I watched the full episode and what happened to the first officer is heartbreaking. He graduated top of his class and went above and beyond what was expected of him. He was still new but he was truly dedicated and said flying was his dream job. It’s horrible he had to fly with this monster. Rest easy man...

For those who are unfamiliar with this case: it was basically discovered that the captain had berated the first officer so much that the FO was intimidated enough to the point where he didn't call out the altitude even as they were crashing. It's such a shame, because that first officer was one of the most dedicated in the business: not only did he consistently declare that being a pilot was his dream job, but he actually made his own navigational charts of every airport in the region and familiarized himself with all of their ins and outs (well above and beyond what's expected of a pilot).

I’ve seen too many videos about all these errors and planes crashing; surely the training courses are much more advanced (in a sense of having to learn a lot about intense situations) nowadays. Personally I’m not scared of going on airplanes as they’re actually deemed to be the most safest transport, but there is always that worry and I just wanted to know if pilots also think about them

Yes he is and he berated the First officer on this flight just because he's a newbie on this flight and for the company and the co pilot has done nothing wrong and just sometimes have trouble understanding and makes mistakes!!!! And it was his fault for yelling, berating, insulting and deliberately and for also being very critical and provocatively bullying and misbehaving with other colleagues!!!! The list just goes on and on!!!! I love the First Officer better for a few reasons Chad Erickson was an extraordinary young man who was very calm, excited and he was enthusiastic and had skills of making calculations and has a passion for his job since he's a very good pilot. And an excellent pilot for sure!!!! But it's a shame both pilots have perished!!

True, this is exactly the type of people that come to mind when considering pilots getting fired. Regional airlines back then were often underpaying and gave pilots a lot of stress, so its also their fault for not noticing this guy earlier. Thanks to negligence, this man cost his own life, his fellow crew members, the passengers onboard, along with the aircraft.

I've worked at an airlines before. More than 60% of the people that work at airports have more than a few screws lose. These are people who pass the highest levels of security and background checks. Baffles my fucking mind.

What this video doesn’t reveal is that Falitz actually failed a very typical problem scenario: loss of an engine. That should have been a HUGE red flag for Northwest Airlink. That is a scenario very commonly addressed in the flight simulator.

It contributed to the crash in a big way, by making his co-pilot so demoralised or afraid of him by the end of the flight that he stopped asking for an explanation of the unusual approach manoeuvre the captain was attempting, so he didn't know what altitude they were supposed to be at, and he also didn't do the required instrument call-outs during descent. It's likely that the crash would have been averted otherwise.

This episode made it sound like the FO completely failed to warn him about altitude, but he did actually warn Falitz when they were almost at 'minimum altitude' (I don't know the exact technical term) at which point Falitz distracts him by talking about something else (the runway lights procedure, which he hadn't bothered to talk the FO through in advance) despite knowing the fast rate of descent...

It is highly improbable that he crashed the plane on purpose. While he was an intimidating and provocative man, he had a reputation for following company procedures and being meticulous with flight checklists. The concluding statement from the NTSB report (NTSB/AAR-94/05) provided the following probable cause for the crash of Northwest Airlink Flight 5719: "The captain's actions led to a breakdown in crew coordination and the loss of altitude awareness by the flight crew during an unstabilized approach in night instrument meteorological conditions. Contributing to the accident were: the failure of the company management to adequately address the previously identified deficiencies in airmanship and crew resource management of the captain; the failure of the company to identify and correct a widespread, unapproved practice during instrument approach procedures; and the Federal Aviation Administration's inadequate surveillance and oversight of the air carrier."

To all of you wanting to know what happened and why they don't show the reason: They can't. It's their own show you have to buy the episodes. Besides don't waste your money on that, just search the incident and you just saved around alot of valuable time. Or instead search this incident and see if anyone has recreated the crash on a flight simulator.

On a flight from Minneapolis to Hobbing, MN. I had just gotten off NWA flight from Seattle to Minneapolis. There were these two airline pilots from Northwest Airlink. I asked one of the pilots a question. The pilot talking to me was super polite and the other pilot was snarly and he had the attitude that I was a complete moron! I knew then that this "snarly" pilot should never be flying. His attitude sucked big time. I never flew with him again! Then I saw this accident on NBC Nightly News. I was shocked!

Jesus my flight by Ryanair in 2016 was insanely smooth and we only had a slightly rough landing on the return flight and even then it wasn't so bad

Just to be clear, in spite of what the video's title implies, he did not crash the plane deliberately. His first officer was too terrified of him to speak up when he started making mistakes on final approach.

captain was wasting away in a commuter job just getting angrier and angrier, lashing out at his colleagues, and finally paid the ultimate price for his rage. he owes all his victims and their families a huge debt. my guess is this guys dad never hugged him. people like this really should be prevented from flying or being responsible for people's lives in general. this person needed a sabbatical and some counseling. if you ever encounter a person like this in real life, and they are responsible for anything big, do the right thing and report them. express your concerns, make sure they are documented, because it could save someone's life. take these people off duty and get them some help. If whatever pilot who initially discovered he was making extreme maneuvers to make passengers afraid to get back at the airline....had made a bigger deal about it, maybe all these people would still be alive today. That would have been the point where, as a colleague, i would have gone to corporate and said straight up "this guy isn't safe to fly, i wont' fly with him, you need to take him off duty, now. this is serious." of course hindsight is 20 20. more likely is that this guy could probably turn on the charm for his superiors so no one really knew how unstable/weird he was. as many have said, this crash was written on the wall for years prior. an ultimate failure in management and human resources. never ever let an arrogant angry person into a cockpit.

The sad thing about almost all air crashes is that they correct things after the fact, whether it be a fault in the aircraft hardware, a fault in the aircraft software or a human fault.

For anyone womdering, NO, his anger DID NOT caused the crash. The aircraft, at JetStream, was not equipped with GPWS. When descending, they were too low, causing them to crash into trees.

The first officer was terrified of him by the end of the flight, and so was too scared to ask about what kind of non-standard landing they were doing (a "slam dunk"), and didn't do any altitude call-outs. So his anger did cause it even though having a GPWS would probably have prevented it.
