What impresses me about what a clinical dietician does is that you prioritize and balance the needs of the patient over time. I assumed those in the medical field offered a solution to treat a specific problem and that they patient had to put up with the negative side effects.

Thanks! At least for nutrition, we have to look at things on a long-term basis because it's one of those things that won't show instant results but really matters over time.

It is interesting to see how things are done differently at each hospital. I would have personally asked MD to order ensure high protein supplements, juven and change his diet order to double protein options to help him increase his protein-energy needs (in addition to the potential diet liberalization). As you said, he has plans to be discharged, but on the off chance he is not, this will make sure he is covered. Very insightful to see how other clinical RDs schedule out their days! thank you! (the current hospital I am interning at seems to be very strict on how they want RDs to approach each pt/ the PES we are allowed to use/ wordings we are allowed to use.

Hello Jenny! Does your hospital order juven for patients with pressure ulcers often? I’ve definitely heard of how helpful it is in wound recovery but just wondering how often other hospitals decide to order it for these patients. Thanks!

I live for case studies now. Currently I'm on my summerbreak before my last year of my Bachelor starts ( I study Communications), BUT I decided to pay €500 to do this homeschool nutritionist thing, so I LOVE every little food-/ healthfact thats out there. thank you thank you thank you

Mmm that lunch looked pretty good. Pretty jealous you got a pretty good cafeteria option there with the patty. I'd also for sure add olives, olive lovers represent.

It's honestly just this veggie patty and the salad bar. but the salad bar has good choices. also there's a lot of veggie side dishes at the hot foods bar :)

@@amandavsevilla Well it looks like a legit salad bar and having a pretty good looking protein to throw on top is really nice. Salads are top tier lunch IMO especially so in the hot summer so I would probably eat there everyday lol.

applesandamandas it’s been over three weeks now. Whenever I get stressed I usually turn to cooked foods. I’m just being kind to myself and allowing myself whatever I am craving for the time being. When this wave of stress has passed in a couple more months it’ll be interesting to see if I go back to fully raw! Right now food is doing its job. Haha

Loving this look into your field! I'm curious...question: do you try to see the patients based on when they are expected to leave? Ex: Does it say when they are expected to be discharged in their chart? That way you would see this person first, and try to make the recommendation before they leave (so that all your time reviewing their chart isn't wasted). Or do you just look at patients in whatever order they arrive on your desk/based on urgency of their medical issue? Thanks girl!

We see our patients based on the kind of trigger. For example if they are a new tube feeding patient, we have to see them within 24 hours. Some patients have nothing wrong with them nutritionally (at the moment) so we have to make sure we see them by 7 days to make sure there isn't anything that we missed. We see discharge orders if they're put on the chart, but sometimes it takes days for a patient to leave after receiving discharge orders so we still have to see them. I hope this answered your Q!

I have an absorption issue. Few years ago my gallbladder was taken out . I never was told how to eat . I take an ox bile help digest fat . Do u recommend doing smoothies to help with digestion. Just your opinion. 💛

So I recently found you & im sooo obsessed .. one bc you’re vegan TWO bc you’re a dietician & im a type 1 diabetic .. I was diagnosed at 15 .. I was 210 pds than I’m 110 now but my sugars are still uncontrolled I just went back in the pump my A1C is 11.4 .. I know I know it’s HORRIBLE .. I was just discharge from the hospital for DKA my sugar was at 1500 .. I could go on but I won’t .. kinda wish there were more vegan dietitians .. kinda wish you were my dietician 🥺

Wow! I've had a couple patients who have had very high A1C (14 one time) as well. Have you checked out Robby Barbaro and Cyrus Khambatta from MasteringDiabetes.com? Robby also lives with Type 1 and has been managing it with a vegan diet! They have programs for all kinds of diabetics and have seen nothing but successful lifestyle results!

So are you legally bound to recommend the standard cardiac or diabetic diet that the hospital uses or can you make further customizations to these plans - for instance, for a patient with diabetes, lowering saturated fat significantly (and fat in general) and increasing whole grains and reducing heme iron, etc. Just wondering what you have the freedom to do!

Hey Jaclyn! I believe we are legally bound to order diets that are compliant with hospital policy and the patients actually choose what they want to eat when the diet clerk comes around and they are helped to choose foods that fit within their "diet" such as "low fat" or "low sodium" etc. For diabetic patients I definitely encourage that but what they worry about most in the hospital is glucose control so the whole "carb counting" thing is necessary. too high glucose levels can interfere with more important medications for that acute event. I have the freedom to counsel a patient on whatever I like but most patients aren't even in the mental state to think about "lifestyle" applications. I hope this makes sense!

Hi Amanda! I have a pretty random question, but do you know Tabitha Farrar? She’s an ED recovery coach who has a lot to say about the body on malnutrition and the connection to the brain during a supposed famine. As you are an RD, I’m very curious about your opinion on what she says and supports!

im worried because i want to study to become a dietitian but being one seems so useless. ive heard of dietitians not being respected by doctors and its very hard to get people to change their diets. i know myself when i had counseling i only went a couple times and then never saw them again. any thoughts about this? does the job seem useless just trying to prescribe diets when theres nurses and doctors doing their own thing that im sure diet is the last thing on their list?

dietitians actually make a huge difference, and the doctors (if they're good ones) know darn well that they don't know what a dietitian knows in terms of diet so they need us as much as we need them! i've never had a problem with nurses and doctrs when i talk to them about the patient's diet. they almost always trust and believe the dietitian's advice in this field (after all, our degree is on this topic!)

What is the best diet and herbs / supplements for primary biliary cirrhosis? Conventional doctors just prescribe medication to help with symptoms such as itchy skin but do not help with treating the condition.

We have a list of doctors who have OK'd for us to do ~some~ order writing (i.e. some doctors are okay with us just doing supplements and tube feeding rates, but nothing else). I hope we all get order writing privileges natiionwide!

Hi Amanda! I've been a huge fan for years, and I have a bunch of questions which I would love to hear you talk about. (prepare for massive word hurl lol) First I was wondering what it's like for you to help patients with eating disorders (if you ever do that?) 1. if they want to stay vegan through recovery and 2. if they want to go vegan at the start of recovery? A lot of dieticians seem really wary of recovery and staying vegan (in my experience anyway), which I can kind of see if they're not experienced with veganism themselves cause they probably view it as unavoidably restrictive... I feel like a lot of vegans who are big on KZbin/ Social Media seem to suffer from disordered eating/ distorted eating views, or are very weight-loss and rules focussed (which is scary, because they're hugely influential particularly to young people). So, what's your advice for people to be/go vegan without forming dangerous food restrictions/ get super hung up on the often borderline orthorexic diets & tips and become obsessed? (like, people who are vulnerable to forming disordered eating, or are trying to recover, or just don't know how to spot when something is unhealthy). And also I was wondering if you still have "fear foods" yourself/ how you got over that? Sorry I know this is a lot of difficult questions, but I feel like you'll have an actual important, educated and honest opinions so if you'd be willing to share them and have the time then I'd love to hear them! (does it ever scare you the sheer amount of false (esp. vegan) nutritional info that's out there on KZbin because oh my gosh some of it is bonkers! The other day I saw someone promote a potato basically mono-diet as nutritionally complete, and she sold it well.) Stay groovy, love your channel xx

Hi Zi! I actually don't encounter any eating disorder patients. usually they go to a different hospital or I just havent seen any yet. My advice for people who want to be vegan is to make sure you're doing it for the right reasons and that your relationship with food and eating in general is solid before making the switch. When I was going through recovery, (vegetarian at the time)I made sure that eating meat didn't scare me so Iactually ate chicken. surprisingly, it didn't matter to me and I just wanted to be vegetarian for ethical reasons. Everyone's process is different (that was almost 6 years ago for me) but i think veganism comes second to having a sound, stable mind. If you would like more help/info, shoot me an email! xx
