The point of the dude in the car was that because it was an adult man, Denji didn't care. He did care about saving the woman and the child earlier but not the dude.

I think that people are missing the fact that denji's indeed a brat/kid, at least at some level. he didn't had a normal childhood (and didn't had any friends/lover/family besides a devil) I guess that's why he's kinda peevish and keeps talking about boobs. ofc he's just 16 and as he grows up he'll start figuring things out, but for now, I think that's just a mechanism for him to "cope" with the absence of love and empathy
