't feel it kicking in or ramping down. This is ritalin short acting I'm speaking of, all i notice is less background noise. One thing recommended by Dr Russel Barkley, a very well known guy is to get your patient (your child) to have a no-risk nap in the afternoon. If the dose is right, they should be able to sleep. I dont have trouble sleeping at 11 if I take a ritalin at 6:30pm... I am not a Dr, that's only a suggestion. I'm starting Concerta tomorrow, unfortunately there isn't a lot on here. I am on it right now but my case is different than others. Suffered closed head injury years ago and have lack of energy issues in relation. It has increased my energy but hasn't affected my hunger or feeling hungry all the time.

Many thanks for posting the video - very interesting. A few points I thought I'd mention: You talk about taking holidays from the medication but many KZbin conferences where professionals who are lifelong experts and practitioners in ADHD advocate for continuation through weekends so that the patient doesn't have strong side effects on the Monday morning, which are unpleasant due to the fact that they've become used to being unmedicated over the weekend. You also didn't mention that Concerta is just one type of ADHD and name other medications are available i.e. amphetamine salts etc. I don't believe you mentioned about how Concerta should be taken early in the morning to ameliorate the difficulty in sleeping side effect. You didn't discuss the whole "pills don't build skills" argument to highlight that Concerta should be a part of a wider approach to manage the symptoms of ADHD. Strategies such as using timers/reminders, externalising thoughts i.e. writing ideas, or to-do lists into a notepad or similar along with a whole raft of others are crucial for leveraging the maximum benefit from the medication. I believe you also didn't highlight the fact that methylphenidate has been in use since 1955, so is one of the most established medications which we know is relatively safe to use even over the very long term. Also, methylphenidate doesn't create dopamine, rather it inhibits the reuptake of dopamine and norepinephrine (recycling what your body is already producing).

Did you expect her to say literally everything about this medicine in the space of 5 minutes? As well as saying everything about all the other ADHD medications? This video is a short & snappy educational video- she would be talking for hours if she were to meet all of your expectations- most people with ADHD such as myself prefer short & snappy videos like this so we don't lose interest or focus, and can take in the information easier. She shouldn't need to mention how people are to take it in the mornings either as their doctor will tell people how to take it when prescribing it to them. This isn't full medication booklet, this is an educational clip.

I originally wrote my comment because 1) She may not have known one of those points and it helps her professionally 2) Other people want to surf the comments section for more information and it helps them. The tone of my comment is curt, I wrote it in a rush with no editing, and could've been more politely. I also personally think having a break to reset Concerta can be really effective (I have one day off per fortnight). If I don't I get a symptom buildup and the efficacy is reduced. I also have Omega 3 daily plus ON Gold Standard Preworkout twice a day which contains L-Carnitine, L-Tyrosine, Caffeine and Creatine, which together allow me to get very close to full symptom relief of ADHD. (And yes, I do use strategies on top, when needed).

@@YT_User- Thanks for the clarification. I understand you are trying to help but the woman in the video is a trained medical professional and knows what she's on about. She will do her learning from the sources that she trusts rather than the unreliability of an anonymous commenter who could've gotten their information from any form of source. And yes, I agree L-Tyrosine along with amino acid complex and other stimulant supplements can help boost energy and focus for some people with ADHD- but it won't help all. 6 years ago I took Tyrosine 3500mg per day for about 2 weeks because it really boosted my concentration and got me out of a depression- but you know when they say "if it seems too good to be true, it probably is" exactly was my case. I had an extreme crash and it brought on a psychosis that lasted for 3 weeks. Never suggest anything for anyone just because it worked for you or someone else, because you aren't a doctor and you don't know how it will effect other people that are built differently. Be careful with tyrosine, that stuff is no laughing matter.

@@LordSolidusI Here's my response: 1) "Just trust the Dr" mentality is the reason I DIDN'T get an ADHD diagnosis until 15 years later when I challenged the expert because I trusted my instincts. That's why there are second opinions in the medical world. 2) I have never given medical advice, I have shared viewpoints of medical professionals from seminars, lectures etc. I can actually reference the source for each comment I made and you can interrogate their credentials. I even disagreed, myself, with the "avoiding medication holidays". I need a break personally. It's more a discussion/debate, it's not me trying to make everyone agree with me. 3) I'm sorry to hear about your awful experience, it must have been terrifying. I hope you have managed to find what works for you. 4) Sharing what I take was done to communicate that I actually use this medication as part of my symptoms management regime. 5) I have never advised that anyone takes anything. If I said "You should..." or "This will help if you take .." then I would agree with you. Any substance can be lethal depending on who takes it. Even eating some peanut butter can kill certain people. Never trust "someone from the internet". It's your responsibility to make your own decisions. 6) Thanks for replying to me (and being respectful). My comments are there just to challenge and make people think for themselves more once they've got all the information (including from professionals). I clarified a couple of points about how I would like my comments interpreted but your points are all valid and true. It's okay to different opinions. We all experience ADHD uniquely, certain aspects are better than worse than others. My doctor himself told me that ADHD medication titration is unscientific and nobody knows for sure how and why it works and it's an anomaly having to use subjective, rather than objective data from their patients to decide the correct prescription. Thanks for the advice about L-Tyrosine(!) I'm researching it myself now.

@@YT_User- I appreciate your views. I apologise for misinterpreting some of your statements. It's always good to share eachothers views and opinions. I also agree with you- doctors aren't always right; it is usually the one with the first-hand experience that knows what's best for themselves. I myself was never prescribed ADHD medicine until I was 20 (because of the abuse-potential statistics, influenced by selfish people abusing the pills which makes it harder for the people that need the medicine to get a prescription), and I was let down a lot by doctors throughout my life. I am glad that you have found a regime that helps you, and I hope you continue to use your amino acid supplements responsibly. You have a good rest of your day 😊

The information is great, and you are a beautiful presenter, but please boost your voice track up higher and consider a closer mic for added proximity effect. You have a very delicate voice that needs proper amplification and representation. Heavy compression and EQ would help with the details as well. Other than that, thank you for the immensely valuable info. I just started on Concerta and I DEFINITELY feel the initial boost and fleeting moments of focus after that. I'm on 27mg and it sounds like I could go up a little or possibly replace with Ritalin for a more consistent effect for my work day (if my psych agrees, obviously). I feel no side effects other than a little dry mouth (other meds are doing that as well), and I do not become irritable as it wears off, but that could be due to the gradual tapering.

This medication is the only thing that has treated my depression and it makes me think and feel completely normal, I can not explain because everything that I have researched says it isn't for depression.

Sometimes depression is the result of other issues such as adhd so managing the issue causing the depression could solve the depression too rather then trying antidepressants.

it's so understandable because adhd people tend to have depression (or depression symptoms) because having ADHD is hard and leads to depression too. so your ADHD is the root. (genetically environmentally) (watch the video: if you have adhd, you are likely to be depressed. something like that. and the video is 2 hours. believe its worth it. its from a live video. i watched that video part by part cuz i have adhd and couldn't finish it fast cuz its so long hahahahah

Hi, I have been taking concerta for the last eight month, I am on 36MG and it does not help with my ADHD symptoms, I am debating whether or not I should switch to Vyvanse or Adderall or go up on 54MG of Concerta, what should I do at this point? I also take trintellix and seroquel.

Do you know if that later 70% release of the second dose is released all at once? Or does it release that throughout the day? I’m finding that with concerta I feel better later on in the day but mornings are still very rough so I’m thinking a different brand with a higher release than 20% in the morning

It is supposed to released through out the second half of the day. Not all at once in the second half of the day. If the medication is not working well for patients due to the timing of effect, there are other mechanisms that may suite your metabolism better. Let us know if you tried to switch to a different medication and if it worked. -Dr. Rob

I started this medication but have decided its not for me, but this is purely my choice. After being diagnosed as having ADHD/Autism last year at the age of 50 I have only just learned who I am. When I went on Concerta it stopped part of my ADHD in terms of a racing mind, but I decided that to be on medication like Concerta is yet again making me mask my flaws/what makes me as a person tick . I get that people will be okay with that. But I want to be me, not some person who is masking by using drugs

What's your opinion about Azstarys? Also do you find that it is less common for your patients to switch from Vyvanse to Concerta?

Great questions! I do not have much of an opinion yet about Azstarys. This is mainly because it was just FDA approved not too long ago. Most of my patients have issues with insurance covering it and even with a coupon, it is still costing $300-400+ for a 30-day supply. Therefore, I have not been prescribing it given that there's an abundance of other stimulants out there that are much more affordable. I have had multiple patients switch from Vyvanse to Concerta or Concerta to Vyvanse. This is mainly due to tolerability or efficacy issues with one vs the other. Vyvanse is an amphetamine product whereas Concerta is a methylphenidate product. Sometimes, patients will tolerate one derivative better than the other. Hope this helps! Dr. Aly

Haha, my doctor started me (20AFAB) on 54 mg. I weighed about 145 lbs then and haven't gained or lost much in the 9 years since.

@@Jjooodiey last week I bought the brand name Concerta and it’s made a huge difference. It’s not even the same medication as generic, completely different. I highly recommend. I think it’s worth the investment. Also you can ask your psychiatrist to give you a cheek swab test through Genomind. They order the kit, swipe in the office and send it in for results. It shows what medications are best for you based on your genes. For instance I’m a weak metabolizer of CYP2D6 which is why Adderall wasn’t the best option for me.

Hi Rajeev- Thanks so much for checking out this video and asking such a great question. From my understanding, Inspiral (methylphenidate hydrochloride) is similar to our version of Ritalin in the U.S. These medications do run the risk of affecting weight and growth. The FDA label of Ritalin states that "consistently medicated children (i.e., treatment for 7 days per week throughout the year) have a temporary slowing in growth rate (on average, a total of about 2 cm less growth in height and 2.7 kg less growth in weight over 3 years), without evidence of growth rebound during this period of development." Monitoring growth is always recommended when taking these medications. If you found this information to be valuable, please don't forget to SUBSCRIBE as more videos are being released weekly. More great questions! I often suggest that kiddos I treat take drug holidays, especially if they are having weight/growth concerns. This won’t eliminate the potential for side effects in all kids, but it does help many! This is especially true when parents hold the medication over long holidays or spring/summer break. Dr. Aly

@@dralywood Does anxiety a part of adhd? My daughter has high separation anxiety with my wife at 11 years That hampers the environment more.But her general anxiety has been reduced.Does stimulants when used help reduce anxiety when it gives confidence in doing things through increased focus?

My doctor just gave me this for ADHD. Is it true that it causes depression? Because I already kind of have depression but my doctor said if it manages the HD symptoms, the anxiety and depression that I have might go away because it'll control me being overstimulated

I can not watch the whole vodeo because the "ding" when you are making a point drives me nuts. I had to turn the volume all the way up to hear your voice.

Yaaa. That was an early days video where we had a lot of growing pains in leaning “how to KZbin” The Concerta vs Vyvanse video is much better all around and as a lot better info too.

hi .. I have ADHD and I want to take CONCERTA only for a year " until I get my shit together". ( I am not asking for professional advice - just a general - is it possible for a Homo sapiens to take Concerta only for a year and then stop relying on it? ) THX

Hey Carol- This was one of our earlier videos when we were learning about KZbin and equipment. The audio on this is wayyyyy low 😫 (growing pains of learning a new skill...) CHADD did an article called 'Could Your Child’s Meds Affect His Height? Likely Not' Which has the relevant studies, in short it's likely to make little, if any difference. If it does reduce growth then some evidence suggests that it's due to appetite suppression and could be remedied by a medication holiday, which would allow the growth spurt to occur and mitigate the lost height. (Some research shows that it may only be ~1cm of height lost, and even then it can be subjective depending on the patient)

I have been on this medication since 12, and it has allowed me to live a productive and functional life. It’s probably also what kept me from putting a gun in my mouth. Anyone who thinks my parents should be ashamed of themselves for that should be ashamed of themselves.

Do you have ADHD yourself? How young should the child be for the prescriber be ashamed? Would a curve showing the level of shame and child's age have a bell curve distribution, or would it be linear? Looking forward to hearing from you. 👍

@@YT_User- Looking forward to hearing from me? Why would that be when clearly you know more about Medication than the doctors themselves. Someone as bright as you waiting for a response from someone as dumb as me? I have Autism and ADHD. I would recommend you go and research about the Long Term side effects of Stimulant Medication given to Children. Adios Einstein 🙂🙏🏼✝️

If you've got no compelling evidence to present then I'll assume you can't back up your claims. P.s. are you saying you're more of an expert than professors, doctors, Psychiatrists and pharmacologists given that they're wrong in prescribing medication to anyone under 18?

@@Icipher4 literally no negative side effects. All upside. I take at 7am and it poops out around 3pm so I take a 10mg ritilin which ware’s off by bed time
