Vitiligo and top surgery scars are easy to develop animations around as they don't change the mesh. Adding larger breasts and buttocks can make standard animations clip through character meshes. Plus, nothing wrong with having top surgery scars or vitiligo. It would be nice for women with larger bodies to be able to represent themselves in the games they play, though.

They're not prioritized, they're just more easily accomplished on the level of variation available to an animation-optimized character model. A character model whose build is more prone to smaller breasts and buttocks, in real life. Again, I'm not advocating that women of the proportions you are suggesting should not be represented in games, (I would prefer an extremely in-depth character creator) but I am providing the logical explanation for why these character customization screens are more limited, particularly on limited animation budgets like with BG3 and Once Human.
