This is my first time doing a DOCTOR REACTS video to TikToks. Some of these are good. Some are just terrible. You'll get to see exactly what I think as its so important that we give context to these videos and challenge the ones that are problematic.
A lot of these are quite specific to psychiatry (duh... I'm a psychiatrist) but send me links if there are other ones you want me to look at. Lets bust some myths, call out some rubbish, and lavish praise and love on the ones that are great.

Patients sometimes don't show pain the way you would expect, especially if it's a chronic problem they've had most of their life and has a high tolerance for it. I have a rare genetic disease that causes me to make kidney stones on a regular basis, I don't writhe on the floor in agony but it still hurts!

I'm not a medical professional, but I have several friends who have chronic pain of some kind. Every one of them has a tremendously high pain tolerance.

Thank you for what you said about never punching down and making the patient the punchline of a joke. I have chronic pain and I struggle so much with getting doctors to believe and understand how much pain I'm in on a daily basis. That TikTok guy gave me flashbacks to being on the verge of crying as a kid and another doctor in a long line "testing" if my pain was real. 🌈🌈🌈

I remember reading an article about a really good doctor who dealt with patients with chronic pain. They said he recommended that whenever you get asked that question, whatever number you feel it is, add 3. Because that's more accurate to the amount of pain that most people would be experiencing. You have a higher pain threshold on account of being used to being in constant pain, so you often underestimate what that number should be and doctors that aren't familiar with you will have a harder time understanding how much it truly hurts for you.

Thank you! As someone who's experienced severe ongoing chronic pain due to Endometriosis & Crohn's disease and was scared to go to A&E because of dismissive doctors & experiences I'd heard, thank you for addressing this. The majority of people I know with chronic pain & myself tend to go totally silent when it's severe, not screaming but trying to focus solely on remembering to breathe. The patient could be on their phone to distract from the pain. People cope differently. Also pain is complex, there are so many different types of pain. Those types of tiktoks just reinforce the fear that we'll be seen negatively and treated badly but also makes it seem acceptable and encouraging others to see us this way. Sadly I've known many that have put themselves in real danger trying to avoid hospitals for fear of this (including myself). I hope there are more like you around.

I am definitely a member of the LGBT. That video about autism sensory issues was actually a repost of a video from the national autistic society. I used clips of it myself in some videos I recently made for the NHS to train people about stimming and sensory issues in autism, because they wanted them to be made by an autistic person. I actually quoted your video about the good doctor about autistic people looking for stimulation in the environment, because it was obvious to me but I thought that was a really good way to make it obvious to others. I also never realised that stimming and sensory issues are two sides of the same coin, due to understimulation and overstimulation, I also put that in my videos. Your stuff really inspired me to make my own videos the best I could and definitely gave me some good quotes to steal.

I've heard of people having diagnosis or treatment delayed because they were 'too calm' to be in the level of pain they described. If they say it's a 10 then it's a fucking 10

Seriously! I have a unusually high pain threshold (nothing to get excited about - I don’t work at it) and medical anxiety. This means I will delay going to a doctor until I am nearly dead and then present like I am not in much visible pain. This means that my husband has had to spend a lot of time advocating on my behalf to not have doctors and nurses ignore me. For example, I had to have my pinky toe removed after I shattered it and then refused to go to the doctor for a month until it was really messed up from lack of blood circulation. I continued to work and I work a job that is very physical and on my feet. The only thing I stopped doing was running. It hurt really badly but I am just able to like segment the pain. No idea how or why it happens - it isn’t voluntary or desirable. It is just a thing. So frustrating to see a doctor perpetuating the myth that pain always looks one way.

@@JayeCole I agree that downplaying the pain being told by the patient is harmful. One thing I wanted to point out though, is that the person in that TikTok wasn't a doctor. He's an ER tech iirc. Not that that makes it any less bad. Just thought it was worth mentioning

@@carmen_says_hi I said to a counsellor that I 'had a good life' by which I meant I had a stable home life, good relationship with my family etc. She said this was evidence of positive thinking because so I couldn't have depression.

I have an unusually low pain threshold which has led to an unusually high pain tolerance. So I feel pain sooner and more, but outwardly react less. If I'm asked how I feel on the pain scale, I have to think hard.

Ugh THANK you for pointing out how awful that steveioe sketch is. People react to pain differently and if someone says their pain is a 10, take them seriously! Just because they’re not rolling around screaming, doesn’t mean they’re not in pain. On the same note, don’t underestimate someone just because they say their number is lower. Many people (especially women) take into account what the worst pain might be in theory, and then rationalise their pain down to a 6 or whatever, when really they’re in horrendous pain and need to be taken seriously. Honestly, the whole 1-10 thing needs to be scrapped and a better system found but 🤷‍♀️

When I was a student nurse we were taught "pain is whatever the patient says it is, wherever the patient says it is, whenever the patient says it is", we should never make fun of people experiencing pain in any way. And to make a joke of a person in enough distress to require sedation is absolutely horrific, so thank you for calling it out.

Thank you for pointing out how we can have humour about mental health and general health but that patients should never be the butt of the joke. These internalised stigmas can make seeking help so so so hard seeking help even when you know you need to. At 33yr it has taken me an extremely long amount of time to finally put my hand up, advocate for myself and say 'I need help. A sleep in, long weekend, or a funny movie won't cut it. I have been hiding my suffering, I need professional help'. And for reasons that everyone would assume one goes to therapy... But still, the stigmas deeply rooted in society are hurting all of us. Thank you for contributing to the normalising of looking after oneself. And for doing it with open kindness, grace, and humour. (Please react to anything created by Josh Thomas, and Fleabag)

From what we can see, I can say I love how you practice medicine 💜😭 it's nice to see and hear one doctor to say people shouldn't make fun of patients' conditions whether mental or physical. I liked this video 💜🏳️‍🌈

I work with alzheimers patients and not many people understand the psych side of it. My center cannot give medicine (we are a day program) and like you said, just verbally trying to de-escalate the situation and also removing them from the place that could be causing it (we have a special calming room) helps. I would never judge a client for hallucinating or getting angry knowing they cant control it.

Chronic pain is serious. When every doctor was scared off from prescribing pain meds to those who really needed them, the rates of suicide among people with chronic pain went through the roof. It's a balancing act, but you can't discount the pain of everyone just because there are cases of addiction. I legitimately almost died of gangrenous cholecystitis because once the hospital staff read my chart and saw that I had a history of addiction, they simply didn't believe I was in pain. It wasn't until ER trip #4 that a nurse happened to walk by and see me vomit that they scheduled my surgery. I couldn't walk, turn over on my own, my elderly parents had to physically carry me. The first trip to the ER was in May. I didn't get my surgery until October...fuckers...

Pain is subjective! People with chronic pain have a high threshold adn very well may be 10/10 even without "objective" pain signs. What's that old saying about assumptions?

Spot on. It’s very confusing cause I have fibro and adhd and numbers confuse the shit out of me lol. Also when they ask, they ask you to compare it to the worst pain you’ve ever had, and I’ve had some pretty bad examples to compare too xx

@@CarryOnChronically Yep, chronic pain or ASD (yay me with both). Most of the external display of pain is social signalling rather than a purely biological response, and folks on the spectrum aren't big on social signalling. At least not until we hit meltdown because of pain (because pain can also overstimulate), and then everybody's gonna know about it.

Even though people with views shown in the haloperidol one are very much in the minority it brings all the entire heath care profession into disrepute and also goes some way to explain why some people seem to think that all we do is take the piss out of our patients on TikTok. It makes me so angry because this leads to people who need help being reluctant to seek it for fear of being judged and discriminated against.

Could you do a reaction to the series “skins” lots of themes of mental illness, mainly depression but also anorexia and maybe ASPD. Some scenes to look out for: - when effy suffers psychotic depression - most scenes with Cassie - when they go camping and take mushrooms, involving a trip to hospital. I haven’t watched it in quite a while but there is plenty to unpack. Just been on a binge of your reactions, love it!

As a 2nd year nursing student, the mentality that it's okay to make fun of patients under the justification that it's a way to "debrief" is very disappointing. Nurses are looking after people at very vulnerable stages of their life and the lack of humility, empathy, and dignity shown is honestly disgusting. Thanks for calling it out!

1:12 I really hate that dude. He did one about what happens if you’re suicidal and you go to the emergency room and he basically made it seem as bad as he possibly could and was super flippant about it. I think he ended with a quick “remember suicide is really just a permanent solution to a temporary problem” as if people don’t struggle with chronic mental illness or other hardships that aren’t temporary. He’s an asshole.

@stevejoe is actually awesome, though. This vid looks bad out of context, but it’s part of a major theme. His vids are about working in an emergency room, not just a regular doc’s office. People coming to emergency rooms for non-emergency care and making it much harder for real emergencies to get through are a HUGE problem. He also has a vid about how easy it can be to get too dismissive and misdiagnose a real emergency, so he is cognizant of the fact that sometimes it can go in the other direction. His channel is great. I recommend you watch some of his other videos

As someone who has lived with horrific chronic pain for over 6 years, it pisses me off so much when I get asked to rate my pain. I've lived with it for over 6 years, it's probably like a 6 for me now, whereas for anyone else having it for the first time, it would be OVER 10. I tell doctors that my pain is excruciating but because I'm not laying on the floor, crying and coughing up blood, it must not be that bad 🌈

Thank you so much for this. As someone with mental health struggles, seeing some of the tik toks making fun of people who seek psychiatric care makes it incredibly hard to seek treatment. I don't want to be someone's punchline. I don't want to scroll through social media and find a psychologist or psychiatrist I see making jokes that I could be the inspiration for. It is hard enough to seek care when you're in a bad state with the stigma on mental health needs, this just makes it infinitely harder.

I don't know exactly how medical internship year works in other countries, but in Mexico we pick a hospital and rotate through the services. Six services, to be exact. And of all those, I got put into surgery for the first two months, that are also the longest throughout the year. I'm dying. It's almost like I'm working two full-time jobs. Before this I had a daily exercise routine, time to feed my dogs and play with them, time to go out if I was up for it (though, granted, I almost never am), time to watch series and time to study. Now I gotta work most of the day, can't study, barely sleep, and I'm not even getting paid. Internship in Mexico sucks.

Psychiatric care in medicine, at least in the United States, is regarded as a nusiance and completely disregarded in the treatment of the patient. Drugs such as ketamine, haldol, seroquel are readily prescribed by the doctor to treat patients with underlying mental disorders. I recall a time when a new resident asked for a psychiatric medication and the pharmacist stated that they do not carry such drugs in the hospital system and would have order it directly from the drug company, taking anywhere from 3-6 weeks. Majority of homeless in the United States are former patients of treatment facilities who kicked them out onto the street because the government will only pay up to 60 days. It's horrifying.

Really happy about the message about not punching down. To not get into too much detail, I had severe blood loss years ago, but because it was quite gradual over a couple of months I was walking about just fine - I was just occasionally dizzy and had to sit down. After quite a while, and it just not stopping I had my blood tested (for the 150th time within those months - this wasn't because my doctor didn't believe me or didn't do a good job, it just wasn't an emergency so we tried to figure out WHAT was going on so I could get better naturally) and my doctor had an appointment for me to receive blood at the local hospital within 10 minutes. I was at the point of my organs shutting down due to blood loss. But because I could walk around and could smile / was happy and had gotten used to the fatigue, I didn't present the typical symptoms of someone in that state. All because it was gradual and I simply learned to live with it. Because I walked in on my own and I was fully conscious and happy, I really became the 'miracle' of the ward for an afternoon, nurses looked like they'd never seen anything like it - and poor nurse who had to get the IV in had a rough time with it (tried elbow like 3 times and couldn't get a vein, tried the wrist another 3 times and couldn't get one and finally had to get it in the back of my hand - again, this wasn't incompetence, but me just having very little blood in my system and having small veins) was so apologetic for not being able to get a vein immediately :o Everyone was really sweet and I've never been more grateful for the healthcare system and especially the people in it. The way that someone looks doesn't always show the whole truth of how they are doing. Especially if it's something they've had for a long time - be it pain, or in my case something like blood loss.. If my doctor hadn't believed me and continued to monitor me then I could easily have died. I'm grateful to him for taking me seriously and continuing to call me in for blood work (even if you do eventually tire of needles) as safety precautions and not blow me off because I didn't 'act' sick - I was just pale and tired, 2 things I usually am anyway xD Also 🌈

glad someone is finally criticising steveioe - absolutely not saying he should be 'cancelled' or anything like that, just often his content isn't very empathetic towards patients and creates a negative impression of healthcare

As a person with mental health issues. One insight exercise they liked to do was had me draw a house a tree and a person. If you don't have this in the E.U. I would love to see you do this one and if you do have it could you talk about the volatility of the exercise.

The pain thing... I understand that you need a baseline so you can ask again the number after you administer treatment. However, I think my pain levels would be along the lines of getting burned is a 10 for sure, having the flu is probably a 2 or 3 and, believe it or not, my gout feels worse than when I broke my hand in a mechanical bull riding accident. That's a 7 when it's bad. it's not always that bad, though. And stepping on a Lego block is a 200... easily.

🌈 the numbered pain scale gives a lot of people pause and that tiktok doc is actually perpetuating it 😅😅 some people significantly underscore their pain because literally exactly "if you sawed my arm off it would hurt more, so this appendicitis is like a 5 i guess". i dont think a numbered scale needs to be used to be honest, we have so many words so just ask question/explain your symptoms via how they affect you

🏳️‍🌈 That comment you made about finding the sweet spot for sensory stimulation was really helpful for me as an autistic person. When I first realized I was autistic, I started experimenting with sensory input and I never knew what to do with the fact that I could struggle with hypersensitivity to sensory stimuli yet still have a reaction to the competing sources of stimulation I introduced to control it.

I absolutely hate the rate your pain question. Compared to what? I get there is no pain thermometer or anything, but it is so wildly subjective I don’t see how it could possibly be a reliable bit of information. Pain medication is something so many doctors fear giving out and I feel so conflicted whether I should ask for something or stay quiet so as I do not look like I’m trying to “score” pain medication. I have gone through multiple surgeries where I’m prescribed Tylenol for pain management. I’m just laying there thinking they just cut me open, removed cartilage, removed bone, cut some tendons, and you want me to go take some Tylenol? I guess I didn’t give them a high enough pain score.

ER, 10pm, patient in acute psychosis, order psych eval.....two hours later....psych calls, "whats wrong?" "patient is screaming about wanting to go to argentina, put their face through the front window of their house, and has a history of ptsd, bipolar, depression, anxiety, chronic pain, alcohol and opioid addiction, adhd, and suicide attempts." Psych, "Ok, I'll be there in a bit.".....Two hours later.....

1:58 I think the idea behind that is that the patient claims his pain in the neck is 10 out of 10 at the moment but he shows no sign of discomfort whatsoever; some people exaggerate the pain they are in maybe to get prescriptions or to get out of work or whatever. I don't think it's aimed at people who are in pain and are ignored by their doctors. I used to work at a private clinic and once a patient call to say she was in so much abdominal pain there had to be something wrong with her, we told her to come see the doctor and she walked in perfectly fine, she was perfectly able to walk, sit down, sit up, talk, ... it was some light discomfort and she wanted to be seen by a doctor on a Sunday for free (we didn't charge for emergency visits) she lied on the phone about her pain claiming it was the worst pain she had ever experienced.

As for SteveJoe poking fun at the patient, that is a difference in emergency medicine between the UK and the US. In the United States, the nurses are responsible for 99% of the care. Everything from requesting meds, plan of care, ordering tests, discharge orders and putting in IVs. A doctor will see a patient once a shift and then never again until the following rotation. SteveJoe is portraying an ER nurse doing triage where they see anything from homicides to the homeless who are having a pyschotic episode and forced to sit in the waiting room along with concussion protocols or chronic constipation. There is a huge problem of opiod abuse here in the United States and many come into the ER for scripts, not wanting actual treatment or a diagnosis. Addiction, like psychiatric issues, are not addressed well (if at all) in the US healthcare system. Empathy does not pay the bills I guess

Maybe not quite relevant to this video, but I was wondering if you’ve ever seen the bbc tv series cardiac arrest that aired in the 90s? I’m an ED fellow who just discovered it (free on KZbin starring Emily from friends among others) and I love it even though it’s quite dated in many respects, and I can’t find any reviews or analyses of the series from other doctors (which is apparently what I like to watch in my free time 😅) - I find it really interesting to see how much things have both changed and stayed the same in medicine since then!

I’ve never done a dermatology post in my life and once misidentified a wart a friend had as a BCC whilst at med school but my family constantly send me photos of their skin problems 😭 (Also 🏳️‍🌈 - wish there was a bisexual flag emoji to add to this!)

I have some mental health issues that I have to work through, and a friend who is an experienced mental health practitioner. He's a great part of my support network, but there is a boundary there. We haven't really spoken about it, but it's sort of mutually understood. I can be candid with him about things that I am dealing with, and he can give really good insight and advice about how to approach getting help through the NHS, but I have to stop short of trying to be a patient, and while he might use his skillset and knowledge of the NHS to support, he doesn't try to treat. I don't think the issue is exclusive to medical professionals. Anyone who is skilled in a particular area might get it. (Though I acknowledge that with medicine you add in a lot of ethical considerations). But whether it be a friend who is a doctor and you have a concern or a friend who works in IT and your laptop won't load properly, the question is are you looking for insight from a friend who knows more than you about something? Or are you trying to make them work for you for free?

It’s not that he’s making the patient the punch line. If you have to decide what pain medication to give your patient, and you have to decide what dose they need to take care of that pain, then you have to have a good idea of where their pain is at. You definitely get patients like this, and it can be very difficult to accurately judge. Nurses are supposed to take patients at their word, but not every patient is honest, or necessarily has great insight. As well, some patients are extremely stoic, and that can be confusing as well, you certainly don’t want to _under_-medicate you patient. I can confirm that a tablet can be a good way to take your mind off your pain. But its apparent that he’s attempting to clarify the patient’s actual level of pain compared to having his arm cut off with a chain saw. Nurses definitely do this, and it's not to scoff at the patient.
