One of the best attacks / counter attacks he consistently would hit on me is what this video is about. Is the zema creating pressure on the ankle or the knee?

Depends on the angle. Generally the ankle and then the knee

Both. First the foot, then moves up the chain.

Is there any mental technique that you can apply when you're hurt or in pain in a fight? For example saying stuff like "don't give up!" or "no pain!" to yourself is helpful? Is there any way of ignoring pain? Maybe focusing on winning the fight and not on the pain itself?

The best advice I could think to offer is not to feel sorry for yourself, and realize you have a job that has to be done. If you give up, things get way worse. (that is for an actual fight though, not necessarily a competition)

Depends on the rules and ref, but most the time this will be considered a foot lock and not a heel hook.
