If I want to start private medical practice with hybrid model where I want to take insurance and also want to provide cash based services, how do I apply for NPI? I have type 1 NPI with part time hospital job where I am credentialed with all the insurance with my NPI type 1 and now have my NPI type 2 which is not enrolled in the insurances.
