+Xavier Stonehewer Thanks Xavier! That’s a really thoughtful question. When it comes down to it, aspiration can’t hurt. If you’re using a needle and injecting close to a large artery such as the infraorbital or angular artery, it probably helps out a lot. At my practice, I use a microcannula. I prefer that because when you’re injecting slowly, while withdrawing the tip and keeping it mobile as much as possible, the risks of intravascular injection are greatly reduced. It makes my life easier. :)

Hi Dr Mabrie, I like what you do with fillers. I had juvederm voluma injected in 2012 to enhance my cheekbones and sub-malar area. I liked the results, the only problem is the juvederm did'nt completely re-asorb it left a small amount in a capsule. I tried to dissolve it with 5 rounds of hyaluronidase but it remains and now i have lost all my sub-malar fat and some malar. My face looks terrible around the cheeks now. I am thinking about solid implants now as im am skeptical about injecting fillers into my face for fear of granulomas. Do you have any recommendations , i am male 30.

Thank yon for submitting your comment. I'm sorry that you didn't have a positive outcome from your Juvederm treatment. Without seeing you in office for an in-person examination, I'm not able to provide a treatment plan. Granulomas can occur with dermal filler injections. Typically, treatments of hyalaurondaise can help resolve this issue. Juvederm usually dissipates within 1 year, if you are still have complications, I suggest consulting with a facial plastic surgeon in your area.
