Sculptra is amazing if you suffer from overall volume loss. I'm 33 (haven't found a lot of comments from people in their 30s, so I hope it will be helpful), but I have always been very thin + I practise sports. I was initially considering fat transfer, but I don't have enough of it. Like many people mention, I was scared about the risk of nodules, but my doctor prepared it 7 days before the procedure to let it "sit". I have had absolutely no side effects, no pain, bruising, swelling, nothing. I felt 100% normal immediatelty after the injection. Different people react in a different way, but many of the comments online are so exaggerated... My only regret is that it can't be injected under the eyes.

So informative, better then most doctors in an actual visit. All your patients look like a healthier version of themselves and not as if they had something done. Very tempting to go to New York from Chicago to get this done.

If using sluptra how many treatments before the skin repairs itself I heard that the sluptra has an ingredient that helps collagen grow or is just a filler type for the face?

Hi Angelic Princess, Sculptra does not actually repair skin, and the collagen it can possibly stimulate isn't much. Sculptra creates volume due to inflammation from the foreign body reaction to poly-L-lactic acid, so volume is unpredictable. Hyaluronic acid fillers on the other hand, do create volume more predictably, and do not cause a foreign body reaction since hyaluronic acid is naturally present in the skin and throughout the body. Dr. Prasad, maybe you help... I had Sculptra injected into my temples that left me with big nodules that look like horns. My doctor flushed them with saline 3 times with no improvement. Recently she treated them again with a cocktail of hyaluronidase and steroid with no improvement. Can you please recommend anything else to dissolve them? I'm desperate and crying daily from this deformity 😭😭😭

Hi Barbara Ott, Sculptra is for creating volume as a bodily reaction to poly-L-lactic acid; while PRP or PRFM is for improving skin quality and stimulating collagen, not for adding significant volume. The two can't really compared as they are used for different issues. I hope this clarifies things for you.

Hi doctor, would you say that Sculptra has the same predictability as a fat transfer? I had a fat transfer 5 years ago when I was 35, and I loved the results, eventhough it was not very predictable and a few areas like my temples and nasolabials ended up not ‘taking’ the fat, my doctor did ‘fine-tune’ and touch up those areas with just abit of HA filler after a few months of waiting for the fat transfer results to settle…but the fat transfer got the bulk of the job done, and overall made my face look fresher more youthful (I literally looked 10 years younger after getting the procedure and my skin looked amazing for a good 2-2.5 years)…I don’t really need ‘structure’ as I already have good facial bone structure, it’s just the overall volume loss that’s making me look tired, so the doctor injected a layer of fat evenly and everywhere on my face (even the forehead) without over-filling or augmenting any specific area. If Sculptra will give me similar results to a fat transfer, that’s very good news for me! because I really don’t want to undergo surgery again, and I really don’t like the look of excessive HA fillers in the face (I’ve seen it on many women), it doesn’t look as natural as filling up the face with fat…it’s much better for fine-tuning imo.

Hi @fatiq8297, Facial fat transfer has predictability issues because 30-70% of grafted fat is quickly absorbed by the body, so usually at least two procedures are needed. Sculptra or poly-L-lactic acid relies on a biochemical reaction within the body to induce collagen, and volume so how the body reacts is also unpredictable. Hyaluronic acid filler is the most predictable as it can be customized by the doctor, but this is very reliant on technique. Placing hyaluronic acid filler at the bone level avoids the look you are describing, as it''s not excessive filler, but placing filler at a stronger space than the fat and soft tissue level most doctors put it in. With the bone level technique we call Structural Volumizing, the muscle layer prevents the filler from migrating, and results look more natural, and defined, without surgery, and the procedure takes minutes. I feel much better knowing what the expected results are from your very clear and concise description of the products. I wish I would have seen this years ago, could have prevented issues, if I would have better informed myself. Very informative video. Dr.Prasad, what is the best way to treat(surgery, fillers, co2 laser, retin-a) pre auricular lines? Mine are mild...I'd say 4/10 on a sliding scale. I have been dermastamping twice a week for about 6 months at home. It seems to have helped a little.

It was really helpful.. i am a 26 year old woman and i am looking sadder and thinner in my face compared to the 2-3 year old me, where my skin was plumper and full of colour. Lifted... i was thinking of doing sculptra anywhere n everywhere i think that ive lost volume.. so definitely cheeks, side of cheeks, temples, even forehead over eyebrows, cheekbone under the eye and nasolabial.. now i am wondering whether or not i should do it. One of the reasons i wanted to do Sculptra was how naturally it produces collagen and lasts longer. But the graph in the video showed voluma lasts longer, however sculptra usually needs 3 sessions if im not wrong. Please answer my question if possible, will sculptra make skin look healthier, plumper wherever injected and give some volume back? I have lost some fat/ volume from my face and skin looks thinner and sagging. If sculptra reverses that, and slightly lifts everything up, Id love to go for Sculptra if Injectable in those areas. Appreciate 🙏🏻

Hi allegri9cl, 26 is still very young for facial filler treatment. I wouldn't recommend treatment at this age unless there is a genetic condition that caused volume loss. I do prefer hyaluronic acid fillers like Voluma because adding volume and shape is more controllable than Sculptra, which relies on an anatomic reaction to create volume, but in a less predictable way. Hi LAMPROS PAMPRIS, I would have to do a physical exam to determine what is best for you as HIV may result in more global facial volume loss. Sculptra was developed specifically for global facial volume loss experienced by HIV-positive people. I would need to see your situation before giving a specific recommendation. Hi @zibtihaj3213, While Sculptra does stimulate collagen, and results can possibly last longer than filler, the collagen and volume stimulated by Sculptra are due to a biological reaction. Hi VidalsKinship, The body's reaction to poly-L-lactic acid is to stimulate collagen, not scar tissue or inflammation. However, since it's a reaction by the body, the volume effect, and shape can be more unpredictable than a moldable hyaluronic acid filler that can be custom shaped. In your experience with your patients on average how long does the Voluma last before the results start to noticeably fade when you do structural filling to plump up the face

Hi hammypie, With deep placement of Volume through Structural Volumizing, we can expect 1-2 years of effectiveness. However, volume from a filler like Voluma diminishes gradually, and not suddenly, so results can last even longer, depending on the metabolism of the patient. I hope this clarifies things for you.

Does voluma shift after placement in the cheek or under eye area. I’m concerned that because I sleep on my side that it will negatively impact my potential results (I’ve been unsuccessful in sleeping on my back!)...my practitioner is recommending 1 per cheek & 1/2 per under eye areas to correct volume loss, sagging & tear trough troubles...🤔🤷🏼‍♀️

Hi Kim Böes Keim, I place Voluma in the cheeks deeper than most doctors, between the bone and the muscle in a procedure called Structural Volumizing/ Y Lift, where it won't shift. If it's placed at the soft tissue level, there is more risk of shifting, but not necessarily caused by your sleeping position. I wouldn't recommend Voluma under the eyes as it's a thicker filler recommended for the cheeks, chin, and jawline. I would apply a softer filler under the eyes like Restylane or Juvederm. Hi Slim Kat, Collagen is generated by the body, and treatments like Sculptra can induce a collagen response. The material in Sculptra is poly-L-lactic acid doesn't actually contain collagen, but it stimulates the body create volume, and collagen. I hope this clarifies things for you.

I love your videos - very educational. I am 67 , look good for my age and am very fit - but recently lost weight ( 12 lbs ) due to stopping all sugar. I lost volume in my face and have vertical wrinkles on the sides of my mouth area. also my eye lids sag. I have been investigating treatments as I would prefer not to do a facelift. what do you think of : cool laser and microneedling RF for skin improvement due to sun damage and tightening - botox on the sides of eyes for a slight lift and around crows feet - hylauronic acid filler ( voluma ) in cheek areas to raise the volume there ? thanks.

At your age, it's important to know that devices for skin tightening cannot replace surgical lifting, and can make things worse. Would sculptra be a good product for someone in there mid 20s with volume loss due to acne scarring and recent weight loss. I can stretch the skin on my cheeks and neck like 2 inches away from my face.

Hi Leroy Jenkins, An examination is needed before recommending specific treatments, especially for someone as young as you who would not normally need filler treatment. Sculptra is tricky because volume is created by biological reaction to poly-L-lactic acid, so results are unpredictable. Filler used for tissue loss due to acne scars for people your age is usually very precise. I hope this clarifies things for you.

Hi Elizabeth Schaible, Poly-L-lactic acid in Sculptra triggers the body to create its own volume, so it's not reversible. I don't advise Sculptra to be used in the tear trough because the volume effects are unpredictable. A softer hyaluronic acid filler for the tear trough is what I would advise for my patients. Hi@@eliadvo, Even surgery won't do it because poly-L-lactic acid causes diffuse inflammation all over the face to create volume. All you can do is wait for the material to metabolize, and effects to dissipate. I hope this clarifies things further for you.

I prefer hyaluronic acid fillers as volume is predictable, can be customized for the patient, and with no foreign body reaction as hyaluronic acid is already naturally in the body. Thank you very much for this informative video! I hope you could help with a question - as a male in my early 30s, I am a runner and have very low body fat, which means my face is very gaunt and I have sunken cheeks which I am not happy about. I look similar to Hugo Weaving from the old Matrix movies if you want a reference - with a "protruding" looking mouth area. Would you recommend Juvederm or Sculptra in this case, to get full cheeks when I won't be able to achieve any volume from natural fat gain?

Hi Dr. Prasad - Do you find patients of different ethnic backgrounds aging differently, and thus, needing different recommendations generally? I may be wrong, but I feel South Asian women, like myself, sag around the jawline more noticeably first, than wrinkle as we age. Are fillers the way to go?

Hi Supna Zaidi, There are some ethnic differences in aging, such as darker skin less prone to photoaging/sun exposure does remain thicker than lighter skin. Hi Dr. Prasad. Very informative Videos. I was thinking of getting Ellanse, as it is also supoposed to boosr the collagen and eventually lead to an overall natural look. What is your opinion on it? I heard that apparently nodules can appear as as side effect.

Hi M F, Ellanse is not FDA-approved in the US, where I practice. I do prefer separate collagen stimulation for improving skin quality, and hyaluronic acid fillers for enhancing volume as this treatment combination is more predictable than relying on a reaction to the filler material to create volume. Hi Jon Malkowvich, There are many treatments for deep acne scars, so I would have to examine your situation before making any recommendation. However, it's important to know that acne scar treatment does take several sessions, so it's important to stick to one doctor and follow their treatment plan, instead of starting from scratch each time with a different doctor. The last diagram is very helpful Many of the medspas warned of Sculptra due to the side effect of generating noodles or granulomas that can either go by massaging or can Never go away. What's your intake about it? and from the diagram Voluma is the longest and (thicker? mean more volume, right?) What if you have high motabolisam is sculptra better? Thanks so much

Hi Wan Das, I do prefer hyaluronic acid fillers like Voluma as they are more predictable in terms of results. Sculptra is a different kind of filler as it relies on biological reactions with your body to create volume, and is more for global facial volume loss due to illness rather than a cosmetic treatment for facial volume loss due to aging. Voluma is the thickest and longest lasting hyaluronic acid filler. It doesn't mean more volume, but different fillers are used on different areas. For example, Voluma is a thicker filler for bone loss in the cheeks or chin, but is not appropriate for the under eye area or lips where you need a softer filler for movement. Hi@@nutech1810, Sculptra or poly-L-lactic acid causes metabolic reactions to create volume, so it's not just a filler. However, since these are caused by reactions within the body, the volume it creates is not as predictable, but the volume can last longer since it's not hyaluronic acid like Voluma which metabolizes in about 2 years. I would have to examine your face and discuss your goals with you before I can make specific recommendations for you. I hope this helps you.

Hi so I've already done Subcision (x2) + TCA Cross (x2) + PRFM (x2) + RF Infini (x3) + Focus Picosure (x3) and now my dermatologist says I'll have to do 1 session of Sculptra. I have severe atrophy loss due to acne scars on both my cheeks. Do you think Sculptra is the best filler for me and also is there anything else I should know about Sculptra? Hi Eddie Zarate, As explained in the video, Sculptra relies on the body's reaction to poly-L-lactic acid to create volume, so results are less predictable. This is in comparison to hyaluronic acid fillers which are moldable and don't rely on systemic reactions to add volume, so results are more predictable and precise, but also may not last as long as the material is gradually and safely metabolized by the body. Also would you still consider Sculptra a good option for myself since I already completed the procedures mentioned in my previous comment? Hi@@gabriellanoelle1606, A good diet and healthy lifestyle will always improve your overall appearance as when you are healthy, it shows in your appearance. I will have to defer to your doctor's judgment as they have examined your face, and already performed treatment, so while I can describe the differences of the two fillers, it's not my place to question their recommendations. Dr. I have a round fat face already in addition to the weight gain so it’s even fatter and more rounder. However, I have those bags and mine seem to go more outward than others like a puffy bag. I also deal with dark circles and drop in my jawls and my lips in the corner turn downward. What would be a good solution for me as I’m scared to add anymore volume I might fly away because I’ll be inflated like a balloon face. Please help! I’m in need!

Hi Nikia Williams, Sorry to hear about your problems. Eye bags can be evaluated for possible surgical sculpting and removal of the fat that causes them. Dark circles under the eyes may be treated with platelet-rich plasma and possibly laser treatment. I would have to physically examine your lips and jowls before recommending treatment. I hope this helps you.

Thank you for your info. I’ve been on the fence on whether to use Sculptra( which my Medi-spa is currently pushing)or Voluma. I like the instant results from Voluma & not sure Sculptra would last much longer or of the results( as they show 6 weeks from the procedure).

Hi Elmer De jesus, If you watch my different videos, you can see my hair length is always different, but always thick, and it grows very fast.
