Mayday: Air Disaster - From Season 5 Episode 8 "Mixed Signals": Birgenair Flight 301 (Boeing 757-225)
Moments after takeoff, the pilots of Birgenair Flight 301 are bombarded by confusing alarms - one says the plane is flying too fast. The other warns the plane is traveling too slowly, and is about to stall. Completely bewildered, the pilots of the Boeing 757 struggle to regain control. But within five minutes of taking off, the airplane slams into the ocean, killing everyone on board. Investigators comb the wreckage for clues and are eventually faced with a mystery - how could the failure of a single instrument cause the crash of one of the world’s most sophisticated jetliners?

Pito Tubes is maintenance failure by not covering them allowing debris and other particles to build up. In addition to allowing the aircraft to sit at the hangar for over 20 days equates to cutting corners. That should have been a lawsuit against the company

This is why there needs to be redundancies on top of redundancies with aircraft. Sure this type of event might be a one in a million chance. But still.

It may be more complicated than that. For example, there may be instances where pito tubes get different data because how the plane is flying, for example if it's turning into the wind or even descending. Having a redundant pito tube on each side might help, but it may also complicate things too, as it could give mixed readings. At least they added the warning about the control disagreeemnt. But in the end, this really all started wit ha pilot who ignored initial warnings (like the speed discrepancy during take-off). And that comes down to training. Second was the maintenance that was not done properly (covering the tubes).

@@HR-wd6cw oh, don’t give me wrong I understand completely that these kinds of things are possible and you know the whole saying of Murphy’s Law. But at the same time like the alteration that they made after the event where they can choose which tubes they get data from. You’d really think that a system like that would’ve already been in place. I don’t know about you, but I’d rather have the development of an aircraft cost a little bit of extra money rather than lives. I’ve known of other incidences where other foreign material has blocked those tubes. In fact, one time the EMS helicopter that I used to work on had to be grounded because the same instrument used on the helicopters is the one on the planes although it’s called a different name. I forget what it is was plugged up with ice.

A later airplane crash was caused by spiders in pito tubes if I recall correctly

When the air speeds didn't match, Captain should have aborted the takeoff.
