Wow, wish there were dr like you in my area. Everyone I talk to is only interested in their highest cost option within their limited menu or nothing at all. In fact, no ortho surgeon I have spoken with regarding my issue have ever mention PRP or similar options at all. Just a surgery to fuse my arthritic bones as the ultimate option and to suck it up with Tylenol for as long as I can take the pain.

Would it be better to use prp BEFORE a meniscus treatment since blood flow of the meniscus is limited and when having to remove anything it makes it more difficult to heal itself and takes away that ability? Is it possible to get a prp to help retain the good tissue and encourage healing of anything bad so the surgeons have less to remove or fix?

Wow to hear that Prolo is cheaper then the PRP as I had 3 treatment of PRP which was less in cost for my deep tear in my left elbow I had three treatments I am still healing its now been a two months I think after you mention about the dextrose as it was put into my PRP not much as I felt everything and it was so much painful for an hour after the treatment then it would get very swollen for a few days after each treatment Now, I am getting the Prolo which is two hundred dollars more for each treatment for my C-6 & C-7 I already had two treatments in a few week I have another treatment I am not getting any mumbling he is just giving me the treatment so I do feel every injection I have degenerative cervical I at times could not turn fully to the right and my left shoulder began to have pain it actually works I can now turn to the right my left shoulder has no pain its just I still feel a little crunches in my cervical not as much as before Now about the dextrose I feel my doctor should have done it separately maybe its why my elbow is still healing

Prolotherapy is less expensive than PRP yes, but it's also less effective. So there's a tradeoff, which is sometimes important but sometimes not. Also most doctors will charge more for spine vs peripheral joints.

one dr told me to try the steroid epidural to L5-S1 first since insurance pays then if that works I would be a candidate for PRP in spine. Is that true? Well if it did not give full benefit could PRP stilll work? I do not like the steroids. Also what is best for the si joint?

We personally don't like using steroids. In our clinic we would be able to know based on physical exam and imaging if the epidural with PRP is likely to work, avoiding steroids all together. As for the SI joint, again we avoid steroids at all costs

I’m a patient receiving the prolotherapy for mainly chronic facet syndrome. Recently I’ve switched to PRP for neck & ribs. I find I have more pain-free days between treatments. PRP is painful (not horrible) but beneficial. After a recent PRP treatment (1) I had relief in 2 days this is day 14 still no neck pain. 🙏❤️

I have a herniated disc at L3/L4 and L4/L5 with an annular tear. The pain is constant and can get very intense with muscle swelling along the spine and weakness/numbness in the left leg. I am considering PRP. Would that be better than prolotherapy for my condition?

Prolotherapy is not used for intradiscal injections, which would be the definitive treatment for an annular tear. This would require PRP

Thanks for a good clip 🙏 I have really heavy pressure and pain in my neck and head for over 21 years "this instability controls my whole life now" and can't get help here in Sweden "try all specialist" but they only say, it's mentally, we can't find something else "I'm not alone with my diagnosis with this answer in Sweden" but finally last year i get answer about my neck issues "in Spain" but can afford the surgery, so have search for 1,5 years for someone ho can treat with Prp or prolotherapy or stemcells, but i can't figure out what's is best for my ligaments "C1-C4" what do you say? PRP i say but not sure and have not find someone ho have right x-ray "skills" for a second opinion with my image either. Thanks again and hope you have time to answer, you help lot's of people in Sweden,Norway,Denmark that's have same diagnosis and can't find alternative for expensive surgery "swedish healthcare doesn't have right x-rays or knowledge... for neck instability" #Atlanto-axial Instability. #Craniocervical Instability. Sorry, can't find a way to search comments. I am wondering what prolotherapy treatment runs for - let's say a hip or knee. I find so few doctors who do it that I it is hard to find if the cost is in line with what one should pay or if I should find another office.

@@DrewTimmermansND i am getting a mixture of both prp and prolo given my current doctor says different things work better or in combination for different joints. Prp plus prolo in cervical and mostly different mix of prolo in the shoulder. I am told i will need 5-6 treatments

PRP injections are stronger than prolotherapy, so I would not agree with the sentiment that "prolotherapy is the way to go". In advanced cases were even considering cellular therapy with bone marrow concentrate

Yes prp injections are better than prolotherapy, hands down. PRP and ozone are different. PRP is platelet rich plasma from your blood. Ozone is ozone.

This can happen, but it's extremely rare. In the 8 years I've been doing injections, we've only had 2-3 patients get worse

There are a lot of factors that go into that decision, but the best literature seems to be on PRP but there's a lot of emerging evidence for stem cell therapy

Yea we have had some success with our approaches. Often times we will end up treating both the cervical spine and the thoracic outlet, but each case is extremely unique.

Thank you so much for the infos! I am able to understand the difference of those two treatments! I just received my first PRP today, Dr use numbing medicine which she said was procaine with dextrose in it. She injected it before PRP injections. May I ask would the dextrose interfere the effectiveness of PRP? I had no idea why would she put dextrose mix into procaine. And the treatment was beyond my tolerance, so much pain, had to take two Tylenol and barely could get up. In comparison, the prolo injection didn’t hurt me at all. Is it normal? Thank you so much for your help!

These are all questions that you should really be asking your doctor! As long as the dextrose and prp weren't mixed in the same syringe, then there shouldn't be any interference

I was diagnosed with ACL tear (happened in basketball) via knee doc (clinical test--knee wasn't stable like my left knee). I didn't do an MRI as doc said that's basically if you want surgery so they can get a bigger picture of any other damage). I have an appointment with Prolotherapy doc in 2 weeks but was reading that Prolotherapy only really works if it's a partial tear. Can it help rebuild a full tear? Thanks.

Prolotherapy likely won't heal a full thickness tear, that would likely need something stronger like prp or bone marrow. Also, unless your doctor is using X ray guidance for the ACL injection, you're likely not going to get a proper treatment of the ligament. ACL injections need to be extremely precise. I personally think an MRI is important if you're undergoing injections because it helps to determine what your chances of success are.

Do you use stem cells as well? I have a grade III pcl tear which healed with laxity. Have you seen prp be able to remove some of this laxity and have you ever seen any patient be able to completely remove their ligament laxity through injections?

Yes we utilize bone marrow and adipose tissue which contain autologous stem cells. PRP can help, but for a grade III we recommend our approaches with stem cells

Hello. Thank you for this informative video. Im am getting prolotherapy followed by PRP injection 4 days after in the lower back and SI joints. Is this enough time between the two treatments in order for prolo not to lessen the effect of PRP? Thank you and I would really appreciate your input.

If the prolotherapy is being injected only into the joint then that's usually okay, but if it's going to be injected into ligaments, then it's likely too close together.

@@DrewTimmermansND very similar situation to the above, I'm due for PRP bilaterally in the SI joints, followed by dextrose prolotherapy for the SI ligaments ~2 weeks after the PRP treatment. Is this okay / won't diminish the effects of PRP? Thanks a lot!

That's a stupid approach in my opinion. They should put PRP into the joint and into the ligaments on the same visit, and then see how you are doing 3 months later. PRP is stronger than prolotherapy, even for ligaments. If they are already injecting your joint, they might as well put the stronger stuff in the ligaments as well.

Complete TFCC tear...cannot weight bear at all. 8 months of PT, 2 cortisone injections....still can’t weight bear. I’m 36 and need to return to function! Am I PRP candidate?

possibly! I can really only determine true candidacy after the intake, physical exam, and imaging review. But as the TFCC tear is not responding to conventional approaches, and we have had success with TFCC tears, you're more than likely in the ballpark.

I get confused about inflammation here. Why is dextrose induced inflammation even needed when injured area is usually already chronically inflamed.

Because acute inflammation and chronic inflammation are different, and are mediated by different parts of our immune system. Chronic inflammation isn't just long term acute inflammation. It's completely different on a cellular mechanism. Plus, most of the time there isn't even a lot of chronic inflammation, it's chronic degeneration that needs to be addressed!

Great video! Hi there i have severe muscle knots in my scapula and muscle alongside my spine and causing me considerable pain even after my c56 fusion. is prolotherapy the better choice for breaking up the knots and relieving the nerve pain associated with this.. I feel like slashing the muscle with a knife to get rid of the tension!! Or is prp a better choice. I'm a bit confused. Please help im desperate!!

A patient presents with swelling but no pain, just discomfort upon squatting. A x-ray shows Grade 2 Kocher and Grade 3+ Hefti OCD- but lesion definitely stable. Aside 6 weeks of rest, is there something other than PRP you'd consider?

Not a doctor here but, I think using a 4 inch needle would be best than the short ones. Hauser opposite to many prolo specialists uses it and goes deep into the roots of the joints. I wonder if using prolo and PRP in the same treatment session would be more productive instead of a single procedure.

the length of the needle is always determined based on the depth the needle needs to travel. It's not an arbitrary number. Sometimes we need 5 inch needles to properly get into a lumbar facet joint, in other patients I could use a 2 inch (obese vs thin patient, respectively). PRP is more effective than prolotherapy, so I can't find a good argument to use prolo in the same session as PRP unless the volume of PRP needs to be high for another reason and we have to use prolo instead of PRP. But if I have enough properly dosed PRP for a case, I'll choose PRP over prolo

We perform microfragmented adipose tissue transfers as well as bone marrow, yes. Prolotherapy can be a great option for ligament issues, but it often requires more treatments than PRP

Excellent informative video. I have been seeing a Pain Management doctor who is a Physiatrist as well, never mentions either PRP or Prolotherapy in 11 years. I have Chronic Cervical and Lumbar & Thoracic pains and stiffness from Spondyliosis, fAcet Joint Syndrome, lid to moderate Stenosis (lUmbar and Cervical). And to boot DDD throughout. I've been watching YTs for months now from Dr. Ross Hauser about Prolotherapy for the Cervical Spine. The cervical issues contribute to the following: TMJ, Tinnitus, Dizziness, Occipital / Temporal pain and Headaches, [ain in the shoulders, tingling / numbness in the arms (the left is worse), and IMO giving me IBS / GERD. I can't find a Prolotherapist in the NY Suburban area (Westchester County). Anyone have a recommendation. I;ve been acupuncture 45X , no help, 5 different Chiropractors in the past 7 years with Minimal help (including a Upper Cervical Chiro 14X (not covered by Insurance - No help). I will denote a notation made by the first MRI of the neck done in 2011, the radiologist report said that I had a More Narrow Than Normal Spinal Canal !!!! And now with sites of disc bulges, DDD, a Stenosis ....is it no wonder it is wreaking havoc with my overall well being. Inflammation is rampart. All and any advice is so much appreciated .

Try contacting a pain management doctor. I had all your problems and back in 1996 had several months of prolotherapy done neck and all the way down. it is the best thing I ever did for this issue. My neck was really bad and now it never bothers me. I am 70 now and would do it again if needed. I would recommend you research good nutrition and get a vitamin D3 test along with a metabolic test. I realized that I was habitually low in protein.

Grade 2 inversion ankle sprain 7 months ago, still some ligament laxity combined with a little pain in plantar flexion along the top of the foot mainly when kicking a soccer ball (I am a soccer player), how many prolotherapy sessions would you likely recommend?

No idea how many you would need. Healing is an individualized response. You might be healed with one, or it could take 4. Wouldn't know until we actually started the injections and we saw how you responded.

all forms of regenerative injections (prolotherapy, PRP, and stem cell therapy) can improve ligament laxity with hypermobility and can help stabilize joints. PRP is stronger and more effective than prolotherapy, and stem cell therapy is the strongest of all of them.

You can, but we wait at least 3 months afterwards as there's evidence that the healing may be blunted with the PRP

Between these two, PRP seems to do better for RA, but this research is still limited. There are some interesting studies coming out on cellular therapy for RA. But, as RA is a systemic condition, we can't just think about injections for local areas, we need to make sure that we have a full body approach

Thank you for posting this video! It was Very informative. Will Medicaid cover these treatments? My knee is all messed up from when I was heavy. I lost about 300lbs 10 years or so ago and all the pain kinda just went away. About a month ago I went jogging and my knee has been in pain since & isn't letting up. I went to get checked, got an MRI and was told I had osteoarthritis. Does this last for the rest of your life? This is terrifying considering I usually try to stay physically active and i'm 37 y/o & now even walking causes discomfort and legit pain in my knee like i've never felt. Do any insurance companies cover these types of treatments? If not, is there a way to get them to cover it, as opposed to surgery? Who can administer these injections? Do you trust any doctors in NYC enough to recommend? Thanks for posting this video.🙏🙏

Not sure as I would have to perform a physical exam on you. Usually we use other approaches for scar tissue (platelet rich plasma most commonly)

For the most part, we dont repeat injections into the same spot until the healing from the first injection is complete. Also it's very rare for us to do prolotherapy after PRP. If the patient needs another treatment, we often stay with PRP as it's a stronger and better treatment.

If I have chronic mild lower back pain due to working out , whenever I lean front down on my body, there is some pain. I think that I have facet joint arthritis, do you think it prolotherapy will work on facet joints? 1)normally how many treatments do I need in total? 2) can I workout after each treatment? If you have increased pain bending forward, it most likely isn't your facet joints (facet joints generally feel better leaning forward and worse leaning backwards). We use PRP on a regular basis for facet joints. Prolotherapy can work, but honestly PRP is exponentially better than prolotheraly. No idea how many treatments you would need (I have lots of videos on the reasons for this, but basically I can't see into the future). You can workout after treatments, but we have people take 1-2 weeks off after PRP.

Ultrasound is primarily used to guide the needle into the correct and safe location, not to diagnose the damages in the soft tissues. But we can and do use it for that in some instances (such as cluneal neuralgia)

And could it have caused my L4-L5 ligament injury that caused my lower back pain while bending foward? Does dextrose prolotherapy work and how it works for herniated disc? Is PRP also more effective than prolotherapy for treating lax ligament? My physician told me my pubis is 7.5mm, which is out of normal range and suggest me to do prolotherapy for 8 times. But I also have lax ligament around the pelvis i.e. hip joint and SI. If multiple joints get involved, is prolotherapy a better option?

Yes, PRP is stronger than prolotherapy and patients require less treatments. Autologous cellular therapy from your bone marrow or adipose is stronger than PRP and also greatly reduces the number of treatments.

We perform a blood draw from a vein, and then we spin it in a centrifuge, isolate the layer rich in platelets, and then inject it back into the body.

PRP not suitable for those on blood thinner. In such case , the option is Prolotherapy which is not as good.

We've done PRP injections in patients on blood thinners. Sometimes we stop, sometimes we continue. Risk stratification. PRP does still work when patients are on thinners

I have a hamstring issue that won’t fully go away thats been lingering for about 2 years and now starting to really affect my performance I’m a track and field athlete and I’m able to subdue the pain for short period of time but the pain will always come back. I’ve had MRIs and no muscle damage has been shown. Since there’s no muscle damage it’s leading me to believe it’s a nerve issue that expresses itself in the hamstring area when I run/activate hamstrings. I have no idea what to do and I desperately need to get rid of this issue as I’ll be a college athlete soon. What would be the best course of action?

Stem cells is putting cells from fat or bone marrow cells that havent become anything so the cells develop into actual tissue prolotherapy cause immflamation so the body strengthens it instead of replacing it prolotherapy is cheaper stem cells are much more expensive but alot more worth it

prolotherapy is 1. traditional dextrose injection or 2. PRP or 3. Stem cell. They're all prolotherapy.

Yes they are all proliferative therapies, except when we use PRP for nerve hydrodissection as an example. I personally don't call it PRP prolotherapy, but rather just PRP. Personal preference that doesn't take away from what they do.

High dose, leukocyte rich PRP injected into the disc is what we use for disc herniations, and PRP with platelet lysate for sciatica.
