Lauda Air Flight 4
When a plane crashes, for investigators, amongst the most important things to locate are the airplane’s black boxed, often coated in bright orange. There are typically two recorders that are of note. The Cockpit Voice Recorder and the Flight Data recorder, both of which can reveal key information about an accident flight. In the case of Lauda Air Flight 4, the disaster rendered the Flight Data Recorder completely destroyed and any data unsalvageable.
These boxes are built to withstand an immense amount of force and pressure as they are built to withstand the forces of a plane crash. What the loss of the Flight Data Recorder tells us about the Lauda crash was that whatever happened here was very violent.
Lauda Air Flight 4 seemed to disappear into the night on May 26th, 1991, over Thailand. We’re going to breakdown the timeline of events which lead to disaster and uncover the mystery that took the lives of over 200 people that night.

Niki described his airline crash was far worse to cope with than his own freighting crash in Nurburgring 76. Niki was very much involved in the crash investigation as you mentioned and flew the situation in a Boeing simulator which resulted in a non-controllable scenario every time is was tried at high altitude. After 8 months, when Boeing finally was convinced of the problem, they were planning to release a statement only after consulting lawyers so that they are not liable to lawsuits. That would take a few more months. Lauda was not impressed and offered to take a 767 upto 28,000feet and deploy the thrust reversers on one engine to show the world the truth right away. Boeing issued the statement the same day. Tough as a rock Niki, be it F1 or his business. RIP you legend.

He said that when he crashed in 76, he chose to get into the car. He chose to drive it around race tracks at high speeds. He chose to accept the risks. The passengers though, they chose to put their trust in his company to get them where they want to go safely. The breaking of that trust is why the plane crash affected him more than his own crash. What a legend.. RIP Niki

The problem is usually electrical. People have crashed airliners, crashed space probes, all from trusting electronics and computers. For flights, if something works 99,999 times out of 100,000 that is completely unacceptable. There is a reason to stick with analog components.

@@That_Guy78 even physical components are prone to failure just like computers. In fact, I’d argue that quality control on electronics are far easier to analyze and detect early than physical defects. And it’s far easier to build redundancy into a computer than a physical component.

I’ll never forget the image of Niki on site in the jungle wearing his usual hat. He’s seen dead bodies of friends on the race track in brutal conditions but he always said this was an entire new level of darkness

I knew nothing about Niki Lauda before learning about this accident, but his involvement, dedication to finding the answers and care for the families just showed what a caring person he was, which is a breath of fresh air in the aviation industry.

You seen that reality show about the ferry fliers, "Dangerous Flights" or some such? They didn't own the planes, but, still... The owner definitely put a lot of pressure on his pilots, and I don't think that was a complete exaggeration by the producers.

Owners as pilots can also be more willing to push unsafe flights to happen due to financial pressure / fear of losing business. See LaMia Flight 2933, which ran out of fuel because the pilot/owner wanted to get a team to a soccer match and feared losing future business if he was cautious and stopped en route to refuel. Also see many contracted private pilots who crashed, such as Kobe Bryant’s pilot.

To give you an idea of how how seriously off Boeing were, NASA conducted a test with a McDonnell Douglas DC-8 and deployed reverse thrust on one of the four engines. Not only was this matching the altitude of Lauda Air Flight 4, but it was over twice as high as the B767 prototype used to demonstrate the effects of reverse thrust in flight. Boeing had estimated that 10% of lift would be lost. The tests proved that a staggering 25% of lift was lost. As it turns out, the newer internal reverse thrust disrupts airflow over the wings. And yes, for anyone currently wondering. Even a four engined plane with three working engines would be sent into a violent dive. So remember, reverse thrust at high altitude isn't just dangerous, but potentially deadly.

it still boggles my mind how boeing got away with such a design flaw that reverse thrust could even deploy while the plane is airborne. i think now additional sensors see to it that the wheels should be rolling on the tarmac for the reverse thrust to be allowed to deploy.

@@admiralsnackbar69 there was also that 747 crash into the sea due to the artificial horizon getting stuck in a bank angle, confusing the pilot who tries to correct the bank, air india flight 855, and boeing got away with saying the pilot was either intoxicated or on drugs.

Boing was a pain in the ass during the investigation. At first they blamed pilot error and after they had to admit that the thrust reverser deployed uncommanded they kept blaming the pilots for not being able to recover. Boing was a huge dickhead even before the Super MAx disaster and it was only though the tenacity and courage of Mr. Lauda believing in his pilots that the true circumstances came to light and people had closure knowing what really happened to their beloved ones.

It took niki asking them to lend him a 767 so he could prove that engaging the reversers during flight was “survivable” boeing said no because it was too dangerous and niki said thank you very much and left with all the answers he needed. Absolute legend

As a Thai this crash is still brought up occasionally. And the fact that the bodies were looted after the crash by the locals made it so much worse. I’m glad you covered the case. There’s not many air crashes in Thailand so this one is one of the biggest.

@@moonprincess500 Thailand was economically isolated as the only capitalist country in the region which had led to hardships during the 80s. This would lead to vulture capitalists in the west undermining Thailand's currency until they were forced to devalue it, causing their loan obligations to western banks to become unsustainable. They were just coming out of these hard times in 1991 but another devaluation of their currency in 1997 would start the Asian financial crisis that had broad economic effects on the region.

I had no idea that this would be on the channel, that really threw me off. If anyone here knows anything about f1, they know about niki lauda and how much of a good person he was. So it’s no surprise he would act the way he did when it came to his airline company. It’s strange to see him from an aviation channel.

Well yes I agree, but it should not be overlooked that Austrian Airlines (aka the Austrian taxpayers) had to bail Lauda Air out in 2001. Niki Lauda started a new airline shortly after, FlyNiki.

@@topiasr628 No not completely, in 2001, 2002 they had to buy the rest so Lauda Air would not go bancrupt. That was not because AA really wanted to, but there was a lot of political pressure to do so. People later claim that was really bad for AA, but I am no expert on this topic

for some odd reason, my family has a lauda air spoon at home in our silverware drawer. no clue how or when we got it, but it's nice to know the history behind the airline

I don't know much about him, but this Lauda guy seems like a very good person- instead of hiding somewhere in his office and collecting money he was a pilot himself and went to Thailand to help with investigation? First time I hear about something like this tbh

When I was young I flew on a B737. I didn't know about the bucket thrust reversers. When they deployed a bolt of fear went through me. I thought the engine was coming apart. 45 years later the memory is as clear as if it had happened yesterday.

As the Boeing 767 is my favorite plane and Nikki Lauda is a huge inspiration to me, this crash really catches my attention. I'm horrified by the severe circumstances that the crew and passengers probably had to endure. May they rest in peace.

I feel you. The Boeing B767 was the first wide body airliner I flew on. And oddly enough, the exact plane I flew on also had Pratt and Whitney PW4060 engines. It terrifies me to think if this issue happened to our plane. So for that, I owe so much to Niki Lauda. To many people worldwide, the lasting background when they think of him is as an immaculate Formula One racing icon, but he also needs serious adoration for how contributions to aviation.

Even if you’re not interested in F1, you should watch rush. It’s a movie about he and James hunt fighting for a championship when he crashes and is in the hospital with horrible burns. He comes out of the hospital after only 2 weeks and nearly wins the championship. It sounds like it’s a fake story. He really was a unique individual with enormous balls and a huge amount of integrity. There’s really a lot more than even these two stories. he also basically sponsored himself to start racing by way of personal loans that would’ve bankrupt him if he wasn’t successful. Just really someone with incredible fortitude.

His reaction to the crash of the aircraft that carried his name sums up Niki Lauda. He wasn't someone who would keep a low profile while investigations were ongoing, he not only actively went to the crash site, but also pressed Boeing to find out why an aircraft that was supposed to be able to recover from an accidental in flight deployment of a thrust reverser (as certified by the FAA) was unable to so. His visits to Boeing in Seatle, taking the flight simulator to prove his point and cornering them by asking to take one up himself (he held a pilot's license) and replicate what happened (but the company said "you can't, it'll crash,")

That shows you just how determined Niki was. That determination, as well as sense of responsibility for safety is what makes Niki one of my racing hero, if not THE racing hero. Fun fact: Niki is the only person ever to win an F1 race with a fan car. The Brabham BT46B 'fan car'. Banned after one race because Ferrari deemed it to be dangerous. Then again that means BT46B has a 100% win record.

Thank you for covering this one. This accident is still on most Austrians' minds to this day, and everyone felt extra terrible not just for the families of those on board, but for Niki having to go through yet another devastating trauma. I was very little when it happened, but I still remember the picture of him at the crash site, with a face of resignation on him. You knew he was not just another CEO of a company dealing with insurance payments.

The fact that there really was no human error involved makes this crash even more terrifying in a way. It really was 'just a glitch'. An awful, tragic glitch.

even though it would be impossible to prove, the g-forces that the passengers went through most likely would have rendered them unconscious, at least I hope it would have so they didn't have to suffer the fear of their impending deaths. I couldn't begin to imagine what they must have suffered through.

I guess if the plane ended up going faster than the speed of sound it is a fair guess to make that the passengers were unconscious for at least the latter half of the crash, probably a little after the breakup started. I’m going to put my belief in the unconscious scenario.

Unfortunately since this is a dive, and not a violent nose up, the passengers were all more than likely fully conscious the entire way through, barring those who weren't strapped in and got bashed headfirst into a wall when the dive initially started. A Human is perfectly capable of handling zero gravity conditions for long periods of time

[ 8:16 ] "Boeing's own testing of the 767 would be criticized as being inadequate." In its quest to shape up and get ready for the coming merger with McDonnell Douglas ( and Wall Street share manipulations ), the safety and inspection apparatus was already being dismantled and reshaped ( with FAA approval, ) and this would culminate in the 737 MAX debacle.

Boeing tried to blame the pilots for being idiots and Niki got furious - this is why he went out there to investigate for himself. He announced that he had complete trust in his pilots and that Boeing had screwed their tests. Of course Niki was correct. This is how every airline owner should behave.

I remember when this happened, seeing the TV footage of rescue crews in the jungle. Nicki Lauda really took this crash personally too, he felt he was personally responsible and he was obviously a broken man when you saw footage of him at the crash site. Maybe I missed this, but whose fault was this determined to be? Boeing's for its' lack of testing of this scenario, P&W for a design flaw/fault or Lauda maintenance for poor procedure?

Boeing was the main ones to blame due to inadequate testing. The Lauda maintenance team actually followed the guidelines for engine repairs down to the letter.

Those poor passengers. I hate to think what they would have gone through in those last horrible few minutes. As for Nikki Lauda, what a brilliant bloke. We could do with more people like him in the world. The good ones are taken too early at times. RIP.

Knowing how insane the forces that plane was subjected to must have been, I'm amazed that sort of failure could have been considered survivable from the start. I'd rather assume it would destroy the plane and kill everyone, that way even if I was wrong I'd be starting from the attitude of "this MUST NOT be allowed to happen" versus "meh, it'll probably be okay if it happens." Which is probably a better attitude to start with if you actually want to prevent an accident from happening. Gotta admit, Mr. Lauda sounds like a great person, the kind of person we need in charge of airlines. As it is blaming the pilots for 90% and mechanics or baggage handlers for roughly 10% and trying to absolve themselves from any real responsibility (and thus also avoid spending money fixing things) is how airlines work and it terrifies me. It doesn't help that whistleblowers basically have their entire careers ruined and can never work in the aviation industry again. How could I possibly feel safe flying while knowing that that's how they operate?

Niki himself escaped death by a fraction, and saw people lose their lives on the tracks during his career in Ferrari His heart must have been broken, a 767 plummeting to the ground, his own airline, innocent lives lost, respect to the legend

Lauda was a licensed pilot and, as long as the medical checkups allowed him to, occasionally flew random passenger-connections for Lauda Air. They would never announce it in advance, allegedly sometimes even having a "full" crew enter the plane with the passengers and him either being in the cockpit already or coming in later (relatively) unseen. Iirc that ended only when Lauda Air was absorbed by AirBerlin (who're now defunct)

I remember seeing an in depth documentary years ago about this,....the real cause was only brought out because of mr lauda and his persistence. that the crew was not to blame

Considering that it's pretty much SOP for airlines to blame "pilot error" for every crash, it's like a breathe of fresh air that he didn't. Shame he resigned, we need more people like him in charge of airlines. I wouldn't be nearly as scared to fly if I knew the person in charge wasn't one of those detestable rug-sweepers who try to cover things up or downplay risk factors and in the process cause so many extra deaths.

I remember watching that crash Niki was in live on TV when I was young. A horrible accident! It led to newer safety features in F1 cars.

In this day and age, So called black boxes don’t have to be on the plane they can be on the ground in the offices of the airlines..with my cell phone I can send and receive Music, Videos, Voice Recordings and Videos from anywhere in the world in real time..it would also be a very good idea to install cameras in the cockpits to record the actions taken by the crew in an emergency..and if the unthinkable happens you wouldn’t have to search crash sites for the boxes..and you wouldn’t have to worry about not finding the black boxes because they will be on the ground.

Sounds like the original owner should have stayed with the company. I understand that he felt responsible, and was likely grief stricken, but those are exactly the reasons why I feel this way.

Lauda is such an honorable man. This event completely destroyed his already broken heart. I feel so deeply for him, and cannot even begin to imagine all of the horrors he has witnessed first hand.

@@dreamthedream8929 When i was a kid i had not a single problem flying on a plane. But then as i grew up, my sister was watching those Air Disaster episodes every night, based on real plane accidents and it got my attention. Since then i’m really scared of flying. Everytime i’m on a plane and it shakes lightly i’m getting scared and if the turbulence is getting stronger, i close my eyes, hold tight on my seat and wait till it goes away. Sometimes i even cry silently because of a strong turbulence. So lucky to have my family and my boyfriend sitting next to me and hold my hand. I know flying on a plane is the safest way to travel, but in my mind it’s like this : Traveling on a ship and the ship is sinking, there are more chances to survive because of the life boats every ship has these days. Traveling on a plane and unfortunately it crashes, the chances of surviving are really low..😥 And all the agony and terror the passengers feel as a plane falls, i don’t want to experience that if i die. 😞 Thank you for asking how i feel about ✈️ 🙂

@@Tina_Vienna Thanks, I see. So it has been affecting you badly. Its interesting to see how it affects different people. I can totally see how something like this would want to make someone not to go on a plane ever again. However ive heard from others watching the same thing that they actually feel safer and more confident flying as from these accidents they learn more about how aircraft work and how many safety procedures and elements there are. I mean also the person that uploads and makes these videos goes into detail of these terrible things and gets exposed to them but i think that he still gets on the plane and flies it. Is it not affecting him badly? What scares me is the way of dying in some plane crashes, its really not a good way to go. However sadly there are so many horrific ways to die in this world. For example dying to a lion would be very painful and sometimes not quick. And many people throughout ages have gone through something like this and have died to lions and of course so many animals go through this torture everyday in the wild. Burning to death is another terrible way to go and used to be common in the world for witches and others. I also learned that most people in plane accidents/crashes survive, the number was quite high there. Do you dislike the sensation of turbulence or do you think that something is not right and it will crash? To me its the sensation, for the same reason i dont do water slides in water parks, roller coaster rides and similar things. Many like it though. They also like and do skydiving and bungy jumping which i imagine would have similar sensations plus i get dizzy and anxious in high places with no protection (fear of heights does not apply so much on planes to me, i feel more or less okay with that when flying because its inside). Fear of heights was not a big problem to me when i was a kid but for some years now it has been bad regarding that. Living on a high floor wouldnt be good and would not be able to go on the balcony because of many balconies there is no protection. Crossing bridges is difficult too if the fence is not high enough. But overall flying commercial planes is really safe. You know what, there are actually many airlines without a single deadly accident while flying for so long and so much. Its unbelievable but true. I think this is the reason why people say that flying is the safest transportation. Probably even safer than ships that you mentioned but definitely safer than cars, buses, trains, motorcycles and probably also bikes

I'm staying in an Austrian cottage and saw a framed photo of the lady who established the guesthouse and died in this plane crash so I came on youtube to see more. Such a sad event. And she was so young 😞

In simulations it coins be saved but only when the crew were briefed for what was coming and knew when.

I'm not an aviation expert, but how on earth can Boeing just stand there and claim that a sudden and unexpected deployment of one reverse thruster mid-flight is easy to recover from? From one moment to another the engines just started to push the aircraft in opposite directions, which resulted in the aircraft spinning out of control within few seconds. I'm not saying it's completely impossible to recover from, but pretty close to impossible. Just imagine you're driving with a car on a highway with great speed and suddenly only the left side wheels start to spin in the opposite direction. Your car would start to spin out of control almost instantly. That's exactly what happened to this aircraft. Being really is a very questionable manufacturer.

Everytime an aircraft accident happened. It is always between Boeing and Mcdonnell Douglas aicrafts. Even the recent China Airline air crash also involved another Boeing aircraft.

Holds poignant memories for me. Hard to believe that was over 30 years ago. Along with my parents, now deceased, we enjoyed a dinner show at our hotel, the meal was excellent and so was the show, featuring exotic Thai dancers and music. We all enjoyed experiencing the local Thai culture. Many other patrons who we mixed with attending the show were due to travel back to Vienna on that fateful flight on Lauda Air the next day. Can only be thankful that those people had a wonderful experience before their untimely demise.

May I ask, is it possible to make the night simulations a little bit more clear? I'll admit my eye sight is not the best, and I up the brightness on my screen but often can't distinguish between the plane and the night background, except for the lights. If not, no worries, but figured it doesn't hurt to ask

Lauda wasn't the only F1 personality involved with an air accident given a plane crash in 1975 took the life of two time world champion Graham Hill (which financially hit his family, including son Damon, hard).

Do you think everyone was conscious the whole way to the ground ... I always wonder these things with the amount of g force and depressurization yet dropping quick. Even the people ejected wonder If they watched themselves fall to their death. That would be the worst waiting mins knowing soon you will splat and nothing you can do

Apparently some of the warnings on LAPA planes were false alarms like the takeoff configuration warning, And pilots “trained” themselves to ignore the warnings. I’m not defending them I’m just saying

I have no professional knowledge of aviation or plane flight. But what I'm wondering is: Could they have turned off the faulty engine to continue to the next airport? I mean... I guess they didn't have the time and were not prepared, so that's why they didn't do anything like that. But in theory: Would that have worked?

It might have, most planes now are designed to fly on only some of their engines. But considering the violent spin from when it engaged subjected the plane to stresses it wasn't designed for, the plane probably started to break up before that could have been done.
