Doc, when it comes to topical sulfur and its antibacterial effect. Can acne bacteria become resistant to sulfur if used long term ?

Likely less of an issue than other topical antimicrobial treatments like clindamycin and erythromycin. C. acnes resistance to products like sodium sulfacetamide have not been widely reported.

Greetings Dr. I am Elisabeth from GCSRT that's where I had your contact. I am trying to treat my (middle) acnea since 10 years. Now I am on isotretinoin gel and topicream( for atopic dematitis)... I would like to know more about this laser treatement ; do you provide it? Does this laser treatment also applicable for seborrhoeic keratosis?

1726nm laser is available at a variety of clinics. It might be helpful for sebaceous hyperplasia, but is unlikely to be helpful for seborrheic keratoses.

Hi Dr Barbieri. This seems like such an exciting innovation. My only concern is if it targets and destroys sebaceous glands if there is a risk of ending up with dry skin that’s vulnerable to wrinkles and signs of aging in the future? Especially after menopause. I would really like to get rid of my acne once and for all.

I think dry skin is a potential issue (similar to isotretinoin), but the nice thing about these devices is that we can control where we target, so by focusing on areas with acne, I think this tends to be less of an issue in practice.

Hey Dr.John, i keep seeing videos about people eating 3 - 4 cloves of garlic daily to fight acne with great results. Whats your opinion on this trend as an acne professional?

There is an absence of evidence to support that garlic is going to have any meaningful effect on acne (pubmed.ncbi.nlm.nih.gov/37878272/). While I cannot say that it has absolutely no role in the treatment of acne, it is unlikely to have a major beneficial effect (especially compared to other strategies). @@DrJohnBarbieri Very interesting, thank you for replying. This might sound silly, but if I do try to eat garlic daily, will it somehow affect the efficacy of a topical adapalene gel? ( in this study, garlic has drug interaction with some medications; pubmed.ncbi.nlm.nih.gov/17918162/ )

I recently completed my 3rd treatment using the Accure laser and I see improvements for sure. I suffer from cystic acne and it is definitely due to hormones as I only really ever break out prior to my period (jawline, cheek area with cystic acne.) I have noticed that my breakouts prior to period are much much smaller and will resolve within a few days as opposed them lingering around for a week +. I was hoping I would only need 3 treatments but I think I’ll need one more which sucks because I pay around $1,400 per treatment. OUCH! But I’m DESPERATE

Under the microscope, we see that it causes selective destruction of the sebaceous gland, so one would expect the changes to be potentially long-lasting and thus it might be able to deliver long-term improvements in acne (which aligns with what we have seen so far in the clinical trials)

Dr. Barbieri, Do you have an opinion on the Aerolase Laser (1064 nm)? I am having trouble locating a dermatologist who offers the lasers you discuss in this video, but found one who recommends Aerolase. Any opinion you have would be greatly appreciated.

I do not think it has as strong evidence to support effectiveness (particularly with respect to long-term improvement) for acne.

How does aviclear differ from other acne lasers on the market such as Alma hybrid, cadella smoothbeam, fotana 4D, or the neoclear laser by aerolase?

Aviclear and Accure are 1726nm wavelength and target the sebaceous (oil) gland. Unlike other lasers that just treat the symptoms of acne, these 1726nm lasers target the root cause of acne and can deliver long-term improvement. They are the only energy-based devices that I would recommend to treat active acne (other lasers can be helpful for scarring and dyspigmentation).

Is this the same as ipl harmony? I noticed some purging with harmony as well after 3 sessions 2 weeks apart for 1 month. I have currently started week 1 of my accutane. I wonder if the laser handled my skin purging or and if i would encounter a purging phase with accutane since they are similar mechanisms

No, it is a different mechanism than intense pulsed light, since it is a different wavelength (1726nm) that targets the sebaceous gland..

1726nm laser has not been studied as a treatment for enlarged pores and mechanistically I wouldn't expect it to be particularly helpful.
