Pilot Here. You absolutely give up any freedom to deny toxicology testing when you are flying commercially. This is written into almost any employment contract with an airline, failing that, in Canada where I fly, Transport Canada has the ability to perform a non invasive drug test at any time they wish, and an invasive (ie bloodwork) after a crash, especially when pilot error is suspect, though usually on every serious accident.

But technically yes, someone with a pilot's license could fly their own private plane and would be responsible for any liability, just like with a driver's license.

The thing why sully's plane survived was, he somehow managed to land so smoothly and slow that his engines did not act like a fricking brick wall. @@wecsam As a plane nerd, my guess is the plane had issue with ailerons - one of the flight control surfaces on the wings; they are responsible for the longitudal rotation and the plane was visibly rolling left and right. The injuries probably were so bad because the plane was in a left bank so the wing struck the water first - and many crash/emergency landing have been made so much worse because of wing strikes. Oh, the rudder (the flight control surface at the end of the vertical stabilizer that allows the plane turn without banking) must have not been working because I'd assume the pilots would try to use it to turn otherwise Or the human factor might have played a role....

@@rilmar2137 But the plane did a 90 degree turn just before impacting the water, so it would seem to me the rudder was working. I also think a sharp turn like that also require aileron inputs to keep the plane mostly level during and just after that sharp maneuver.

Outlander has a lot of good medical scenes that would be interesting to see you react to! The main character who has gone back in time often applies her knowledge of mid 20 th century medicine to mid 18th century patients. Really cool! She does use a lot of herbal remedies but there are lots of surgery scenes as well!!!

The cardio department has at least 4 doctors (3 women and Dr. Reynolds) and 2 residents, as shown in one of the episodes. Oncology has Dr. Castro. Neuro has Dr Agnes Kao, who is an attending physician even when she looks young enough to be a resident (and spent 2 seasons assisting Dr Kapoor). Apparently there is an ED resident too, though I don't remember her. That said, they aren't shown as much as the leading characters.

On the smelling gum part, I had swore I smelled gas at my neighbors house. She called the fire department and they ran tests and found nothing. I came back in and still smelled gas, so she called housing again and they sent a plumber specialized in gas leaks. Turns out I was right and there was a tiny gas leak in her stove. Some people have super sensitive sense of smell.

You might have saved her life. Gas is actually odorless, so they mix it with methane gas in order for most people to smell it, so that they know there is a leak.

I remember growing up, me and my mom used to watch ER together. And I remember there was one doctor who got his arm cut off by an propella from a helicopter. He survived. Only to later get crushed by a crashing helicopter, right outside of the hospital.. Always thought that was kinda insane, but I see now that it's pretty standard to have these kind of episodes in medical dramas. 😅

The aircraft depicted looks like a Boeing 737, those aircraft don't have anti freeze fluids. They make use of hot air from the engines to warm up certain systems, i.e the leading edge of the wing. So how the poor FO got anti freeze fluid in his system is rather interesting. Great video Dr. Mike, super informative! 😁

8:30 Pilot here. What they are describing is a stall recovery, not an inoperative airspeed indicator. The procedure would be to diable autopilot, turn on pitot heat and possibly alternate static source, and maintain the current attitude and power setting. Then consult the applicable checklist in the QRH. The plane is still flying even if the ASI shows otherwise. Stalls can be determined independently of airspeed indication. Declare an emergency as either a mayday or pan-pan depending on urgency, and return to the field. If the airspeed doesn't recover, continue flying the plane, and use your power and vertical speed to judge your airspeed. Verify this with GPS ground speed, and fly it to the nearest airport. Have emergency services present and deplane when it is safe to do so. If the pilot has gotten too slow, then there is a problem there. The tried and true method of attitude + power = performance will always hold. If you know your attitude and power, you can safely fly the plane, albeit with difficulty maintaining a specific airspeed. If you let ATC know, this would be the least of your worries.

You are 100% correct. An ER physician would never attempt to do an aortic repair like that. For one, they're likely not qualified as they don't specialize in vascular surgery. And secondly, the risk level is far too high for something to go wrong and opening a patient in the ER like that is incredibly dangerous for a number of reasons. They are always brought up to the OR for the operation.

Yep. Wings are attached to the fuselage very, very firmly. Just think about it: when a plane is flying, all of the lift (the force pushing it up) comes from the wings so they are designed to carry the entire weight of the plane Fun fact: there are fuel tanks in the wing to slightly reduce the weight that's hanging from them, optimalise the center of gravity and so that the plane is able to carry more fuel

My sister's mother in law (basically part of the family as she was very close to my mother and grandmother) had severe stabbing back pain and fainted. When she woke up, she was paralyzes from the waist down and dragged herself onto her bed thinking she herniated a disc. She took a nap for a few hours before then calling Donny and Kylee. They are the ones who called 911. At the hospital they couldn't figure out what happened. They did a CT scan and said it was normal and maybe she had sciatica but they weren't sure and just gave her painkillers and physical therapy for her LOWER BODY PARALYSIS OF UNKNOWN CAUSE! Then in the middle of the night my sister called me that Cathy's organs were failing and she was going into shock, so they flew her to a hospital that had a good cardiothoracic unit. She was then in ICU and comatose and only at this hospital did they discover she had an aortic dissection (which was apparently visible on the original CT but missed) and her lower body and limbs were all clotting and essentially dead. They determined then that she was brain dead (this was Thanksgiving Day 2018), and Donny and his uncle (Cathy's brother) and cousin were going the next day to remove her ventilator and allow her to pass. The incident happened on a Monday and the aortic dissection wasn't identified until Thursday!!! Meanwhile in those few days, my mom, grandma, and brother in law and numerous friends had visited her in the hospital and had lucid conversations. Finally on Friday after determination of brain death, Donny (her son/my BIL) was holding her hand and his cousin was holding her other hand waiting for her to pass. My mom was there too and serves as another witness to this next part...Donny said "I'm going to be ok, Ma," and she opened her eyes, looked at him and said "I love you son", and then took her final breath. Crazy that something so severe abd emergently life threatening was missed for DAYS. Crazy that for 3 days she was completely normal cognitively yet her peripheral vasculature was all shutting down like a corpse body. Crazy that as a doctor, I had to talk to my brother in law and his uncle about what the doctors were saying and aortic dissections and brain death on Thanksgiving. Crazy that she apparently "came back" to say goodbye to Donny. Since I do neuro rehab I see the survivors of some crazy things, but I've never heard of someone having an aortic dissection and survive for so long afterwards without any appropriate intervention. 💔

@@nataliepacker6998 It was a dissection but I don't know or remember any more details. We use triple A here too and unfortunately, my dad's side of the family has a history of them. He lives a much healthier lifestyle than they do, but I still want him to get checked.

I'm just relaying my experience. Any medical information was related to me by family members who do not have a healthcare background. My thought is that she was comatose ir some minimally conscious state as opposed to complete no brain activity. Additionally, neurons fire and reflexes go crazy in that situation of comatose patients and people dying. The point of the story was how she had an aortic dissection that was missed and it's crazy she lived/was conscious for so long with such an aortic injury that can kill within minutes.

The first few minutes of this episode were particularly emotionally brutal. It's the only medical drama that I know of that showed the damage to the healthcare workers' faces from the PPE. Other shows showed them using PPE, but not the bruising and rashes from all that PPE.

I'd love to see Dr Mike reacting to "Once upon a time... Life". Does anyone here know/remember that series? I think it might have not been aired in the US, which is actually a shame. It was a French animation, based on a similar premise as "Cells at Work!", but geared more towards children and educating them about the inner workings of the human body. It was released in 1987, so it is old(er that Dr Mike himself) and has this unmistakable retro charm. I was 13 at the time, perhaps on the older side of the intended audience, but still remember it as an absolute blast. There are some episodes on YT and if you check out the comments, people keep getting nostalgic and saying thngs like "yeah, I was watching that as a kid and it made me want to become a doctor, now I am one". So it really was a show that made a difference.

Because after each episode you gain more knowledge on the comprehensiveness of normal flying and emergency procedures, the engineering, and pilot training, and how they keep improving as time passes.

My father n law passed away late last year because he went in to the hospital by ambulance not feeling well complaining about back/chest pain and his leg was kind of numb and the hospital ruled it as pain from when he had hurt his back. He died 2 days later from an aortic dissection that could have been prevented had the hospital did their job and ran tests. I had reached out to you about this last year seeing if you wanted to talk to my mother n law and make a video to raise awareness about these issues but you never responded. Sad part about the whole thing is they were self contractors and without him she is now losing the business and their house. They did not have life insurance because due to her having cancer a few years prior they were not yet eligible for coverage. Very sad situation and the hospital is getting away with it because they put in his chart that is was pain from his back even though he clearly told them otherwise and my mother n law was NOT allowed in the hospital with him because of Covid.

Around 25% of americans delay treatment for serious medical conditions due to the high cost, according to the American Cancer Society around 56% of american experience financial hardship due to medical bills, and a study by harvard found that around 45000 people die every year in the US as direct result of not being able to cover the cost of treatment. The US spends more than any other country on healthcare while covering fewer people and achieve overall worse health outcomes. United States ranks 24th globally in achieving health goals according to UN guidelines. But ya know the US is the "best' most developed country in the world and stands as a shining example to all the other countries. The best part is that the government would SAVE money by implementing universal healthcare, but that is far to socialist of an idea to even be considered in a country where it's totally fine for large corporation to lobby against legislation that directly impacts the wellbeing of insignificant citizens like myself.

Aortic aneurisms are more likely to tear because the blood pressure in there is higher than in the rest of the vessel. My last exam of fluid dynamics, master degree of biomedical engineering. But yeah, thinner wall of the pipe may also be playing its part. That’s why the treatment for aneurisms in some cases is a sleeve stent inside, to cut out the flow into the enlargement.

I just came back to this and realized... ethylene glycol is used on the ground not in the air. Aircraft (commercial jets at least) don't use antifreeze in any situation I am aware of. They use low-temperature fuels, bleed air, and electric heaters.
