This video was incredibly helpful in giving me perspective as I pursue a Nurse Practitioner in Psychiatry (NP Psych) or a Family Nurse Practitioner (FNP). I have a strong passion for psych and know of the significant shortage in that field. However, I am also interested in the medical aspects, particularly cardiology. Great video!!!!!! Helpful and informative and real. Love that you work with case managers. I'm one and with my dual diagnosis , coordination is key yet a lot of primary care providers won't engage.

If I review a chart more than about 15 minutes before a visit I won't remember anything, lol. I don't remember anything after they leave either so I do everything in the 20 or so minutes they're sitting in front of me. I've been at or fove years now and have great quality metrics and productivity. I do not become overly involved in therapy. There are other staff with time more adequately detailed to that. I IM the desk staff to rescue me if I get a heavy talker. Great job.

i’m starting my first GN position on an inpatient psych floor as soon as i pass my boards this summer, and i am going straight into my alma mater’s psych NP MSN program this fall! thank you so much for the video. i hope to work somewhere when i’m an NP where i get at least 30 minutes with patients! i’m very excited to have the independence to develop my own unique approach to the practice. i hope to provide a lot of education and referrals! do you ever get worried about making a mistake and being accused of malpractice? that’s my biggest fear... like i know i’m going to try my absolute best to provide the safest and most effective care to my patients, but what if i make a terrible mistake somehow?

yeah, its definitely nice having 30 mins for follow ups. of course being accused of malpractice is always on my mind, but as long as im providing the highest quality of care as i can, patient will understand.

I’m currently getting my nurse-midwifery masters and plan on getting a post-masters in psych-NP my dream would be to marry my passion for mental health and women’s health. Especially focusing on post-partum depression and psychosis, along with pregnancy related grief and loss. Have you seen such a combo in your experience as a psych NP?

@@jaynewu3714 no, just need an RN. But to be a psychiatric nurse practitioner you of course need one. It’s a specialized role, basically acting as a low level psychiatrist.

I have a question about interventions - is it ok to include holistic plans of treatment to the patient that may seem like "alternative" medicine, like yoga, brainwave entertainment, herbs, etc.? If there are peer-reviewed scientific studies done confirming efficacy of some of these alt treatments for diseases. I'm asking from a legal standpoint. I would hate feeling like my hands are tied when I want to tell a patient something for their benefit, but cannot. Also, do you get lonely in the profession?

of course, im glad i can help. i would say grab some med surg experience then move on into psych. if you love psych then gain 1-2 years of eperience to make sure you love this population, then apply. Make sure you get good BSN grades, get psych experience, get good letters of recs and gain shadow hours with a psych NP. i had 2 years of psych experience prior to NP school

Yes, i do provide supportive therapy, but depending on where i work, medication management is the main component. it isnt like that for everyone tho.

Thanks for another great video! Few questions for you if I may? Are you able to share how many patients in total you are managing? In addition, how many are you seeing on average per day? The reason I ask is because I have heard of psych NPs and MDs being overloaded with patients (up to 25-30/day). So essentially they are only seeing each patient for a med-review for only like 15-20 minutes which I think is horrible. Are you aware when entering new contracts as a psych NP, can you outline in your contract the size of your caseload you want along with how much time you want for each med-review and eval? I am curious to know, thanks!

Everyone has a different style, I do my charting when they leave the office. I've noticed that when I talk and chart while they are in the room, they stay a little longer but when I give them my full undivided attention, they don't stay as long so I have more time to chart when they leave. I'll have enough time to finish before my next patient

If you don’t want to prescribe meds, then psychiatrist or psych nurse practitioner is out the window. You have many options to diagnose and provide talk therapy. You can be a social worker, marriage family therapist or psychologist

The Psych Dude I wouldn’t mind prescribing medication but I am not so much interested with going to school more than 6 years.. so I’m really trying to see what I could do to work with kids who have mental health issues, that get payed a decent yearly salary! Mental health and psychology interests me so much!!

This is an extremely complicated question to answer but, honestly it depends on which setting you’re in and what state you’re practicing in. For the most part we both evaluate, diagnose, and treat patients in a similar manner. Some restrictions might be along the lines of breaking involuntary holds, but very few that I can think of right now lol.

I start with "Do you see or hear things that other people don't" and probe more depending on the response and totality of symptom history.

for providers in psychiatry, there are so many options but the main ones are psychiatrists (MD/DO), and psychiatric NPs and psychiatric RNs. I think you need to attend a post masters psych NP program in the united states.

I’m a Psych NP in CMH, as well. I do med mgmt but also try to incorporate Motivational Interviewing techniques, which can be brief therapy. Yes psych NPs can prescribe medications. Yes, depending on which state youre from, you dont need permission. There are 20+ states where NPs can practice independently

Right now I work a little more that 45 hours a week on average. I probably work anywhere from 40-50 hours a week, depending on my patient load at my private practice

Hahha it's a lot for now because I'm slowly building up my private practice. Once I'm done building it up, I'm planning on working 30-40 hours a week

Hi there , just hired as psych nurse,i am looking forward to become a Psych NP, i am a correctional nurse before handling some psych patient with behavioral disorder , and few psychotic patient ,. thank you so much. Honestly, i got to experience both sides as an RN and they are both great. I like outpatient right now because its less hectic, but eventually i want to do rounds on inpatient basis.

So technically based on our training and certification, we are allowed to see patients across the lifespan, meaning treat patients of all ages. We don’t have to get any other certifications, but that doesn’t mean we can seek more training. I would say finding a child psych willing to take you under their wing and teaching you and doing more continuing education on treating pediatric patients

from what ive heard, they dont have any psychiatric programs for RN, its more of a board certification by getting hours and taking a board certification exam for RNs

I have a close family member who suffers from mental illness they of course don't think anything is wrong.. what can we do to get them help they are 30 yrs old and I'm sure if they can get some form of medication they will be just fine..I'm a nurse and I just dont know how to get them to a dr if they dont think they have an issue please advise

Hello @dinahmite425 you may want to look into an organization called NAMI (google it). They have a lot of information and programs for people experiencing mental illness as well as their family members. The actually have a 12 week family to family class that gives families an ample amount of resources to help their family member as well as to help themselves

I have been accepted into a nursing program and have always wanted to further my studies to PMHNP. After watching your video, the job seems very repetitive. Is the 30 minutes the most you spend with a client?

Bruce Desa well if you think about it nursing and any field is repetitive but every patient is different and the way you treat them is different. Typical time is 1 hour for initials and 30 mins for follow up, sometimes less.

I do prescribe them but honestly, I only do it as last resort and on an as needed basis. I have strict requirements, but if it’s needed and justifiable to why they need it, then I will.

Anywhere from 12-25 depending on where I work. If it’s at my community mental health center, I see max 14. If it’s at my private practice, I see 20+. Usually 8-5

You are a very dedicated and knowledgeable practitioner. I wonder why psychiatrists' and psychiatric mental health nurse practitioners' roles are being narrowed into prescribing medications. It almost seems as if their role has been re-focused as promoters of the pharmaceutical industry. This is not to disparage you or any individual psychiatrist or psychiatric nurse practitioner. Psychiatrist Daniel Amen emphasizes natural treatments such as herbs, vitamins, and minerals, altho he does prescribe medications in a limited way for some patients. His book, The End of Mental Illness describes his BRIGHT MINDS approach for brain health: B-Blood flow increase R-Retirement/Aging factors I-Inflammation G-Genetics H-Head Trauma T-Toxins M-Mind storms (abnormal electrical activity) I-Infections/Immunity N-Neurohormones D-Diabesity S-Sleep I hope that the super well-educated Psych MDs and PsychNPs will not be limited in their healing roles to prescribing pharmaceutical medicines when there are many other natural, less addictive approaches with less side effects.
